Randomized, controlled clinical trial with three parallel arms: face-to-face intervention group, virtual intervention group and waiting list control group. The study aims to evaluate the efficacy and feasibility of group cognitive-behavioral therapy (CBT), in face-to-face and virtual modalities, in reducing psychological symptoms and anthropometric measurements. After identifying the database of patients who underwent bariatric surgery throughout the year 2018, participants aged 30 to 65 years, of both sexes, will be selected.Sixty participants will be selected based on the inclusion and exclusion criteria. Following an initial contact to present the research objectives, the semi-structured clinical interview to DSM V– SCID-5 – will be conducted to assess the participants' lifetime and current mental disorders. At the end of the interview, participants will be informed about the possibility of participating in the face-to-face group CBT intervention, the online group CBT intervention, or remaining on the waiting list (control group). The interview will be conducted in a single session (scheduled via phone or email). Its duration will depend on the complexity of the participant’s psychiatric history and comorbidity pattern. All interviews will take place at the Institute of Psychiatry of the Hospital das Clínicas, School of Medicine, University of São Paulo, located at Av. Dr. Enéas de Carvalho Aguiar, 255 – 4th floor. All participants will be asked about their internet access, particularly those allocated to the online group. All contact will be made by the therapist and co-therapist. Participants will be blinded to whether they will be placed in the face-to-face CBT group, the online CBT group, or the control group. Eligible participants will be randomly assigned to one of the three groups (face-to-face, online, or control). Randomization will be performed using the Excel tool (=RANDBETWEEN), which enables random allocation.

Ensaio clínico randomizado, controlado, com três braços paralelos: grupo intervenção presencial, grupo intervenção virtual e grupo controle em lista de espera. O estudo visa avaliar a eficácia e viabilidade da terapia cognitivo-comportamental (TCC) em grupo, nas modalidades presencial e virtual, na redução de sintomas psicológicos e medidas antropométricas. Após a identificação do banco de dados dos pacientes que realizaram a cirurgia bariátrica ao longo do ano de 2018, serão selecionados os participantes de 30 a 65 anos, de ambos os sexos. Serão selecionados 60 participantes dentro dos critérios de inclusão e exclusão. Após contato inicial para apresentação dos objetivos da pesquisa, será realizada a Entrevistada clínica semi-estruturada para o DSM V- SCID-5 para identificação de transtornos mentais dos participantes ao longo da vida e no momento presente. Ao final da entrevista, os participantes serão informados sobre as possibilidades de participarem da intervenção da TCCG presencial, TCCG online ou aguardarem na lista de espera (grupo controle). A aplicação da entrevista será em um único encontro (agendada por telefone/email). A duração dependerá da complexidade das alterações na história psiquiátrica e no padrão de comorbidade. Todas as entrevistas ocorrerão no Instituto de Psiquiatria do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, localizada na Av. Dr. Enéas de Carvalho Aguiar, 255 – 4º andar. Todos os participantes serão questionados sobre o acesso à internet, para os que participarem no grupo online e todos os contatos serão realizados pela terapeuta e coterapeuta. Os participantes serão cegos sobre participação no TCCG presencial, online ou grupo controle. Os elegíveis serão randomizados aleatoriamente para os grupos três grupos presencial, online ou controle. Para a randomização será utilizada a ferramenta do Excel (=aleatórioentre) que promove um sorteio.

The sample consisted of 54 participants, predominantly women (84%), with a mean age of 52 years (range: 35–65). Most participants were professionally active (70%). All had undergone bariatric surgery, on average, four and a half years before the interview and five years before the intervention. The mean BMI at the time of surgery was 47.7 kg/m², while the current BMI was 34.5 kg/m². The average weight regain was 21.1%. No significant differences were found between the intervention groups. Regarding current psychopathology, 84% of participants met the criteria for at least one mental disorder. Among them, 62% had anxiety disorders, and 48.6% had either eating disorders or mood disorders. When considering lifetime prevalence, 95% of participants reported having experienced at least one mental disorder. Mood disorders were the most prevalent (78.4%), followed by anxiety disorders (70.3%).

The initial sample included 174 patients who underwent bariatric surgery in 2018 at Hospital das Clínicas. Of these, 57 completed the SCID-5 interview to assess mental disorders, conducted by trained psychologists between July 2022 and January 2023, with good inter-rater reliability (Kappa = 0.78). In the end, 54 patients were randomized into three groups: virtual CBT, in-person CBT, and control (18 in each). After exclusions and refusals, the final sample included 13 in the virtual CBT group, 12 in the in-person CBT group, and 12 in the control group. This is considered a pilot study, as the ideal sample size calculated for statistical significance would require 1,374 participants per group.

