This is a randomized controlled two-arm clinical trial, double-blind, and an interventional and prospective study conducted at the Orthopedics and Traumatology Service of the Armed Forces Hospital (HFA). Fifty-six patients aged 60 to 79 years, diagnosed with knee osteoarthritis (OA), for whom ESWT is not contraindicated, will be recruited. The sampling will be by convenience, obtained through non-probabilistic sampling, according to the demand served. The purpose of the proposed study is to evaluate the exposure factors and causes in the onset and clinical outcome of knee OA. Controls will be conducted both concurrently (patients treated at the same time) and through historical analysis (obtained from medical records). Participants should have the same opportunity to receive or not receive the proposed intervention, and these groups should be as similar as possible, so that the only difference between them is the intervention itself, allowing the impact on the outcome in one group to be evaluated against the other. This is the standard of excellence in studies aiming to assess the effect of an intervention on the course of a clinical situation. It helps eliminate various biases, as the intervention and control groups are allocated using random techniques, and characteristics are distributed similarly among the groups. An Identification Form will be prepared by the researchers, containing personal, sociodemographic, anthropometric, and clinical data. An electronic spreadsheet in Microsoft Excel®, version 2016, will be created, containing the following variables: medical record number, age, marital status, comorbidities, medications for continuous use, allergies, restrictions to any analgesic, date of study admission, and treatment regimen. The data extracted from the Visual Analog Scale (VAS) assessment applied at three time points—before the start (T0), at the end (T1), and one month after the end of the therapeutic plan (T2)—will also be organized in an electronic spreadsheet. Additionally, the results observed from the improvement of OA symptoms, obtained through the WOMAC (Western Ontario and McMaster Universities Osteoarthritis Index), will also be presented. Cases of OAJ observed in the study will be evaluated weekly over 5 weeks of follow-up, during which dates, session number, pain level at that moment, and any reported adverse events will be recorded, along with the treatment completion date or possible discontinuation, and all will be reassessed again after one month following the same evaluation criteria. All included patients will undergo a 5-week therapeutic plan. At T0, the patient will participate in a draw with two balls, one blue and one red, with the red indicating the control group, which will undergo the procedure with the device disconnected from the gun, and the sound will be reproduced by the cell phone speaker attached to the equipment; the blue indicates the execution of the actual treatment with ESWT. The sessions will be carried out weekly with F-ESWT (3,000 pulses; up to 0.15 mJ/mm²). If, during the research, the therapeutic procedures used in the intervention group prove to be efficient and effective, the participants in the control group will begin receiving the same treatment until the end of the study

Trata-se de um estudo clínico randomizado controlado de dois braços, duplo-cego, e um estudo intervencionista e prospectivo realizados no Serviço de Ortopedia e Traumatologia do Hospital das Forças Armadas (HFA). Serão recrutados 56 pacientes na faixa etária de 60 a 79 anos, com diagnóstico de OAJ, para os quais a ESWT não seja contraindicada. A amostragem será por conveniência, obtida por amostragem não probabilística, conforme a demanda atendida. O tipo de estudo proposto tem como finalidade avaliar os fatores de exposição e causas no aparecimento e desfecho clínico da OAJ. Os controles se darão de forma contemporânea (pacientes tratados ao mesmo tempo) e sob análise histórica (obtida de registros médicos). Os participantes devem ter a mesma oportunidade de receber, ou não, a intervenção proposta e esses grupos devem ser os mais parecidos possíveis, de forma que a única diferença entre eles seja a intervenção em si, podendo-se, assim, avaliar o impacto na ocorrência do desfecho em um grupo sobre o outro. É o padrão de excelência em estudos que pretendem avaliar o