The masking will be of the "triple-blind" type. Experimental group: 15 patients will undergo TAP block using a ropivacaine solution. Control group: 15 patients will undergo TAP block using a saline solution. Randomization is performed using sealed envelopes previously prepared with the sealedenvelope.com tool. The preparation of the solution to be applied is done just before its use by a person who will not be part of the surgical team of the patient in question, so it is not possible for the surgeon to know the solution being used (saline or ropivacaine). This intervention is performed only once, in a timely manner. The TAP block technique consists of the bilateral infusion of 10 mL of solution at (1) the mid-axillary line at the midpoint between the iliac crest and the costal margin; and 10 mL of solution (2) one centimeter below the costal margin, at the anterior axillary line (total volume 40mL). This procedure will be performed by the surgeon at the beginning of the surgical procedure, as soon as laparoscopy is performed, and guided by it. For the puncture, a 16G Jelco or BBraun Stimuplex® A bevel 30° 22G 50mm (or 100mm, if necessary, especially in more obese patients) needle will be used at the surgeon's discretion, which will be inserted at the puncture points and introduced to the subperitoneal space, then withdrawn until the needle tip is no longer visible. A test infusion of 0.5 mL allows verification of the bulging of the transverse abdominal muscle and creation of the "Doyle bulge," confirming the correct positioning of the needle tip and proceeding with the infusion of the indicated volume. In patients under 50kg, a maximum dose of 3mg/kg of ropivacaine will be considered, with a proportional reduction in the volume infused among the 4 puncture points. All patients will receive a fixed postoperative analgesia regimen with IV dipyrone and IV tramadol as requested (on demand). The outcome assessment is performed by a person who is not aware of which solution was used in the TAP block, characterizing a triple-blind study.

O mascaramento será do tipo "triplo cego". Grupo experimental: 15 pacientes serão submetidos ao TAP block com o uso da solução de ropivacaína. Grupo controle: 15 pacientes serão submetidos ao TAP block com o uso da solução de salina. A randomização é realizada com uso de envelope fechado previamente confeccionado com o uso da ferramenta sealedenvelope.com. O preparo da solução a ser aplicada é realizada logo antes do seu uso, por pessoa que não fará parte da equipe cirúrgica do paciente em questão, de modo que não é possível o cirurgião saber a solução que está sendo utilizada (salina ou ropivacaína). Tal intervenção é realizada uma única vez, de forma pontual. A técnica do TAP block consiste na infusão bilateral de 10 mL de solução na (1) linha axilar média em ponto médio entre crista ilíaca e o rebordo costal; e 10 mL de solução (2) um centímetro abaixo do rebordo costal, na linha axilar anterior (volume total 40mL). Tal procedimento será realizado pelo médico cirurgião no início do procedimento cirúrgico, assim que realizada a laparoscopia, e sendo guiado por ela. Para a punção, será utilizado um jelco 16G ou agulha BBraun Stimuplex® A bevel 30° 22G 50mm (ou 100mm, se necessário, especialmente nos pacientes mais obesos), a critério do cirurgião, que será inserida nos pontos de punção e introduzida até o espaço subperitoneal, sendo em seguida regredida até a não visualização da ponta da agulha. Infusão de teste de 0.5 mL permite verificar o abaulamento do músculo transverso abdominal e criação da “protuberância de Doyle”, confirmando o correto posicionamento da ponta da agulha e procedendo-se com a infusão do volume indicado. Em pacientes abaixo de 50kg, será considerada dose máxima de 3mg/kg de ropivacaína, com redução proporcional no volume infundido entre os 4 pontos de punção. Todos os pacientes irão receber esquema de analgesia fixa no pós-operatório com dipirona EV e tramadol EV conforme solicitado (sob demanda). A avaliação dos desfechos é realizada por pessoa que não possui ciência de qual solução foi utilizada no TAP block, caracterizando um estudo triplo cego.

Regarding sociodemographic variables, no statistical difference was found between the groups (p>0.05). The majority of the sample was male (55.2%), married (65.5%), with technical or higher education (61.6%), and a median age of 60 years. The first quartile, 50 years, indicates that 25% of the sample is under 50 years old. The third quartile, 67 years, indicates that 25% of the sample is over 67 years old. Concerning comorbidities presented by the patients, there was no statistical difference (p>0.05) in the prevalence of any comorbidity between the groups. Overall, the most prevalent comorbidity was hypertension (37.9%), followed by obesity (13.8%). Regarding the underlying disease that led to surgical indication, there was also no statistical difference between the groups (p=0.373). The majority (89.7%) of the patients had colorectal adenocarcinoma.

