This is a randomized controlled clinical study with three arms, which is non-blinded. A total of 90 adults with post-COVID-19 syndrome will undergo three modalities of care in a randomized manner: remote care; personal assistance; and counseling group. Both remote and face-to-face activity groups will participate in a training program with Pilates physical exercises on the ground three times a week for 12 weeks in the morning. Each session will last 50 minutes, with the first five minutes of stretching, followed by the main part, subdivided into training A and training B (performed interspersed) lasting approximately 40 minutes, and, finally, the cool-down at five final minutes of training. Between the sets of exercises, a 45-second rest was performed. The training progression will be performed every three weeks, with the first phase starting with 10 repetitions for each exercise, the second with 12 repetitions, the third with 12 repetitions plus increased load and the last phase 15 repetitions with increased load. The load will be increased through shin guards, dumbbells, body weight and number of breathing cycles during the exercise. The counseling group will participate in an initial counseling meeting and will receive a booklet with advice on lifestyle changes that can help in the process of recovery from the disease, with nutritional and physical activity advice. The intervention will be carried out at the Faculty of Physical Education and Physiotherapy of the Federal University of Uberlândia

Trata-se de um estudo clínico randomizado controlado de três braços, não cego. Um total de 90 adultos com síndrome pós-COVID-19 serão submetidos a três modalidades de atendimento de forma aleatorizada: atendimento remoto; atendimento presencial; e grupo de aconselhamento. Ambos os grupos de atividades remotas e presenciais participarão de um programa de treinamento com exercícios físicos de Pilates no solo com frequência de três vezes semanais durante 12 semanas no período da manhã. Cada sessão terá duração de 50 minutos, sendo os primeiros cinco minutos de alongamento, seguidos pela parte principal, subdividida em treino A e treino B (realizados de forma intercalada) com duração de aproximadamente 40 minutos, e, por último, o resfriamento aos cinco minutos finais do treino. Entre as séries de exercício foi realizado descanso de 45 segundos. A progressão de treino será realizada a cada três semanas, sendo que a primeira fase iniciou com 10 repetições para cada exercício, a segunda com 12 repetições, a terceira com 12 repetições mais a carga incrementada e a última fase 15 repetições com a carga incrementada. A carga será incrementada através de caneleiras, halteres, peso corporal e quantidade de ciclos respiratórios durante o exercício. O grupo de aconselhamento participará de uma reunião de aconselhamento inicial e receberá uma cartilha com conselhos de mudanças de estilo de vida que podem auxiliar no processo de recuperação da doença, com conselhos nutricionais e de atividades físicas. A total of 139 people expressed interest in participating, with 70 excluded based on inclusion criteria. Sixty-nine individuals were randomized, resulting in 23 participants per group. There was attrition during follow-up of 8 participants in the in-person group, 7 in the online group, and 5 in the control group. The final analysis included 15 participants from the in-person group, 16 from the online group, and 18 from the control group.

Values are presented as baseline and post-intervention assessments (mean ± standard deviation) and interaction p-values. A significant improvement in endurance strength was observed in the face-to-face Pilates group (16 ± 4 to 23 ± 4 repetitions) compared with the control group (16 ± 4 to 18 ± 6) and the online Pilates group (19 ± 5 to 21 ± 5), with a significant between-group interaction (p = 0.001). Trunk flexion strength increased significantly in the face-to-face Pilates group (6.6 ± 3.0 to 12.7 ± 6.6 kg) compared with the control group (10.1 ± 4.8 to 8.8 ± 2.7 kg) and the online Pilates group (9.3 ± 7.5 to 11.0 ± 5.6 kg), with a significant between-group interaction (p < 0.01). Similarly, the distance covered in the 6-minute walk test increased significantly in the face-to-face Pilates group (463 ± 73 to 514 ± 39 m), whereas it remained essentially unchanged in the control group (464 ± 67 to 461 ± 44 m) and the online Pilates group (467 ± 53 to 467 ± 42 m), resulting in a significant between-group difference (p = 0.03). Fatigue scores decreased from 35 ± 6.45 to 32 ± 5.98 in the control group, from 31 ± 6.37 to 30 ± 7.08 in the online Pilates group, and from 33 ± 5.52 to 27 ± 7.18 in the face-to-face Pilates group.

This study investigated the impacts of online and face-to-face Mat Pilates training in adults with persistent symptoms of long COVID on health outcomes. Forty-nine patients (52 ± 5.85 yr.) diagnosed with long COVID related to fatigue symptoms were randomly included in three groups: online Mat Pilates training (n = 16), face-to-face Mat Pilates training (n = 15), and a control group (n = 18) without training. Mat Pilates training was conducted three times a week for 12 weeks. Fatigue, functional capacity, anthropometrics, body composition, and cardiometabolic markers were assessed before and after the interventions. Two-factor Generalized Estimating Equation analyses identified significant differences with Bonferroni post hoc testing (p < 0.05). After the intervention, only the face-to-face Mat Pilates training group had an improved total, physical and mental fatigue,
trunk isometric strength, upper limb muscle endurance strength, and aerobic capacity (p < 0.05). No changes were found in fat mass, muscle mass, free fat mass, % of fat, body mass, body mass index, or waist and hip circumferences. No significant changes were observed in blood glucose, glycated hemoglobin, triglycerides, total cholesterol, high-density lipoprotein, low-density lipoprotein, or blood pressure (p > 0.05). Our results highlight the potential of face-to-face Mat Pilates training as an effective intervention to mitigate persistent symptoms of long COVID related to fatigue and functional capacities.pt-br
Este estudo investigou os impactos do treinamento de Mat Pilates (pilates de solo) online e presencial nos desfechos de saúde de adultos com sintomas persistentes de COVID longa. Quarenta e nove pacientes (52 ± 5,85 anos) diagnosticados com COVID longa associada a sintomas de fadiga foram alocados aleatoriamente em três grupos: treinamento de Mat Pilates online (n = 16), treinamento de Mat Pilates presencial (n = 15) e um grupo controle (n = 18) sem treinamento. O treinamento de Mat Pilates foi realizado três vezes por semana, durante 12 semanas. Fadiga, capacidade funcional, dados antropométricos, composição corporal e marcadores cardiometabólicos foram avaliados antes e depois das intervenções. Análises por Equações de Estimativa Generalizada (GEE) de dois fatores identificaram diferenças significativas, confirmadas pelo teste *post hoc* de Bonferroni (p < 0,05). Após a intervenção, apenas o grupo de treinamento de Mat Pilates presencial apresentou melhora na fadiga total, física e mental, na força isométrica de tronco, na resistência muscular de membros superiores e na capacidade aeróbica (p < 0,05). Não foram encontradas alterações na massa gorda, massa muscular, massa livre de gordura, percentual de gordura, massa corporal, índice de massa corporal ou nas circunferências de cintura e quadril. Também não foram observadas alterações significativas na glicemia, hemoglobina glicada, triglicerídeos, colesterol total, lipoproteína de alta densidade (HDL), lipoproteína de baixa densidade (LDL) ou pressão arterial (p > 0,05). Nossos resultados destacam o potencial do treinamento de Mat Pilates presencial como uma intervenção eficaz para mitigar sintomas persistentes de COVID longa relacionados à fadiga e à capacidade funcional.