efeito de uma intervenção no curso de uma situação clínica. Permite eliminar diversos vieses, pois os grupos intervenção e controle são alocados usando técnicas aleatórias, e as características são distribuídas de um modo semelhante entre os grupos. Será elaborada Ficha de Identificação pelos pesquisadores, contendo dados pessoais, sociodemográficos, antropométricos e clínicos. Será elaborada uma planilha eletrônica do Microsoft Excel®, versão 2016, contendo as seguintes variáveis: número do prontuário, idade, estado civil, comorbidades, medicamentos de uso contínuo, alergias, restrições a algum analgésico, data de admissão ao estudo, regime de tratamento. Serão também dispostos em planilha eletrônica os dados extraídos da avaliação da Escala Visual Analógica (EVA) aplicada nos três momentos: antes do início (T0), ao término (T1) e após um mês do término do plano terapêutico (T2). E serão dispostos também os resultados observados pela melhora dos sintomas da OAJ obtidos por meio do Índice WOMAC (Western Ontario and McMaster Universities Osteoarthritis Index). Os casos de OAJ observados no estudo serão avaliados semanalmente por 5 semanas de acompanhamento quando serão registradas datas, número da sessão, nível de dor naquele momento e eventos adversos relatados, data de conclusão do tratamento ou de eventual abandono desse e todos reavaliados novamente com um mês seguindo os mesmos critérios de avaliação. Todos os pacientes incluídos serão submetidos a um plano terapêutico de 5 semanas de duração, no momento da T0 o paciente irá realizar um sorteio no qual terão duas bolinhas, uma azul e uma vermelha, sendo a vermelha indicada ao grupo controle, no qual irá realizar o procedimento com aparelho desconectado da pistola e o barulho será reproduzido pela caixa de som do celular acoplado ao equipamento; e a azul será a indicação para realização do tratamento efetivo com ESWT. As sessões serão realizadas semanalmente de F-ESWT (3.000 pulsos; até 0,15mJ/mm2. Se, no decorrer da pesquisa, os procedimentos terapêuticos utilizados no grupo de intervenção se mostrarem eficientes e eficazes, os participantes do grupo controle passarão a receber o mesmo tratamento até a finalização da pesquisa

The sample was constructed using the 2010 Consort flowchart, with 29 assessed for eligibility, and 9 excluded for not meeting the inclusion criteria (n = 7) and withdrawing from participation (n = 2). In total, 20 were randomized, with 10 allocated to the intervention group and 10 allocated to the control group. Intragroup Analysis of WOMAC Scores (Paired t Tests) - The results revealed that the GI showed statistically significant differences (p < 0.05) in all assessed domains (Pain, Stiffness, and Physical Function) across the three time comparisons (Pre (T0) vs. Post (T1), Post (T1) vs. 30 days (T2), and Pre (T0) vs. 30 days (T2)). These findings indicate that the intervention with TOCE promoted an immediate and sustained clinical improvement of symptoms related to knee osteoarthritis. It is noteworthy that the lowest p-values were observed in the Pre (T0) vs. 30 days (T2) comparison, reinforcing the evidence that the therapeutic effect was maintained, or even enhanced, over time. In the GC group, no statistically significant differences were observed in the Pre (T0) vs. Post (T1) comparison in the domains of Pain (p = 0.3436) and Stiffness (p = 0.4951), suggesting that the response to conventional treatment was less immediate. However, in the comparisons between Post (T1) vs. 30 days (T2) and Pre (T0) vs. 30 days (T2), statistically significant improvements in physical function were observed, suggesting that the therapeutic effects of conventional treatment manifest more progressively, consolidating after the end of the protocol. In all domains, there is a progressive reduction in WOMAC scores over time for both groups. In the Pain domain, a marked difference stands out at the Post (T1) moment, with Group GI showing a lower mean score. From 30 days (T2) onward, the mean scores of both groups become visibly similar, supporting the hypothesis of therapeutic convergence. Overall, the findings indicate that both groups showed improvement in the symptoms assessed by the WOMAC index. However, the dynamics of the clinical response differ between treatments. TOCE demonstrated a quicker onset of therapeutic effect, while conventional