This study was formalized in accordance with resolution 466/12 of the National Committee for Research Ethics (CONEP), CAAE: 01586818.6.0000.5020. Participants in this study will complete and sign the Free and Informed Consent Form, demonstrating that they are aware of all procedures performed. The research was also registered in the Brazilian Registry of Clinical Trials (REBEC) and in clinicaltrial.gov. Due to the absence of invasive procedures, clinical research presents a low risk for patients with OA, and the benefits will be directed in their favor. This study was conducted in the Physiotherapy Laboratory of the Institute of Health and Biotechnology of the Federal University of Amazonas, in the city of Coari, Amazonas. This is a double-blind randomized clinical trial that followed the recommendations of the Consolidated Guidelines for Reporting Trials (CONSORT), recommendations for non-pharmacological trials. Participants in this study were randomly allocated to groups using Random Allocation software, version 2.0, and were distributed in a 1:1 ratio by a researcher not involved in the research. To achieve blinding in this study, patients who underwent treatment protocols with Low Level Laser Therapy (LLLT) or Placebo Therapy (PT) were deprived of information about whether the equipment was on or off, as were the evaluators who conducted the statistical analysis of the studied groups, who were also blinded to the study groups. The clinical trial samples were divided into 3 groups: control, placebo, and photobiomodulation (LLLT). The sample was distributed in a 1:1 ratio, randomly. The total number of participants was 73 individuals, allocated among the three intervention groups. The description of the intervention followed the recommendations of the "Template for Intervention Description and Replication" (TIDieR). Study participants were selected and recruited through screening at the outpatient clinic of ISB-UFAM, Specialized Rehabilitation Center, Coari Regional Hospital, and Basic Health Units. Individuals of both sexes, aged between 50 and 74 years, diagnosed with symptomatic knee osteoarthritis participated in this study. Participants presented a minimum pain intensity of 3 on the Visual Analog Scale (VAS). Participants met the classification criteria of the American College of Rheumatology, presenting: joint pain, functional alterations, age 50 years or older, knee crepitus, joint stiffness lasting an average of 30 minutes, and the presence of osteophytes identified by imaging tests. In addition to the aforementioned criteria, individuals with knee osteoarthritis presented knee pain and functional disability for at least 3 months, with a classification between 2 and 4 grades on the Kellgren–Lawrence scale. Individuals with the following complications were excluded from the study: neoplasms, dementia, neurological deficits (sensory or motor), uncontrolled systemic arterial hypertension, patients using antidepressants, or those with acute infections, vascular complications, epilepsy, gravid uterus, burns, as well as patients using pacemakers and patients using anti-inflammatory or analgesic medications, patients with symptomatic hip OA, acute diseases, other rheumatic, systemic or orthopedic diseases that could interfere with the results and performance of the tests. All these cases were removed from the study. The primary outcome measure, Visual Analog Scale (VAS) (range: 0–100 mm), was used to estimate the sample size. Using a minimum clinically important difference of 20 mm between groups, 20 mm SD, with a significance level of 0.05 (two-sided) and a power of 80%, we needed at least 17 participants in each group. The statistician was blinded to the groups, and patients were assessed using an intention-to-treat analysis, with missing values being imputed by the last observation carried forward method.

