This is a prospective, randomized study, which will be carried out in the Adult Intensive Care Unit of the Galileo Hospital and Maternity. Data collection will be carried out after approval of the project by the Research Ethics Committee, including in the study all patients admitted to the hospital, and who meet the inclusion criteria, and mainly, agree to participate in it. After delivery of the written informed consent form, simple randomization was carried out using the Excel program to prepare the envelopes and determine which group the participant would be distributed to, Control Group (GControl), where the patient will perform conventional physiotherapy maneuvers ( FC) consisting of manual compression, vibrocompression and airflow acceleration (AFE) maneuver; Group A (GA), where the patient will undergo alveolar recruitment; Group B (GB), where the patient will perform the chest compression and decompression maneuver/manual lung reexpansion maneuver and Group C (CG) where the patient will perform the breath stacking technique. Data collection will be carried out before and after the application of the alveolar recruitment maneuver, the thoracic compression and decompression maneuver and the breath stacking technique, to quantify the changes resulting from their application, prior to carrying out the recommended maneuvers, the patients will undergo a physiotherapeutic assessment. Next, the patient will be positioned in the supine position, in an anatomical position, with the headboard elevated to 30°; after positioning, he will undergo bronchial hygiene, performing only aspiration in the presence of adventitious sounds identified on lung auscultation and changes in lung mechanics; and will then be subjected to the procedure of their respective drawn groups. In the case of CG, patients will undergo bronchial hygiene using expiratory compression, vibration and/or EFA maneuvers. GA patients will undergo the alveolar recruitment maneuver using a positive end-expiratory pressure of 20cmH2O and an inspiratory pressure of 15cmH2O above PEEP, for 2 minutes, using the Dixtal ventilator. In the case of GB, patients will be positioned in the supine position, in an anatomical position, with the headboard elevated to 30-45°, undergoing ten (10) repetitions of the thoracic compression and decompression maneuver in 10 consecutive breaths in each hemithorax. In the GC, the expiratory branch of the mechanical ventilator circuit will be occluded for 20 seconds; then the expiratory branch will be released allowing expiration. The technique will be performed six consecutive times totaling 120 seconds with a two-minute interval between one application and the next. Each group will have 30 participants

Trata-se de um estudo prospectivo, randomizado, que será realizado na Unidade de Terapia Intensiva Adulto do Hospital e Maternidade Galileo. A coleta dos dados será realizada após aprovação do projeto pelo Comitê de Ética em Pesquisa, incluindo no estudo todos os pacientes internados no hospital, e que preencherem os critérios de inclusão, e principalmente, concordarem em participar do mesmo. Após entrega do termo de consentimento livre esclarecido, por escrito, foi realizada randomização simples utilizando o programa excel para o preparo dos envelopes e determinar qual grupo o participante seria distribuído, Grupo Controle (GControle), onde o paciente realizará as manobras de fisioterapia convencional (FC) composta por manobra de compressão manual, vibro compressão e aceleração do fluxo aéreo (AFE); Grupo A (GA), onde o paciente realizará recrutamento alveolar; Grupo B (GB), onde o paciente realizará a manobra de compressão e descompressão torácica/manobra de reexpansão pulmonar manual e Grupo C (GC) onde o paciente realizará a técnica de breath stacking. A coleta dos dados será realizada antes e após a aplicação da manobra de recrutamento alveolar, da manobra de compressão e descompressão torácica e da técnica de breath stacking, para quantificar as alterações decorrentes da aplicação das mesmas, previamente a realização das manobras preconizadas, os pacientes serão submetidos a uma avaliação fisioterapêutica. Em seguida, o paciente será posicionado em decúbito dorsal, em posição anatômica, com a cabeceira elevada a 30°; após o posicionamento será submetido à higiene brônquica realizando apenas aspiração na presença de ruídos adventícios identificados na ausculta pulmonar e alterações na mecânica pulmonar; e então será submetido ao procedimento de seus respectivos grupos sorteado. No caso do GC os pacientes serão submetidos à higiene brônquica por meio das manobras de compressão expiratória, vibração e/ou AFE. Os pacientes do GA serão submetidos à manobra de recrutamento alveolar utilizando uma pressão expiratória positiva final de 20cmH2O e uma pressão inspiratória de 15cmH2O acima da PEEP, durante 2 minutos, empregando-se o ventilador Dixtal. No caso do GB os pacientes serão posicionados em decúbito dorsal, em posição anatômica, com a cabeceira elevada a 30-45° sendo submetidos à realização de dez (10) repetições da manobra de compressão e descompressão torácica em 10 respirações consecutivas em cada hemitórax. No GC será ocluído o ramo expiratório do circuito do ventilador mecânico durante 20 segundos; em seguida, o ramo expiratório será liberado permitindo a expiração. A técnica será realizada seis vezes consecutivas totalizando 120 segundos com intervalo de dois minutos entre uma e outra aplicação. Em cada grupo será inserido 30 participantes