treatment proved to be effective in the medium term, especially from the 30-day mark. Comparative Analysis Between Groups – Independent t Tests – The comparison between the GC and GI groups was conducted using independent t tests, aiming to evaluate possible differences between WOMAC scores in three domains – Pain, Stiffness, and Physical Function – at three points in the study: pre-intervention, immediate post-intervention, and 30 days after the end of the intervention. The scores of the GI Group were significantly higher at the beginning of the study in the domains of Pain (p = 0.0328) and Stiffness (p = 0.0314), indicating a more severe initial clinical condition compared to the GC Group. At the immediate post-intervention moment (T1), the GI Group showed a significantly lower average Pain score in relation to the GC Group (p = 0.0259), suggesting a more pronounced and earlier clinical response to TOCE. However, in the assessment conducted 30 days after the intervention (T2), no statistically significant differences were observed between the groups in any of the domains (p > 0.05), indicating that the therapeutic effects leveled out over time. The data indicate that, although Group GI started the study in a worse clinical condition, the TOCE intervention resulted in faster improvement, especially in pain relief. At the end of 30 days (T2), both groups reached similar levels of functional and symptomatic improvement, suggesting comparable efficacy in the medium term. To assess differences in the proportion of responder patients between the GC and GI groups, proportion tests (z-test) were performed for each domain of the WOMAC index at the immediate post-intervention (T1) and 30 days after the intervention (T2). Pain: After treatment, there was a tendency for a higher proportion of responders in the GI Group (73.3%) compared to the GC Group (38.5%), with p = 0.0629 (significance threshold). At 30 days, this difference became statistically significant (p = 0.0203), indicating superior efficacy of the GI Group in reducing assessed knee pain. Stiffness: The proportion of responders was consistently higher in the GI Group at both time points, with a significant difference at 30 days (p = 0.0490). Physical Function: The GI Group had a significantly higher proportion of responders already at the Post (T1) time point (86.7% vs. 30.8%; p = 0.0025), and, as the results remained unchanged, the same rate persisted at T2, suggesting an earlier and more effective functional improvement. The z-test results reinforce the previous findings that TOCE (GI Group) promoted a higher proportion of patients with a significant clinical response, especially in the domains of Physical Function and Pain. With the aim of identifying the functional and symptomatic aspects most impacted by the therapeutic intervention, an item-by-item analysis of the WOMAC index was performed, calculating the mean absolute change between pre- and immediate post-intervention scores for each individual item of the instrument. The five items with the greatest mean score reduction in absolute change are presented, reflecting the areas in which participants reported the most symptom relief or functional improvement. The items are coded according to the WOMAC section (A – Physical Function), followed by the identification of the knee assessed (LE: left knee; RE: right knee), in relation to patients numbered from both groups. Main findings: Item A6_RE showed the greatest mean absolute reduction, with Δ = 3.25 points. Item A11_RE also demonstrated a clinically relevant response (Δ = 3.17). Three of the five items with the greatest improvement involve the right knee. Item A13_JE (Δ = 3.04) also indicates clinical response in the left knee. Clinical interpretation: Item-level analysis complements domain- and group-level analyses by allowing a detailed and specific visualization of therapeutic effects. Items with the largest absolute variation may indicate activities most affected by osteoarthritis, with greater potential for functional recovery with appropriate treatment. Key observations: A6_JD and A11_JD were the items with the highest mean variation (Δ = 3.25 and Δ = 3.17, respectively), reflecting substantial improvement in the physical function of the right knee. Item A13_JE (Δ = 3.04) also stands out, indicating that the left knee showed