Thirty-three patients were assessed for eligibility, with three excluded due to the presence and treatment of chronic pain in the lower limbs (two patients) and the proposal for concomitant resection of other organs (one patient). Thirty patients were randomized, with 14 of them at the IPSEMG hospital and the remaining 16 at Felício Rocho Hospital. All patients received the proposed interventions. One patient at IPSEMG hospital developed a suspected allergy to dipyrone in the immediate postoperative period and was excluded from the statistical analysis due to the protocol violation regarding the predetermined frequency of dipyrone use, which could introduce potential bias from the absence of this medication in pain levels. Later, it was identified that this patient was part of the placebo group. Thus, 29 cases were evaluated, with 14 using saline solution (control group) and 15 using ropivacaine solution (intervention group).

Primary Outcome: The resting pain scale at 24 hours had a median of 3 points (Q1=2; Q3=5) in the aggregate data. When evaluated within each group, the median in the control group was 4 points, while in the intervention group it was 2 points. Although the absolute difference of 2 points is considered clinically significant, from a statistical point of view, however, there was no difference between the two groups (p = 0.252).
Secondary Outcomes: The pain scale when coughing at 24 hours had a median of 4 points (Q1=2; Q3=6) in the aggregate data, while the resting pain scale at 48 hours had a median of 2 points (Q1=0; Q3=4), with a median of 3 points (Q1=1; Q3=5) when coughing. No statistical difference was found in these evaluations either (p=0.235). Regarding the use of opioids, the majority of patients did not request them postoperatively (58.6%). Among those who did use opioids, the dosage of the opioid used (which consisted of morphine or tramadol) was converted to morphine equivalents (DOWELL, 2022). When considering all patients, the median dose of opioids in morphine equivalents within the first 24 hours was 20 mg (Q1=7; Q3=24), with 22 mg (Q1=9; Q3=33) in the control group and 10 mg (Q1=6; Q3=20) in the intervention group, with no statistically significant difference between the groups (p=0.608).
Nausea or vomiting was not reported by the majority of patients (62.1%) in the first 24 hours postoperatively. At 48 hours, 86.2% of patients did not report nausea or vomiting. No statistically significant difference was found either (p=0.451).
Regarding postoperative complications, the majority of patients (57.7%) did not have any type of postoperative complication. Grade I complications (CLAVIEN, 2009) occurred in 26.9% of patients, predominantly represented by nausea/vomiting and pain. Grade II complications (CLAVIEN, 2009) occurred in 4 patients (15.4% of the total), with 3 patients in the control group (2 cases of parenteral nutrition use and 1 case of conservatively treated enteric fistula) and 1 patient in the intervention group (parenteral nutrition use). There were no grade III, IV, or V complications (CLAVIEN, 2009). No statistically significant difference was found either (p=0.450). The total hospital stay (in hours) had a median of 96 hours (Q1=96; Q3=126). No statistically significant difference was found (p=0.322). The overall satisfaction reported by patients was 10, except in 2 cases: one control patient who scored 9 and one intervention group patient who scored 8. No statistically significant difference was found (p=0.982). The median duration of the TAP block was 3 minutes, with 25% of the sample (Q1) having a duration of less than 2 minutes and 25% (Q3) having a duration of more than 4 minutes. No statistically significant difference was found (p=0.928).
The median duration of the surgery was 241 minutes, with 25% of the sample (Q1) having a surgery duration of less than 209 minutes and 25% of the sample (Q3) having a duration greater than 287 minutes. No statistically significant difference was found (p=0.094).
When comparing the median duration of the TAP block (3 minutes) with the median duration of the surgery (241 minutes), it was identified that the duration of this procedure corresponds to 1.24% of the total surgical time. Just over a quarter of the patients (27.6%) were sent to the postoperative intensive care unit, based on a joint decision by the surgical and anesthesiology teams. No statistically significant difference was found (p=0.344). Regarding ICU stay (in hours), the median value was 24 hours, with 25% of patients (Q1) staying less than 14 hours, while another 25% (Q3) stayed more than 47 hours in the ICU. No statistically significant difference was found (p=0.071). The diet was well tolerated in 96.2% of the cases. No statistically significant difference was found (p=0.271). Regarding the introduction of the diet (in hours), the median value was 14 hours, with 25% of patients (Q1) tolerating the diet reintroduction before 10 hours postoperatively, while another 25% (Q3) resumed the diet after 24 hours. No statistically significant difference was found (p=0.999). The median time to ambulation was 24 hours, ranging from 18 hours (Q1) to 24 hours (Q3). Dentre aqueles que fizeram uso de opioide, convertemos a dosagem do opioide utilizado (que consistiu em morfina ou tramadol) para equivalentes de morfina (DOWELL, 2022). Quando considerados todos os pacientes, o valor mediano da dose de opioide em equivalentes de morfina nas primeiras 24 horas foi de 20 mg (Q1=7; Q3=24), sendo que no grupo controle foi de 22 mg (Q1=9; Q3=33) e no grupo intervenção foi de 10 mg (Q1=6; Q3=20), sem se identificar diferença estatisticamente significativa entre os grupos (p=0,608).