O presente estudo foi formalizado de acordo com a resolução 466/12 do Comitê Nacional de Ética em Pesquisa (CONEP), CAAE: 01586818.6.0000.5020. Os participantes deste estudo preencherão e assinarão o Termo de Consentimento Livre e Esclarecido, demonstrando que estão cientes de todos os procedimentos realizados. A pesquisa também foi registrada no Registro Brasileiro de Ensaios Clínicos (REBEC) e no clinicaltrial.gov. Devido à ausência de procedimentos invasivos, a pesquisa clínica apresenta baixo risco para os pacientes com OA, e os benefícios serão direcionados a seu favor. O presente estudo foi conduzido no laboratório de Fisioterapia do Instituto de Saúde e Biotecnologia da Universidade Federal do Amazonas, no município de Coari, Amazonas. Este é um ensaio clínico randomizado duplo-cego que seguiu as recomendações das Diretrizes Consolidadas para Relato de Ensaios (CONSORT), recomendações para ensaios não farmacológicos. Os participantes deste estudo foram alocados aleatoriamente nos grupos usando o software Random Allocation, versão 2.0, e foram distribuídos em uma proporção de 1:1 por um pesquisador não envolvido na pesquisa. Para alcançar o cegamento neste estudo, os pacientes que passaram pelos protocolos de tratamento com Terapia a Laser de Baixa Intensidade (LLLT) ou Terapia Placebo (PT) foram privados de informações sobre se o equipamento estava ligado ou desligado, assim como os avaliadores que conduziram a análise estatística dos grupos estudados, que também não tinham conhecimento dos grupos em estudo. As amostras do ensaio clínico foram divididas em 3 grupos: controle, placebo, fotobiomodulação (LBI). A amostra foi distribuída em uma proporção de 1:1, aleatoriamente. O número total de participantes foi de 73 indivíduos, alocados entre os três grupos de intervenção. A descrição da intervenção seguiu as recomendações do "Template for Intervention Description and Replication" (TIDieR). Os participantes do estudo foram selecionados e recrutados através de triagem na clínica ambulatorial do ISB-UFAM, Centro de Reabilitação Especializada, Hospital Regional de Coari e Unidades Básicas de Saúde. Indivíduos de ambos os sexos, com idades entre 50 e 74 anos, com diagnóstico de osteoartrite sintomática no joelho, participaram deste estudo. Os participantes apresentaram uma intensidade mínima de dor de 3 na Escala Visual Analógica (EVA). Os participantes atenderam aos critérios de classificação do American College of Rheumatology, apresentando: dor articular, alterações funcionais, idades a partir de 50 anos, crepitação no joelho, rigidez articular com duração média de 30 minutos e a presença de osteófitos identificados por exames de imagem. Além dos critérios mencionados, indivíduos com osteoartrite de joelho apresentaram dor no joelho e incapacidade funcional por pelo menos 3 meses, com uma classificação entre 2 e 4 graus na escala de Kellgren–Lawrence. Indivíduos com as seguintes complicações foram excluídos do estudo: neoplasias, demência, déficits neurológicos (sensitivos ou motores), hipertensão arterial sistêmica não controlada, pacientes em uso de antidepressivos, ou aqueles com infecções agudas, complicações vasculares, epilepsia, útero gravídico, queimaduras, bem como pacientes em uso de marcapassos e pacientes em uso de medicamentos anti-inflamatórios ou analgésicos, pacientes com OA sintomática de quadril, doenças agudas, outras doenças reumáticas, sistêmicas ou ortopédicas que pudessem interferir nos resultados e na realização dos testes. Todos esses casos foram removidos do estudo. A medida de resultado primário, Escala Visual Analógica (EVA) (faixa: 0–100 mm), foi usada para estimar o tamanho da amostra. Usando uma diferença clinicamente importante mínima de 20 mm entre os grupos, 20 mm de DP, com um nível de significância de 0,05 (bilateral) e um poder de 80%, precisávamos de pelo menos 17 participantes em cada grupo. O estatístico não tinha conhecimento dos grupos, e os pacientes foram avaliados através de uma análise por intenção de tratar, com valores ausentes sendo atribuídos pelo método de última observação levada adiante.

The study sample consisted of a total of 73 participants of both sexes, with 83% female and 17% male in the photobiomodulation group and 85% female and 15% male in the placebo and control groups. Of these 73 participants, 8 were excluded for dropping out of the study. The studied group presented the following anthropometric characteristics: average age of 61.64±7.74 years (photobiomodulation), 62.68±7.42 years (placebo), 60.48±5.48 years (control), average height of 1.58±0.07 m (photobiomodulation), 1.56±0.08 m (placebo), and 1.57±0.05 m (control), body mass of 77.60±12.42 kg (photobiomodulation), 76.2±11.98 kg (placebo), 77.20±13.22 kg (control), body mass index of 30.84±3.84 kg/m² (photobiomodulation), 29.3±4.25 kg/m² (placebo), 28.3±3.95 kg/m² (control). Among the three groups of participants in the knee osteoarthritis study, the photobiomodulation group exhibited the following characteristics: osteoarthritis grade II, 2 (10%) participants, osteoarthritis grade III, 16 (76%) participants, and osteoarthritis grade IV, 3 (14%) participants. The study began with the evaluation of 77 participants for eligibility. Of these, 4 were excluded: 3 because they did not meet the inclusion criteria and 1 because they refused to participate. The remaining 73 participants were allocated into three groups: photobiomodulation (LLLT), placebo, and control, each with 24 participants. All 24 participants assigned to photobiomodulation received the intervention, as did all 24 assigned to the placebo. The control group did not receive any active intervention. After 10 weeks of follow-up, there were no losses in follow-up in any of the groups. However, 3 participants in the photobiomodulation group and 2 in the placebo group discontinued the intervention, although the reasons for these discontinuations were not specified. The control group had no reported interruptions. In the analysis phase, 21 participants from the photobiomodulation group, 22 from the placebo group, and 22 from the control group were analyzed.