significant improvement. The graph highlights that the greatest improvements occurred mainly in functional activities such as walking, standing up, climbing stairs, or performing daily tasks – all covered by section A of the WOMAC.The predominance of right knee (JD) items among the top 10 may indicate greater initial functional impairment on this side or greater responsiveness to intervention. Clinical importance: Item-by-item analysis allows for a granular view of therapeutic effects, helping to identify which daily activities have benefited most from treatment. This information can guide physiotherapists and physicians to personalize rehabilitation strategies focusing on the aspects with the greatest functional impact on the patient. Clinical Pain Classification (VAS Scale). Statistical analysis. A statistical analysis of data from the VAS Pain Scale, complementary to the WOMAC Scale, used to assess subjective pain in patients with knee osteoarthritis (KOA), is presented. Comparison Between Groups at Time T2. The exact version of the Fisher test was applied to assess differences in the proportion of patients with mild pain between the groups at T2 time. The data were simplified into two categories: Mild Pain vs Non-Mild Pain (Moderate or Severe). Although Group GI had 100% of patients with mild pain, the difference between the groups was not statistically significant (p = 0.217), likely due to the small sample size. Intra-Group Evolution (T0 → T2): The McNemar test (exact) was applied to assess the significance of clinical improvement within each group between times T0 and T2, considering the transition from Non-Mild pain to Mild pain for JE and JD. Both groups showed statistically significant improvement. However, Group GI reached 100% mild pain at T2, while Group GC still had 25% of patients with moderate pain. There was a complete reduction of severe and moderate pain throughout the treatment. At T0, more than 90% of patients had severe or moderate pain. By T2, 100% of patients were classified as having mild pain.This result highlights the high effectiveness of Extracorporeal Shock Wave Therapy in reducing the subjective perception of pain related to joint stiffness. The results indicate that both conventional treatment and ESWT are effective in reducing subjective pain in patients with OA. However, Group GI showed a more complete and consistent clinical improvement, with all patients reporting mild pain at the end of the study (100%). Studies with larger samples are recommended to confirm these conclusions. In this GC group, a significant improvement was observed, though less pronounced. At T0, there were no patients with mild pain. At T2, 30% of patients moved to the mild pain category, but 70% remained with moderate pain. This demonstrates that, although conventional treatment is effective in reducing pain, the response was less homogeneous compared to the ESWT intervention.The analysis is based on the categorization of patients according to the VAS Scale: severe pain (7–10), moderate pain (4–6), and mild pain (0–3). Analysis of Imaging Test Completion: The data show that both groups had a high adherence to completing tests at the pre-intervention stage, with 90% of participants in each group undergoing MRI or X-ray. However, a significant drop in test completion was observed after the interventions: only 60% of patients in Group GI and 20% in Group GC completed the post-treatment tests. This decline may be attributed to logistical factors, lack of motivation, subjective perception of improvement (especially in the Control group), or the absence of reinforcement regarding the importance of follow-up tests. Additionally, it may reflect operational or financial limitations faced by the participants. The lower adherence to post-tests compromises objective comparative analysis through imaging and should be considered a limitation of the study or bias in estimating the effect of the RCT intervention. Even so, performance behavior can be interpreted as an additional clinical indicator of the subjective response to treatment, warranting emphasis in the study's final discussion. No clinically detectable neuromuscular, systemic, or device-related adverse effects were observed after TOCE, nor were there any patient complaints

