This is a controlled prospective randomized clinical trial. 64 patients were included and one patient was excluded, totaling the analysis of 63 patients. Patients will be randomized into groups, according to the drainage system to be used: the control group (WS group)– using a conventional water-seal pleural collector (31 patients); and the study group (V group)– using a flutter valve device, the Sinapi® Model XL1000® pleural collector employing a flutter valve (Biomedical, Stellenbosch, Idasvalle, África do Sul) (32 patients). The surgeon will be informed which type of system to be used only after completion of the surgical procedure. In the WS group, the pleural collector comprises a polyvinyl chloride (PVC) bottle with volume of 2 liters, under a sterile water seal initially of 2 cm. In the V group the pleural collector is a chamber in the form of a rectangular transparent rigid prism of ABS polymer (acrylonitrile butadiene styrene), with numbering on its external surface to identify the volume stored, and internal capacity of 1,000 mL. This has an evacuation nozzle on its interior surface, of the half-turn type. It is connected to the tubular drain by a silicone tube, with an adapter for various different calibers of drain. Connected to the silicone tube, in the initial part of the rigid prism, is a one-way mechanical anti-reflux ‘flutter’ valve, with the same format and effect as the Heimlich valve. The flap in the flutter valve is made of an anti-adherent polymer. There is also a compressible suction valve, made of silicone, which is connected at the end of the connection tube of the drain, close to the flutter valve. When pressed, this bulb creates a vacuum in the drain system, permitting verification whether there is an air leak. When the bulb is pressed and does not immediately return to its resting inflated position, it is assumed that there is no air leak via the pleural drain. All patients will undergo an anteroposterior chest x-ray within 24 hours after the end of the operation, to check for pneumothorax or pleural collection. Patients whose x-rays show residual pleural space in the post-operative period will be submitted to suction by the drainage system. In the WS group, pleural suction of 10 cmH2O will be applied, with the standard system of three bottles. In group V, patients will be submited to pleural aspiration who present residual pleural space or for whom the bulb does not remain compressed after compression will be assumed to have residual air leaks and will be submitted to pleural suction of 20 cm H2O. Air leaks will be stopped in the WS group, when there is no longer any bubbling of air in the water seal during a cough with exertion and in the V group, when the bulb remain collapsed for 10 minutes after compression, indicating complete absence of air leaks. In both groups, the drain will be removed when: (i) there is no air leak, (ii) the liquid drawn is less than 200 mL in 24 hours, and (iii) a prior chest x-ray shows complete expansion of the lung. A further chest x-ray will be taken on the return outpatient visit 7 days after hospital discharge.

Trata-se de ensaio clínico prospectivo randomizado controlado de dois braços. Foram incluidos 64 pacientes sendo um paciente excluído, totalizando a análise de 63 pacientes. Os pacientes serão randomizados em dois grupos, conforme o sistema de drenagem a ser utilizado: Grupo controle (SA), com coletor pleural em selo d’água convencional (31 pacientes); Grupo de estudo (V), com coletor pleural com válvula vibratória Sinapi® modelo XL1.000® (Biomedical, Stellenbosch, Idasvalle, África do Sul) (32 pacientes). O cirurgião terá acesso ao tipo de sistema de drenagem a ser utilizado apenas após a finalização do procedimento cirúrgico. O coletor pleural SA é composto por um frasco na forma de cilindro circular com capacidade de 2.000 mL, feito de policloreto de Vinila (PVC), sob um selo de água inicial de 2 cm. O coletor pleural V é composto por um frasco na forma de prisma retangular de polímero ABS (acrylonitrila, butadieno, estireno), rígido, transparente, numerado em sua parte externa para identificação do volume armazenado. A capacidade interna é de 1.000 mL e possui um bico esvaziador na face inferior, do tipo meia-volta. Conecta-se ao dreno tubular do paciente por meio de uma extensão por uma mangueira de silicone com um adaptador para diferentes calibres de dreno. Conectada à mangueira de silicone, na parte inicial do prisma rígido, há uma válvula mecânica unidirecional vibratória antirrefluxo, com o mesmo formato e efeito da válvula de Heimlich. O folheto da válvula é composto por um polímero antiaderente. Há, ainda, um bulbo de sucção, compressível, feito de silicone, que acompanha a mangueira de conexão do dreno, estando instalado na parte final desta, proximalmente à válvula vibratória. Este bulbo realiza um vácuo no sistema de drenagem, quando pressionado, permitindo a verificação da presença ou não do escape de ar. Quando este bulbo é comprimido e não retorna imediatamente à sua posição insuflada de repouso, assume-se que não há saída de ar pelo dreno pleural. Os pacientes serão submetidos a radiografia de tórax em posição anteroposterior em até 24 horas após o término do procedimento cirúrgico, para investigação da presença de pneumotórax ou coleção pleural. Os pacientes que apresentarem, pela radiografia de tórax, espaço pleural residual no pós-operatório serão submetidos à aspiração do sistema de drenagem. No grupo SA, será utilizada a aspiração pleural inicial de 10 cmH2O, com sistema em três frascos. No grupo V, serão submetidos à aspiração pleural os pacientes que apresentarem espaço pleural residual, ou a ausência do colapso temporário do bulbo após sua compressão, serão assumidos como portadores de algum espaço residual e serão submetidos à aspiração pleural de 20 cmH2O. O escape aéreo será considerado interrompido quando: no grupo SA, não houver borbulhamento de ar no sistema de selo d’água durante manobra de tosse; e, no Grupo V, quando o bulbo permanecer colapsado por 10 minutos após sua compressão, caracterizando a ausência total de escape aéreo. Nos dois grupos, o dreno será retirado na ausência de escape aéreo e quando o débito líquido for menor que 200 mL em 24 horas. Além disso, uma radiografia de tórax prévia deverá evidenciar expansão pulmonar completa. Uma nova radiografia de tórax será realizada no retorno ambulatorial, sete dias após a alta hospitalar.