Introduction: This study addresses the search for safe and effective alternatives in managing postoperative pain in colorectal surgeries. In particular, the research explores laparoscopically guided transversus abdominis plane (TAP) block as a method to improve patient recovery, focusing on reducing opioid use, which could accelerate the return of bowel function and allow for earlier hospital discharge.
Objectives: The study aimed to evaluate the analgesic efficacy of laparoscopically guided TAP block in patients undergoing laparoscopic colorectal surgeries, the feasibility of the block by surgical teams from different hospitals, and the impact of this intervention on patients' perception of treatment quality.
Methods: A randomized placebo-controlled clinical trial was conducted with 30 patients undergoing laparoscopic colorectal resections in two hospitals, with the intervention group receiving TAP block with ropivacaine and the control group receiving TAP block with saline solution. All patients received predetermined doses of dipyrone for analgesia and rescue opioids. Postoperative pain was assessed using the visual analog scale (VAS) at different time points after surgery. The primary outcome was pain at rest 24 hours after surgery. Secondary outcomes included pain at 24 hours when coughing, pain at 48 hours at rest and when coughing, nausea and vomiting, opioid dosage in the first 24 hours, time to ambulation, time to tolerate the first diet, surgical complications, length of hospital stay, and treatment satisfaction.
Results: There was no statistically significant difference between any of the evaluated outcomes. There were no complications or adverse events related to the use of TAP block. The block was feasible and easy to learn for the assisting surgical teams. Treatment satisfaction was similar between the groups.
Conclusion: Laparoscopically guided TAP block proved to be a viable and safe alternative in laparoscopic colorectal resections. Although statistically significant efficacy was not identified in this study, further research is necessary to assess the true effectiveness of the technique and to guide future recommendations in major guidelines on the subject.pt-br
Introdução: Este estudo aborda a busca por alternativas seguras e eficazes no manejo da dor pós-operatória em cirurgias colorretais. Em particular, a pesquisa explora o bloqueio do plano transverso abdominal (TAP block) guiado por laparoscopia como método para melhorar a recuperação dos pacientes, com foco na redução do uso de opioides, que poderia acelerar o retorno das funções intestinais e permitiria uma alta hospitalar mais precoce.
Objetivos: O estudo teve como objetivos avaliar a eficácia analgésica do TAP block guiado por laparoscopia em pacientes submetidos a cirurgias colorretais laparoscópicas, a exequibilidade do bloqueio pelas equipes cirúrgicas de diferentes hospitais, e o impacto dessa intervenção na percepção de qualidade do tratamento pelos pacientes.
Métodos: Foi realizado um ensaio clínico randomizado com placebo com 30 pacientes que se submeteram a ressecções colorretais laparoscópicas em dois hospitais, tendo como grupo intervenção a realização do TAP block com ropivacaína e o grupo controle o TAP block com solução salina. Todos os pacientes receberam analgesia com dipirona em doses pré-determinadas e opioides de resgate. A dor pós-operatória foi avaliada utilizando a escala visual analógica (EVA) em diferentes momentos após a cirurgia. O desfecho primário foi a dor em repouso 24 horas após o fim da cirurgia. Avaliou-se como desfechos secundários a dor em 24h ao tossir, dor em 48h em repouso e ao tossir, náuseas e vômitos, dose de opióides nas primeiras 24h, tempo para deambulação, tempo para tolerar dieta pela primeira vez, complicações cirúrgicas, tempo de internação e a satisfação com o tratamento.
Resultados: Não houve diferença estatisticamente significativa entre nenhum dos desfechos avaliados. Não houveram complicações ou eventos adversos relacionados ao uso do TAP block. O bloqueio foi exequível e de fácil aprendizado pelas equipes cirúrgicas assistentes. A satisfação com o tratamento entre os grupos foi semelhante.
Conclusão: A realização do TAP block guiado por laparoscopia se mostrou uma alternativa viável e segura nas ressecções colorretais laparoscópicas. Apesar e não ter sido identificada eficácia estatisticamente significativa neste trabalho, mais estudos são necessários para avaliar a real eficácia da técnica e direcionar as futuras recomendações dos principais guidelines do tema.