De modo geral, os achados apontam que ambos os grupos apresentaram melhora nos sintomas avaliados pelo índice WOMAC. Contudo, a dinâmica da resposta clínica difere entre os tratamentos. A TOCE demonstrou maior precocidade no efeito terapêutico, enquanto o tratamento convencional mostrou-se eficaz a médio prazo, especialmente a partir da marca de 30 dias. Análise Comparativa entre Grupos – Testes t Independentes - A comparação entre os grupos GC e GI foi realizada por meio de testes t independentes, com o objetivo de avaliar possíveis diferenças entre os escores WOMAC em três domínios – Dor, Rigidez e Função Física – nos três momentos do estudo: pré-intervenção, pós-intervenção imediata, e 30 dias após o término da intervenção. Os escores do Grupo GI eram significativamente mais elevados no início do estudo nos domínios de Dor (p = 0,0328) e Rigidez (p = 0,0314), o que indica uma condição clínica inicial mais grave em comparação ao Grupo GC. No momento pós-intervenção imediata (T1), o Grupo GI apresentou escore médio de Dor significativamente menor em relação ao Grupo GC (p = 0,0259), o que sugere uma resposta clínica mais pronunciada e precoce à TOCE. Entretanto, na avaliação realizada 30 dias após a intervenção (T2), não foram observadas diferenças estatisticamente significativas entre os grupos em nenhum dos domínios (p > 0,05), indicando que os efeitos terapêuticos se nivelaram ao longo do tempo. Os dados indicam que, embora o Grupo GI tenha iniciado o estudo em pior condição clínica, a intervenção com TOCE resultou em melhora mais rápida, especialmente no alívio da dor. Ao final de 30 dias (T2), ambos os grupos alcançaram níveis semelhantes de melhora funcional e sintomática, sugerindo eficácia comparável no médio prazo. Para avaliar diferenças na proporção de pacientes respondedores entre os grupos GC e GI, foram realizados testes de proporção (z-test) para cada domínio do índice WOMAC, nos momentos pós-intervenção imediata (T1) e 30 dias após a intervenção (T2). Dor: Após o tratamento, houve tendência de maior proporção de respondedores no Grupo GI (73,3%) em comparação ao Grupo GC (38,5%), com p = 0,0629 (limiar de significância). Aos 30 dias, essa diferença tornou-se estatisticamente significativa (p = 0,0203), indicando eficácia superior do Grupo GI na redução da dor do joelho avaliado. Rigidez: A proporção de respondedores foi consistentemente maior no Grupo GI em ambos os momentos, com diferença significativa aos 30 dias (p = 0,0490). Função Física: O Grupo GI apresentou proporção significativamente maior de respondedores já no momento Pós (T1) (86,7% vs. 30,8%; p = 0,0025) e que, não se modificando os resultados, permaneceu o mesmo índice no T2, sugerindo uma melhora funcional mais precoce e efetiva. Os resultados do z-test reforçam os achados prévios de que a TOCE (Grupo GI) promoveu maior proporção de pacientes com resposta clínica significativa, especialmente nos domínios de Função Física e Dor. Com o objetivo de identificar os aspectos funcionais e sintomáticos mais impactados pela intervenção terapêutica, foi realizada uma análise por item do índice WOMAC, calculando-se a variação média absoluta entre os escores pré e pós-intervenção imediata para cada questão individual do instrumento. Apresentados os cinco itens com maior redução média de escore, na variação absoluta, refletindo as áreas nas quais os participantes relataram maior alívio de sintomas ou melhora funcional. Os itens são codificados conforme a seção do WOMAC (A – Função Física), seguidos da identificação do joelho avaliado (JE: joelho esquerdo; JD: joelho direito), na relação de pacientes numerados de ambos os grupos. Principais achados: O item A6_JD apresentou a maior redução absoluta média, com Δ = 3,25 pontos. O item A11_JD também demonstrou resposta clínica relevante (Δ = 3,17). Três dos cinco itens com maior melhora envolvem o joelho direito. O item A13_JE (Δ = 3,04) indica resposta clínica também no joelho esquerdo. Interpretação clínica: A análise por item complementa as análises por domínio e por grupo ao permitir uma visualização detalhada e específica dos efeitos terapêuticos. Os itens com maior variação absoluta podem indicar atividades mais impactadas pela osteoartrite, com maior potencial de recuperação funcional com o tratamento adequado. Principais observações: A6_JD e A11_JD foram os itens com maior variação média (Δ = 3,25 e Δ = 3,17, respectivamente), refletindo uma melhora