Regarding pain intensity, a significant difference was observed in the intragroup analysis in the photobiomodulation groups (pre and post) with p < 0.001, and in the placebo group with p < 0.05. In the control group, there was no significant difference after 10 weeks. In the between-group analysis, the photobiomodulation group (post-therapy) showed a significant reduction in pain (p < 0.05) compared to the placebo group (post). These data were graphically represented, showing the pain intensity in the three groups before and after treatment, with values expressed as mean ± standard deviation. In the WOMAC, which assesses pain, stiffness, and functional limitations, there was a significant improvement in the photobiomodulation group (pre and post) with p < 0.05. In the other groups, there was no significant difference (p > 0.05). These results are illustrated. The KOOS questionnaire, which assesses six domains (symptoms, stiffness, pain, activities of daily living, sports activity, and quality of life), showed significant differences in several domains. In the symptom domain, there was a significant improvement in the photobiomodulation and placebo groups (p < 0.05). In the domain of stiffness, the difference was significant in the photobiomodulation group (p < 0.01) and in the placebo group (p < 0.05). In the pain domain, there was a significant improvement in the photobiomodulation and placebo groups (p < 0.01). In the domain of activities of daily living, only the photobiomodulation group showed significant improvement (p < 0.01). In the domain of sports activity, there was no significant difference between the groups. In the domain of quality of life, there was a significant difference in the photobiomodulation group (p < 0.001). These data are detailed. In the Timed Up and Go (TUG) test, there was a significant difference in the intragroup analysis in the photobiomodulation group (pre and post) with p < 0.05. In the placebo and control groups, there was no significant difference. These results are represented. The temperature analysis using a thermographic camera did not show significant differences (p > 0.05) between the photobiomodulation and placebo groups (pre and post). Similarly, there was no statistically significant difference in the control group after 10 weeks. These data are presented, with temperature values in degrees Celsius and standard deviation. These outcome measures, including significance levels and dispersion estimates, were fundamental for evaluating the effectiveness of the interventions and comparing the results between the studied groups.

This study aimed to evaluate the effects of photobiomodulation (low-intensity laser therapy) in patients with symptomatic knee osteoarthritis. A double-blind, randomized clinical trial was conducted, adhering to the Consolidated Standards of Reporting Trials (CONSORT) guidelines for non-pharmacological interventions. Following the WALT recommendations, the photobiomodulation group adjusted the device to a wavelength of 790 nm and a dose of 4 joules per point. The treatment involved the irradiation of nine specific points: four in the medial region and five in the lateral region of the knee. To assess pain and disability, the following variables were used: VAS (Visual Analog Scale), WOMAC (Western Ontario and McMaster Universities Osteoarthritis Index), KOOS (Knee Injury and Osteoarthritis Outcome Score), TUG (Timed Up and Go), and infrared thermography. A total of 65 individuals participated in the study, divided into two groups: Photobiomodulation Group (n=21), Placebo Group (n=22) with the equipment turned off, and Control Group (n=22) with a 10-week follow-up without any type of intervention. In the photobiomodulation group, 83% of the participants were female and 17% were male, while the control and simulation groups had 85% female participants and 15% male participants. The results showed a significant reduction in pain (p< 0.05) in the photobiomodulation group compared to the placebo group (post-therapy). Pain, stiffness, and functional limitations showed significant improvements in the photobiomodulation group (pre and post-therapy) (p<0.05) compared to the other groups. These results show that low-level laser therapy (790 nm, 120 mW, 4 J/point) effectively reduces pain, improves daily activities and quality of life, and leads to better clinical outcomes in knee osteoarthritis (WOMAC). However, we did not observe significant changes in skin temperature before or after photobiomodulation.

Este estudo teve como objetivo avaliar os efeitos da fotobiomodulação (terapia a laser de baixo intensidade) em pacientes com osteoartrite sintomática do joelho. Foi realizado um ensaio clínico randomizado, duplo-cego, aderindo às diretrizes das Normas Consolidadas de Relato de Ensaios (CONSORT) para intervenções não farmacológicas. Seguindo as recomendações da WALT, o grupo de fotobiomodulação ajustou o dispositivo a com comprimento de onda de 790 nm e dose de 4 joules por ponto. O tratamento envolveu a irradiação de nove pontos específicos: quatro na região medial e cinco na região lateral do joelho. Para avaliar a dor e incapacidade utilizou-se as variáveis: EVA (Escala Visual Analógica), WOMAC (Western Ontario and McMaster Universities Osteoarthritis Index), KOOS (Knee Injury and Osteoarthritis Outcome Score), TUG (Timed Up and Go) e a termografia infravermelho. Participaram do estudo 65 indivíduos, divididos em dois grupos: Grupo fotobiomodulação (n=21) , Grupo Placebo (n=22) com o equipamento desligado e Grupo Controle (n=22) com o acompanhamento de 10 semanas sem nenhum tipo de intervenção. No grupo de fotobiomodulação, 83% dos participantes eram do sexo feminino e 17% do sexo masculino, enquanto os grupos de controle e simulação tinham 85% de participantes do sexo feminino e 15% do sexo masculino. Os resultados mostraram uma redução significativa na dor (p< 0,05) no grupo de fotobiomodulação em comparação com o grupo placebo (pós-terapia). A dor, rigidez e limitações funcionais, revelou melhorias significativas no grupo de fotobiomodulação (pré e pós-terapia) (p<0,05) em comparação com os outros grupos . Esses resultados mostram que a terapia a laser de baixo nível (790 nm, 120 mW, 4 J/ponto), reduz efetivamente a dor), melhora as atividades diárias e a qualidade de vida e leva a melhores resultados clínicos na osteoartrite do joelho (WOMAC). No entanto, não observamos mudanças significativas na temperatura da pele antes ou depois da fotobiomodulação.