BDI-II: Regardless of the assessment time, no significant effect of CBT on depression levels was observed (p = 0.92). Only the effect of time (assessment point) was significant (p = 0.002). At the third assessment (T2), there was an average decrease of 7.5 points in depression scores compared to the first assessment (T0) (95% CI = [-12.4, -2.6], p = 0.001), regardless of the group. BES: No significant effect of CBT on binge eating scores was found, regardless of the assessment time (p = 0.85). Only the effect of time was significant (p = 0.022). At the third assessment (T2), there was an average reduction of 3.9 points in binge eating scores compared to T0 (95% CI = [-7.3, -0.5], p = 0.02), independent of group allocation. GAD-7: CBT had no significant effect on anxiety levels across assessment times (p = 0.496). However, the effect of time was significant (p = 0.00076). At the second assessment (T1), anxiety scores decreased by an average of 2.8 points compared to T0 (95% CI = [-5.0, -0.6], p = 0.01), and at the third assessment (T2), the decrease was 3.3 points on average (95% CI = [-5.5, -1.1], p = 0.001), regardless of the group. AUDIT: There was no significant effect of CBT on alcohol consumption levels, regardless of assessment time (p = 0.35). Similarly, no significant time effect was observed (p = 0.064), regardless of the group. Anthropometric Measurements: No significant interaction effect between group and assessment time was observed for any anthropometric measurements, indicating that CBT did not influence these measures over time. Weight: No significant effect of CBT was found (p = 0.30). A marginal effect of time was observed (p = 0.053), regardless of the group. Neck Circumference: No significant effect of CBT was detected (p = 0.059). Abdominal Circumference: A significant effect of CBT was observed (p = 0.02), as well as a significant time effect (p = 0.002). Regardless of group, there was an average 3% reduction at T1 compared to T0 (RM = 0.97; 95% CI = [0.94, 0.99], p = 0.03) and a 5% reduction at T2 compared to T0 (RM = 0.95; 95% CI = [0.92, 0.99], p = 0.003). Hip Circumference: No significant effect of CBT was detected (p = 0.30), nor was a significant time effect observed (p = 0.07), regardless of group.

Introduction: Evidence from the literature shows that Group Cognitive-Behavioral Therapy (CBT) is a widely used approach both in the preoperative and postoperative periods to address associated mental health conditions. Objective: This study investigated the efficacy and feasibility of Group Cognitive-Behavioral Therapy (CBT), in both in-person and virtual formats, to treat mental health symptoms and changes in body composition in patients who underwent bariatric surgery. Method: The sample included 54 participants selected from a total of 174 individuals who had surgery in 2018 at the Hospital das Clínicas. They were randomized into three groups: virtual CBT, in-person CBT, and a control group. The intervention groups received 12 sessions, with a nutritionist and a physical educator participating in two of them. The control group only took part in the assessments. Assessments were conducted before, after the intervention, and six months later, using standardized instruments (BDI-II, GAD-7, BES, and AUDIT), in addition to anthropometric measures. A high prevalence of mental disorders was observed (84% currently, 95% lifetime). Results: All groups showed reductions in depressive symptoms, anxiety, and binge eating, which were maintained at follow-up. A marginal reduction in body mass and abdominal circumference was also observed, with no significant changes in alcohol consumption. Conclusion: The results indicate that both in-person and virtual group CBT interventions are feasible and beneficial for post-bariatric surgery patients, with good adherence. Standardizing these protocols may facilitate their implementation as part of the clinical care for these patients.

Introdução: Evidências da literatura mostram que a Terapia Cognitivo-Comportamental em Grupo (TCC) é uma abordagem amplamente utilizada tanto no pré quanto no pós-operatório para tratar condições de saúde mental associadas. Objetivo: O estudo investigou a eficácia e viabilidade da Terapia Cognitivo-Comportamental em Grupo (TCC), nos formatos presencial e virtual, para tratar sintomas de saúde mental e alterações na composição corporal em pacientes que realizaram cirurgia bariátrica. Método: A amostra incluiu 54 participantes, selecionados de um total de 174 operados em 2018 no Hospital das Clínicas. Eles foram randomizados em três grupos: TCC virtual, TCC presencial e controle, recebendo 12 sessões, com participação de nutricionista e educador físico em duas delas. O grupo controle apenas participou das avaliações. As avaliações ocorreram antes, após a intervenção e seis meses depois, utilizando instrumentos padronizados (BDI-II, GAD-7, BES e AUDIT), além de medidas antropométricas. Observou-se alta prevalência de transtornos mentais (84% no momento atual, 95% ao longo da vida). Resultados: Todos os grupos apresentaram redução nos sintomas de depressão, ansiedade e compulsão alimentar, mantidas no seguimento. Houve ainda redução discreta na massa corporal e circunferência abdominal, sem alterações significativas no consumo de álcool. Conclusão: Os resultados indicam que as intervenções de TCC em grupo, tanto presencial quanto virtual, são viáveis e benéficas para pacientes pós-cirurgia bariátrica, com boa adesão. A padronização desses protocolos pode favorecer sua implementação como parte do cuidado clínico desses pacientes.