Introduction: The Breath Stacking (BS) maneuver has been considered a strategy to improve lung expansion in patients admitted to the Intensive Care Unit. It is defined as respiratory stacking, using a unidirectional valve with the expiratory branch occluded, coupled to an interface such as a mask or mouthpiece to the patient's face. The technique consists of performing consecutive insufflations without exhaling, and subsequently, the expiratory branch is released; it can be performed voluntarily, in which the patient is responsive and oriented and will perform consecutive inspirations until the expiration branch of the mask used is released; as well as in unresponsive and unresponsive patients, using the ventilator coupling or manually using the Artificial Maintenance Breath Unit (AMBU) resuscitation bag. Objective: To compare two methods of performing the Breath Stacking maneuver, using a mechanical ventilator and Airway Maintenance Breathing Unit (AMBU), in patients on mechanical ventilation admitted to the ICU. Subjects and method: This is a quasi-experimental prospective clinical study comprising 32 patients admitted to the ICU of the Hospital e Maternidade Galileo Valinhos-SP, Brazil. The selected subjects were initially submitted to the Breath Stacking technique with a mechanical ventilator (BSV) occluding the expiratory branch of the mechanical ventilator circuit for 20 seconds. The technique was performed six consecutive times with a two-minute interval between maneuvers. After 24 hours, the second technique of Breath Stacking with AMBU (BSA) was performed with 20 seconds of insufflations with the valve closed, being performed 10 consecutive times with 1-minute pauses between applications. The variables analyzed were: Blood pressure (BP), heart rate (HR), respiratory rate (RR), peripheral oxygen saturation (SatO2), tidal volume, static compliance (Cst), maximum airway pressure (PIT) and drive pressure (DP). To analyze the data, the deltas (post-pre) were calculated and the Wilcoxon tests and paired T test were used. p values <0.05 were considered significant. Results: It can be seen that the Breath Stacking maneuver, when analyzing hemodynamic variables, there were no significant changes after application of the techniques, therefore, it was safe with no change in vital signs. In the analysis of pulmonary variables: respiratory rate (RR), SatO2, tidal volume (Vt) and static compliance (Cst), there were no significant variations after application of the techniques. In the variables: Peak Pressure (PIT) (p-0.02) and Driving Pressure (DP) (p-0.03), it can be observed that PIT and DP were better in both others, however, in the BSV group, there was a superiority of the technique in relation to the BSA. Conclusion: The BSV technique compared with BSA was more effective in lung expansion, being a safe and effective maneuver in the respiratory mechanics of patients on invasive mechanical ventilation

Introdução: A manobra de Breath Stacking (BS) tem sido considerada estratégia para melhora da expansão pulmonar de pacientes internados em Unidade de Terapia Intensiva. É definida como empilhamento respiratório, utilizando-se de válvula unidirecional com o ramo expiratório ocluído, acoplada a interface como máscara ou bocal ao rosto do paciente. A técnica consiste em realizar insuflações consecutivas sem exalar, e posteriormente, o ramo expiratório é liberado; podendo ser realizada de maneira voluntária, na qual o paciente está responsivo e orientado e irá realizar inspirações consecutivas até a liberação do ramo expiratório da máscara utilizada; como também em pacientes desacordados e não-responsivos, utilizando-se o acoplamento em ventilador ou manualmente utilizando bolsa de ressuscitação Artificial Maintenance Breath Unit (AMBU). Objetivo: Comparar dois métodos de realização da manobra de Breath Stacking, com utilização de ventilador mecânico e Airway Maintenance Breathing Unit (AMBU), em pacientes em uso de ventilação mecânica internados em UTI. Sujeitos e método: Trata-se de estudo clínico, não randomizado composto por 32 pacientes, internados na UTI do Hospital e Maternidade Galileo-Valinhos-SP. Os sujeitos selecionados foram submetidos inicialmente à técnica de Breath Stacking com ventilador mecânico (BSV) ocluíndo o ramo expiratório do circuito do ventilador mecânico durante 20 segundos. A técnica foi realizada seis vezes consecutivas com intervalo de dois minutos entre as manobras. Após 24 horas foi realizada a segunda técnica de Breath Stacking com AMBU (BSA) com 20 segundos de insuflações com a válvula fechada, sendo realizada 10 vezes consecutivas com pausas de 1 minuto entre as aplicações. Foram analisadas as variáveis: Pressão arterial (PA), frequência cardíaca (FC), frequência respiratória (FR), saturação periférica de oxigênio (SatO2), volume corrente, complacência estática (Cst), pressão máxima de vias aéreas (PIT) e drive pressure (DP). Para analisar os dados foram calculados os deltas (pós-pré) e utilizados os testes de Wilcoxon e teste T pareado. Valores de p <0,05 foram considerados significativos. Resultados: Pode-se observar que a manobra de Breath Stacking, quando analisadas variáveis hemodinâmicas, não houve alterações significativas após aplicação das técnicas, portanto, foi segura sem alteração dos sinais vitais. Na análise das variáveis pulmonares: Frequência respiratória (FR), SatO2, volume corrente (Vt) e complacência estática (Cst), não tiveram variações significativas após aplicação das técnicas. Nas variáveis: Pressão de pico (PIT) (p-0,02) e Driving Pressure (DP) (p-0,03), pode-se observar que a PIT e a DP foram melhores em ambos os outros, no entanto, no grupo BSV, houve uma superioridade da técnica em relação ao BSA. Conclusão: A técnica BSV comparada com BSA foi mais efetiva na expansão pulmonar sendo manobra segura e eficaz na mecânica respiratória de pacientes em ventilação mecânica invasiva
