60 Brazilian individuals, of both sexes, aged between 18 and 40 years old, complaining of pain in the temporomandibular joint region, living in the city of Lavras, Minas Gerais, were recruited. The volunteers were evaluated by a trained and experienced physiotherapist, and the initial diagnosis of temporomandibular disorder was obtained according to the guidelines of the “Diagnostic Criteria for Temporomandibular Diseases” tool. Subsequently, participants were randomly distributed into three groups: Dry Needling Group, Myofascial Release/Manual Therapy Group and Cognitive Behavioral Therapy/Control Group, with 20 participants in each. After distribution between groups, participants responded to the Coronavirus-19 Fear Scale, the Visual Analogue Scale and jaw joint mobility were assessed. Data were collected at three different times: Before the intervention, immediately after the last treatment session and 30 days after the end of each treatment. All assessments were carried out by the same researcher. After the first data collection, participants underwent interventions for four weeks. Each week consists of a 30-minute treatment session. After the last treatment session, participants were evaluated again and 30 days after the last session, patients were evaluated again. In the Cognitive Behavioral Therapy Group, participants received simple verbal instructions. In the present study, we assumed cognitive behavioral group therapy as a control in relation to the other groups. Participants in the dry needling group will be provided with the needle in the trigger points for 30 minutes each session

Foram recrutados 60 indivíduos brasileiros, de ambos os sexos, com idade entre 18 e 40 anos, com queixa de dor na região da articulação temporomandibular residentes da cidade de Lavras, Minas Gerais. Os voluntários foram avaliados por um fisioterapeuta treinado e experiente, e o diagnóstico inicial de disfunção temporomandibular foi obtido conforme as diretrizes da ferramenta “Critérios de Diagnóstico para Doenças Temporomandibulares". Posteriormente, os participantes foram distribuídos aleatoriamente em três grupos: Grupo Dry Needling, Grupo Liberação Miofascial/Terapia Manual e Grupo Terapia Comportamental Cognitiva/Controle, com 20 participantes em cada. Após a distribuição entre os grupos, os participantes responderam a Escala de medo do coronavírus-19, a Escala Visual Analógica e a mobilidade articular da mandíbula foi avaliada. Os dados foram coletados em três momentos distintos: Antes da intervenção, imediatamente após a última sessão de tratamento e 30 dias após o término de cada tratamento. Todas as avaliações foram realizadas pelo mesmo pesquisador. Após a primeira coleta de dados os participantes foram submetidos às intervenções por quatro semanas. Cada semana consistiu em uma sessão de 30 minutos de tratamento. Após a última sessão de tratamento, os participantes foram avaliados novamente e 30 dias após a última sessão os pacientes foram avaliados novamente. No Grupo Terapia Cognitiva Comportamental, os participantes receberam instruções verbais simples. No presente estudo, assumimos o grupo terapia cognitivo comportamental como um controle em relação aos demais grupos . Os participantes do grupo dry needling permaneceram com a agulha nos pontos de gatilho por 30 minutos em cada sessão

60 Brazilian individuals were recruited, 46 women and 14 men, with an average age of 30.5 years and complaining of pain in the temporomandibular joint region who lived in Lavras, Minas Gerais. As for the level of education, 15 completed high school, 23 completed or are pursuing an undergraduate degree and 22 completed or are pursuing a postgraduate degree. 25 participants were married or in a stable relationship, while 35 participants were single or divorced. Study participants reported pain in the temporomandibular joint region for, on average, 5.6 years

According to the responses collected, at baseline all participants reported having already felt pain in the jaw, temples, or ear region, on either side, and this pain was felt for the first time on average 5.6 years ago. Of the 60 participants included, two, 49 and nine individuals reported having felt 'no pain', 'pain comes and goes', and 'pain is always present' in the last 30 days, respectively. Furthermore, considering the last 30 days, 48% of participants reported pain when chewing consistent foods, 57% reported having felt pain when moving their jaw and 70% reported habits such as grinding or clenching their teeth or chewing gum. Forty-eight participants reported experiencing a headache in the past 30 days and none of the participants reported joint noises in the past 30 days or jaw locking/hesitation.