substancial na função física do joelho direito. Item A13_JE (Δ = 3,04) também se destaca, indicando que o joelho esquerdo apresentou melhora significativa. O gráfico evidencia que as maiores melhoras ocorreram principalmente em atividades funcionais como andar, levantar-se, subir escadas ou realizar tarefas diárias – todas contempladas pela seção A do WOMAC. A predominância de itens do joelho direito (JD) entre os 10 principais pode indicar maior comprometimento funcional inicial deste lado ou maior responsividade à intervenção. Importância clínica: A análise item a item permite uma visão granular dos efeitos terapêuticos, ajudando a identificar quais atividades cotidianas foram mais beneficiadas com o tratamento. Essa informação pode orientar fisioterapeutas e médicos a personalizarem estratégias reabilitativas com foco nos aspectos de maior impacto funcional para o paciente. Classificação Clínica da Dor (Escala EVA). Análise Estatística. Apresenta-se uma análise estatística dos dados da Escala EVA de dor, complementar à Escala WOMAC, utilizada para avaliar a dor subjetiva em pacientes com osteoartrite de joelho (OAJ). Comparação Entre Grupos no Tempo T2. Foi aplicada a versão exata do Teste de Fisher para avaliar diferenças na proporção de pacientes com dor leve entre os grupos no tempo T2. Os dados foram simplificados em duas categorias: Dor Leve vs Dor Não Leve (Moderada ou Intensa). Embora o Grupo GI tenha apresentado 100% dos pacientes com dor leve, a diferença entre os grupos não foi estatisticamente significativa (p = 0,217), provavelmente devido ao tamanho reduzido da amostra. Evolução Intra-Grupos (T0 → T2): O Teste de McNemar (exato) foi aplicado para avaliar a significância da melhora clínica dentro de cada grupo entre os tempos T0 e T2, considerando transição de dor Não Leve para Leve de JE e JD. Ambos os grupos apresentaram melhora estatisticamente significativa. No entanto, o Grupo GI atingiu 100% de dor leve no T2, enquanto o Grupo GC manteve 25% de pacientes com dor moderada. Mostra-se uma redução completa das dores intensa e moderada ao longo do tratamento. No tempo T0, mais de 90% dos pacientes apresentavam dor intensa ou moderada. Já no tempo T2, 100% dos pacientes foram classificados com dor leve. Esse resultado evidencia a alta eficácia da Terapia por Ondas de Choque Extracorpóreas na redução da percepção subjetiva da dor relacionada à rigidez articular. Os resultados indicam que tanto o tratamento convencional quanto a TOCE são eficazes na redução da dor subjetiva em pacientes com OAJ. Contudo, o Grupo GI apresentou melhora clínica mais completa e consistente, com todos os pacientes relatando dor leve ao final do estudo (100%). Estudos com amostras maiores são recomendados para confirmar essas conclusões. Neste grupo GC, observa-se uma melhora significativa, porém menos acentuada. No T0, não havia nenhum paciente com dor leve. No T2, 30% dos pacientes migraram para a categoria de dor leve, mas 70% permaneceram com dor moderada. Isso demonstra que, embora a tratamento convencional seja eficaz na redução da dor, a resposta foi menos homogênea em comparação à intervenção com TOCE. A análise baseia-se na categorização dos pacientes conforme a Escala EVA: dor intensa (7–10), dor moderada (4–6) e dor leve (0–3). Análise da Realização de Exames de Imagem: Os dados mostram que ambos os grupos apresentaram uma adesão elevada à realização dos exames no momento pré-intervenção, com 90% dos participantes em cada grupo realizando a ressonância magnética ou RX. No entanto, observou-se uma queda significativa na realização dos exames após as intervenções: apenas 60% dos pacientes do Grupo GI e 20% do Grupo GC realizaram o exame no pós-tratamento. Essa queda pode ser atribuída a fatores logísticos, desmotivação, percepção subjetiva de melhora (especialmente no grupo Controle), ou à ausência de reforço sobre a importância do exame de seguimento. Além disso, pode refletir limitações operacionais ou financeiras enfrentadas pelos participantes. A menor adesão no pós-exame compromete a análise comparativa objetiva por imagem e deve ser considerada como uma limitação do estudo ou viés na estimativa do efeito da intervenção dos ECR. Ainda assim, o comportamento de realização pode ser interpretado como um indicativo clínico adicional da resposta subjetiva ao tratamento, merecendo destaque na discussão final do estudo. Não foram observados efeitos adversos neuromusculares, sistêmicos ou relacionados ao dispositivo clinicamente detectáveis após a TOCE e nem queixas dos pacientes