The majority (63%) of the patients suffered from lung cancer; there was no difference in diagnoses between the two groups. The surgery most commonly carried out was wedge resection (52% of the sample; 45% in the WS group (SA) and 59% in the V group. 34 of the surgeries were by VATS, with similar distribution between the two groups. Pleural suction was applied to 18 patients: four (6%) of the WS group (SA), and 14 (21%) of the V group. Time of air leak (TAL) and time in hospital (TIH) were similar between the two groups. TAL varied from one to 20 days, with median of two days. TIH varied from one to 23 days. The time to removal of drain (TRD) was slightly less in the V group (Median TRF in in the WS group was four days [IIQ 3-6], vs. three days [IIQ 2-4] in the V group), but without statistical significance.

Objective: To compare the time of air leak, time to drain removal and length of hospital stay when using a standard water seal drainage system or a pleural collection system with a unidirectional flutter valve and rigid chamber. Methods: Randomised prospective study of patients undergoing open or video-assisted thoracoscopic lung wedge resection or lobectomy. Patients were randomised into groups according to the drainage system used: the control group (WS) using a conventional water seal pleural collector and the study group (V) using a flutter valve device (Sinapi® model XL1000®). Variables related to the drainage system, time to air leak, time to drain removal and hospital stay were compared between groups. Results: Data from 63 patients were analysed, 31 in the WS group and 32 in the V group. The majority (63%) had lung cancer; there was no difference in diagnosis between groups. Time to air leak and hospital stay were similar between groups. The time to drain removal was slightly shorter in the V group. Seven patients had postoperative complications, five in the WS group and two in the V group. Conclusions: The results for air leak, time to drain removal and hospital stay were similar for the V group (using a flutter valve) and the group using a conventional water seal system. However, the system used in the V group had fewer complications and no adverse events and was safe and effective.

Objetivo: Comparar o tempo de vazamento de ar, o tempo de retirada do dreno e o tempo de internação no uso de um sistema padrão de drenagem em selo d'água ou de um sistema coletor pleural empregando uma válvula de flutuação unidirecional e uma câmara rígida. Métodos: Estudo prospectivo randomizado de pacientes submetidos à ressecção pulmonar em cunha aberta ou videotoracoscópica ou lobectomia. Os pacientes foram randomizados em grupos, de acordo com o sistema de drenagem utilizado: grupo controle (WS) com coletor pleural convencional em selo d'água; e o grupo estudo (V) com aparelho flutter valvulado (Sinapi® Modelo XL1000®). Variáveis relacionadas ao sistema de drenagem, tempo de vazamento de ar, tempo para retirada do dreno e tempo de internação foram comparadas entre os grupos. Resultados: Foram analisados dados de 63 pacientes, 31 do grupo WS e 32 do grupo V. A maioria (63%) com câncer de pulmão; não houve diferença nos diagnósticos entre os grupos. O tempo de escape aéreo e o tempo de internação foram semelhantes entre os grupos. O tempo para retirada do dreno foi ligeiramente menor no grupo V. Sete pacientes apresentaram complicações relacionadas à cirurgia, cinco no grupo WS e dois no grupo V. Conclusões: Os resultados para vazamento de ar, tempo para remoção do dreno e tempo no hospital foram semelhantes para o grupo V (usando uma válvula flutuante) e o grupo usando um sistema convencional de selo d'água. Já o sistema utilizado no grupo V apresentou menos complicações e nenhum evento adverso, mostrando-se seguro e eficaz.