In the clinical examination, the pain sites noticed in the last 30 days were: TMJ exclusively (n=12); temporal muscles, masseter and soft structures (n=34), and TMJ and musculature (n=14). Most participants reported feeling pain on both sides of the face during the examination (n=49) and presented corrected deviation as an opening standard (n=51). An increase in pain-free opening amplitude was observed after 30 days in the three groups (time effect; p<0.001). A time*group interaction effect was observed for maximum opening, protrusion and pain measured by the Visual Analogue Scale for pain. The maximum mouth opening measurement increased in the DN and TM groups (p=0.005) (Figure 1), while protrusion increased only in the DN group (p=0.007; test power=97% and large effect size). There was also a decrease in pain scores over time only in the DN and TM groups (p<0.001; test power>99% and large effect size). Left and right laterality measurements did not change throughout the follow-up. Regarding the categorical clinical outcomes compared between the groups over time before (baseline), immediately after and 30 days after the intervention, no noises in the TMJ or locking of the jaw were observed on clinical examination during the follow-up period, in any of the groups. .
Answers to the question “In the last 30 days, how long did any pain you had in the jaw or temporal region on either side last?” were considered on an ordinal scale (a. I had no pain; b. Pain appeared and disappeared; c. Pain was always present) and tested using the Friedman test and Bonferroni correction. Only the ‘Manual Therapy’ group showed a decrease in the score, that is, a lower frequency of pain in the jaw or temporal region, during follow-up (p<0.001); the Dry Needling and Control groups showed no difference over time (p>0.05). A time effect was observed for questions 1, 2, 5, 6 and 7 on the Covid-19 Fear Scale, that is, a decrease in the fear score was observed in the three groups. De acordo com as respostas colhidas, ao baseline todos os participantes relataram já terem sentido dor na região da mandíbula, têmporas, ou ouvido, em qualquer um dos lados, e esta dor foi sentida pela primeira vez há 5,6 anos em média. Dos 60 participantes incluídos, dois, 49 e nove indivíduos relataram terem sentido ‘nenhuma dor’, ‘a dor vem e vai’, e ‘a dor está sempre presente’ nos últimos 30 dias, respectivamente. Ainda, considerando os últimos 30 dias, 48% dos participantes relataram dor ao mastigar alimentos consistentes, 57% relataram terem sentido dor ao movimentar a mandíbula e 70% relataram hábitos como ranger ou apertar os dentes ou mastigar goma. Quarenta e oito participantes relataram terem sentido dor de cabeça nos últimos 30 dias e nenhum dos participantes relatou ruídos articulares nos últimos 30 dias ou travamento/hesitação da mandíbula.
No exame clínico, os locais de dor percebidos nos últimos 30 dias foram: ATM exclusivamente (n=12); músculos temporal, masseter e estruturas moles (n=34), e ATM e musculatura (n=14). A maior parte dos participantes relatou sentirem dor em ambos os lados da face ao exame (n=49) e apresentou desvio corrigido como padrão de abertura (n=51). Um aumento da amplitude de abertura sem dor foi observado após 30 dias nos três grupos (efeito tempo; p<0,001). Um efeito de interação tempo*grupo foi observado para a abertura máxima, protrusão e dor medida pela Escala Analógica Visual para dor. A medida de abertura máxima de boca aumentou nos grupos DN e TM (p=0,005) (Figura 1), enquanto a protrusão aumentou apenas no grupo DN (p=0,007; poder do teste=97% e tamanho do efeito grande). Houve também diminuição nos escores de dor ao longo do tempo apenas nos grupos DN e TM (p<0,001; poder do teste>99% e tamanho do efeito grande). As medidas de lateralidade esquerda e direita não se alteraram ao longo do seguimento. Quanto aos desfechos clínicos categóricos comparados entre os grupos ao longo dos tempos antes (baseline), imediatamente após e 30 dias após a intervenção, não foram observados ruídos na ATM ou travamento da mandíbula ao exame clínico durante o tempo de seguimento, em nenhum dos grupos.