Introduction and Objectives: The study addresses the use of extracorporeal shock wave therapy (ESWT) in the therapeutic management of knee osteoarthritis (KOA). The main objective was to evaluate the effectiveness of ESWT in reducing pain and functional disability in participants with KOA. Methodology: This is a parallel randomized clinical trial (RCT), a double-blind, interventional, and prospective study conducted at the Orthopedics and Traumatology Service of the Armed Forces Hospital (HFA), Federal District, from June 20 to August 31, 2025, with 20 patients aged 60 to 79 years, divided (10 in each) into the Intervention Group (IG) and Control Group (CG). The study was approved by the Research Ethics Committee, Opinion 6.714.207, from UCB, and Opinion 6.878.155, from HFA.Results and Discussion: The GI showed statistically significant differences (p < 0.05) in all assessed domains (Pain – 0.0003, Stiffness – 0.0003, and Physical Function – 0.0006) across the three time comparisons, according to the VAS scale and the WOMAC Index, over 5 sessions, one per week, with evaluation 30 days after the last session. The findings indicate that both groups showed improvement in symptoms assessed by the WOMAC index and VAS. In the evaluation conducted 30 days after the intervention (T2), no statistically significant differences were observed between the groups in any of the domains (p > 0.05), indicating that the therapeutic effects leveled out over time. The data suggest that the GI started the study in worse clinical condition. The TOCE intervention resulted in faster pain relief. At the end of 30 days, both groups reached similar levels of functional and symptomatic improvement, suggesting comparable efficacy in the medium term. The groups showed high adherence to undergoing examinations at time T0, with 90% of participants in each group having an MRI or X-ray, and afterwards, 60% of patients in Group GI and 20% in Group GC underwent the exams at T2, considering the risk of bias in estimating the effect of the intervention in RCTs. No adverse effects were reported in either group. Conclusion: TOCE may improve pain and functionality in elderly individuals with knee osteoarthritis in the short term. It is recommended to conduct longitudinal studies with longer follow-up periods, as well as control for clinical variables (physical activity level, BMI, comorbidities), to consolidate the applicability of TOCE as an effective treatment alternative in the management of knee osteoarthritis. Introdução e Objetivos: o estudo aborda sobre o emprego da terapia extracorpórea com ondas de choque (TOCE) no manejo terapêutico da osteoartrite de joelho (OAJ). O objetivo geral buscou avaliar a efetividade da TOCE para reduzir a dor e a incapacidade funcional em participantes com OAJ. Metodologia: trata-se de um ensaio clínico randomizado (ECR) de estudo paralelo, e um estudo duplo cego, intervencionista e prospectivo realizados no Serviço de Ortopedia e Traumatologia do Hospital das Forças Armadas (HFA), Distrito Federal, no período de 20 de junho a 31 de agosto, de 2025, com 20 pacientes na faixa etária de 60 a 79 anos divididos (10 em cada) em Grupo Intervenção (GI) e Grupo Controle (GC). A pesquisa foi aprovada pelo CEP, Parecer 6.714.207, da UCB e Parecer 6.878.155, do HFA. Resultados e Discussão: o GI apresentou diferenças estatisticamente significativas (p < 0,05) em todos os domínios avaliados (Dor – 0,0003, Rigidez – 0,0003 e Função Física – 0,0006), nas três comparações temporais, pela escala EVA e o Índice WOMAC, de 5 sessões, uma por semana, e avaliação 30 dias após a última sessão. Os achados apontam que ambos os grupos apresentaram melhora nos sintomas avaliados pelo índice WOMAC e EVA. Recomenda-se realizar estudos longitudinais com maior tempo de acompanhamento, bem como o controle de variáveis clínicas (nível de atividade física, IMC, comorbidades), para a consolidação da aplicabilidade da TOCE como alternativa de tratamento efetiva no manejo da OAJ. A combinação entre métodos pode ser uma via de investigação relevante, considerando o efeito sinérgico potencial na recuperação funcional e na qualidade de vida dos pacientes