As respostas à questão “Nos últimos 30 dias, quanto tempo durou qualquer dor que você teve na mandíbula ou na região temporal em qualquer um dos lados?” foram consideradas em escala ordinal (a. The social isolation measures imposed during the COVID-19 pandemic exacerbated psychological and behavioral disorders. Even with the gradual return to activities, many individuals remained afraid of becoming infected and this perpetuated anxiety states that potentially influenced variables related to temporomandibular disorders (TMD). Non-drug therapies such as myofascial release and dry needling (DN) can decrease temporomandibular joint (TMJ) pain in patients with TMD. Furthermore, it is possible that these techniques beneficially influence psychological outcomes and fear of COVID-19. Thus, with the present study, we aimed to evaluate the effects of manual therapy (MT) and NP on pain, joint mobility, and fear of COVID-19 in patients with TMD. Sixty participants diagnosed with myofascial TMD by the Diagnostic Criteria Tool for Temporomandibular Disorders (DC/TMD) were randomly distributed into three groups: Manual Therapy Group (GTM) and Dry Needling Group (GDN) and Cognitive Behavioral Therapy/Control Group (GCO). Participants responded to the COVID-19 Fear Scale (FCV-19S), the Visual Analogue Scale (VAS) and jaw joint mobility was assessed. Data were collected at three different times: before the intervention (T0); immediately after the last treatment session (T1) and 30 days after the end of each treatment (T2). All participants were treated for four weeks (one 30-minute session per week). The data were submitted to ANOVA Two-way mixed linear model, considering the effect of the group (dry needling x Manual Therapy x Control) and time (baseline, immediately after and 30 days later) and the interaction time*group in the variance observed in the outcomes. A time*group interaction effect was observed for maximum opening, protrusion, and pain/VAS, where the maximum opening measure increased in the DN and MT groups (p=0.005), while the protrusion increased in the DN group (p=0.007; test power=97% and large effect size). A decrease in pain scores (VAS) over time was observed only in the NP and MT groups (p<0.001; test power >99% and large effect size). Furthermore, the total COVID-19 fear score decreased in the DN and MT groups after 30 days compared to the GCO (p=0.033). The results show that both ND and TM treatment provided significant improvements in pain, joint mobility, and fear of COVID-19. It is concluded that the treatment with the TM and DN techniques are effective for improving parameters related to TMD

As medidas de isolamento social impostas durante a pandemia do COVID-19 exacerbaram transtornos psicológicos e comportamentais. Mesmo com o retorno gradual às atividades, muitos indivíduos continuaram com medo de se infectar e isso perpetuou estados de ansiedade que, potencialmente, influenciaram variáveis relacionadas à disfunção temporomandibular (DTM). Terapias não medicamentosas como a liberação miofascial e o dry needling (DN) podem diminuir a dor na articulação temporomandibular (ATM) em pacientes com DTM. Além disso, é possível que estas técnicas influenciem beneficamente desfechos psicológicos como o medo do COVID-19. Assim, objetivamos com o presente estudo avaliar os efeitos da terapia manual (TM) e DN sobre a dor, mobilidade articular e medo do COVID-19 em pacientes com DTM. Sessenta participantes diagnosticados com DTM miofascial pela Ferramenta Critérios de diagnóstico para doenças temporomandibulares (DC/TMD) foram distribuídos aleatoriamente em três grupos: Grupo Terapia Manual (GTM) e Grupo Dry Needling (GDN) e Grupo Terapia Cognitiva Comportamental/Controle (GCO). Os participantes responderam à Escala de medo do COVID-19 (FCV-19S), a Escala Visual Analógica (VAS) e a mobilidade articular da mandíbula foi avaliada. Os dados foram coletados em três momentos distintos: antes da intervenção (T0); imediatamente após a última sessão de tratamento (T1) e 30 dias após o término de cada tratamento (T2). Os dados foram submetidos à ANOVA Modelo linear misto de duas vias, considerando efeito do grupo (dry needling x Terapia Manual x Controle) e do tempo (baseline, imediatamente após e 30 dias depois) e a interação tempo*grupo na variância observada nos desfechos. Foi observado um efeito de interação tempo*grupo para a abertura máxima, protrusão e dor/EVA, onde a medida de abertura máxima aumentou nos grupos DN e TM (p=0,005), enquanto a protrusão aumentou no grupo DN (p=0,007; poder do teste=97% e tamanho do efeito grande). Observou-se diminuição nos escores de dor (EVA) ao longo do tempo apenas nos grupos DN e TM (p<0,001; poder do teste>99% e tamanho do efeito grande). Além disso, o escore total de medo do COVID-19 diminuiu nos grupos DN e TM após 30 dias em relação ao GCO (p=0,033). Os resultados evidenciam que tanto o tratamento com DN quanto com TM proporcionaram melhoras significativas na dor, mobilidade articular e medo do COVID-19. Conclui-se que o tratamento com as técnicas de TM e DN são eficazes para melhorias dos parâmetros relacionados à DTM
