This is a study conducted at a single center, located at UNICAMP Hemocenter, and it is a randomized, prospective, double-blind, and controlled clinical trial. The patients will be randomly assigned to Group A and Group B using opaque, sealed envelopes. Group A will consist of 20 patients who will undergo treatment for partial supraspinatus tendon injury using ultrasound-guided dry needling technique. Group B will consist of 20 patients who will undergo treatment for partial supraspinatus tendon injury using ultrasound-guided dry needling technique combined with intralesional and subacromial injection of Platelet-Rich Plasma (PRP). Clinical and ultrasound imaging evaluation will be conducted at diagnosis, in the sixth week, and sixth month after the procedure. The researchers evaluating the outcomes will be blinded to the group allocation of each participant. Inclusion criteria will include patients of both genders, aged over 25 years, with a partial supraspinatus tear measuring equal to or less than 1 cm in the longitudinal axis as evaluated by ultrasound. Patients will be referred for ultrasonographic analysis for diagnosis and evaluation of the Supraspinatus tendon treatment outcome. The patients will be examined in a seated position with internal rotation. A General Electric logic 5 ultrasound device (Healthcare, Milwaukee, USA) with a linear transducer of 4-12 MHz will be used. The evaluation will be performed by a single ultrasound physician and will consist of two transverse measurements of the Supraspinatus tendon. The results will be expressed in millimeters. For the assessment of quality of life aspects, the self-report questionnaires Medical Outcomes Study 36 Short Form Health Survey (SF-36) and The Western Ontario Rotator Cuff Index (WORC) will be administered three times. Pre-treatment assessment will be conducted before the start of treatment (0 days), at 6 weeks, and 12 weeks after treatment. If the control group (treatment with dry needling) shows the best result after 6 months of treatment, they will receive PRP treatment as well.

Trata-se de um estudo realizado em um único centro, localizado no Hemocentro da UNICAMP. É um ensaio clínico randomizado, prospectivo, duplo cego e controlado. Os pacientes serão designados aleatoriamente para os Grupos A e B, utilizando envelopes opacos e lacrados. O Grupo A será composto por 20 pacientes que receberão tratamento para lesão parcial do tendão supraespinhal utilizando a técnica de agulhamento seco guiado por ultrassom. O Grupo B será composto por 20 pacientes que receberão tratamento para lesão parcial do tendão supraespinhal utilizando a técnica de agulhamento seco associado à injeção intralesional e subacromial de plasma rico em plaquetas, também guiado por ultrassom. Será realizada uma avaliação clínica e de imagem por ultrassom no momento do diagnóstico, na sexta semana e no sexto mês após o procedimento. Os pesquisadores responsáveis pela avaliação dos resultados não terão conhecimento sobre a qual grupo cada participante pertence. Os critérios de inclusão são pacientes de ambos os sexos, com idade acima de 25 anos, apresentando uma rotura parcial do tendão supraespinhal igual ou menor que 1 cm no eixo longitudinal, conforme avaliado por ultrassom. Os pacientes serão encaminhados para análise ultrassonográfica para diagnóstico e avaliação dos resultados do tratamento do tendão supraespinhal. Os pacientes serão examinados sentados, com rotação interna. Será utilizado um aparelho de ultrassom General Electric logic 5 (Healthcare, Milwaukee, EUA), com um transdutor linear de 4-12 MHz. O exame será realizado por um único médico ultrassonografista e consistirá em duas medidas transversais do tendão supraespinhal. Os resultados serão expressos em milímetros. Para avaliar os aspectos de qualidade de vida, serão utilizados os questionários de autorrelato Medical Outcomes Study 36 Short Form Health Survey (SF-36) e The Western Ontario Rotator Cuff Index (WORC), que serão aplicados em três momentos. A avaliação pré-tratamento será realizada antes do início do tratamento (0 dias), após 6 semanas e 12 semanas do tratamento. Caso o grupo controle (tratamento com agulhamento seco) apresente os melhores resultados após 6 meses de tratamento, eles também receberão o tratamento com plasma rico em plaquetas (PRP).

In Group B (PRP), we analyzed the improvement in the WORC questionnaire using the Wilcoxon test. We found a significant positive difference in the T0-T6s and T0-T6m time points, but no difference was found in the T6w-T6m time point. The results were as follows: T0-T6w: p value < 0.0001 (with difference), T0-T6m: p value < 0.0001 (with difference), T6w-T6m: p value = 0.6974 (without difference). The collected data demonstrate a significant improvement in the first 6 weeks after treatment, which was sustained at 6 months. In Group A (Placebo), we analyzed the improvement in the WORC questionnaire using the Wilcoxon test. We found a significant positive difference in the T0-T6w and T0-T6m time points, but no difference was found in the T6w-T6m time point. The results were as follows: T0-T6w: p value = 0.0034 (with difference), T0-T6m: p value = 0.0157 (with difference), T6w-T6m: p value = 0.5336 (without difference). The collected data demonstrate a significant improvement in the WORC questionnaire in the first 6 weeks after treatment in both groups, which was sustained at 6 months. However, the p values in the first two phases of Group A (Placebo) were lower than the calculated p value for Group B (PRP). When comparing the two groups (A and B) using the Mann-Whitney test, we did not find a statistically significant difference in the WORC questionnaire at T0 (p value = 0.7796, without difference), nor at T6s (p value = 0.1624, without difference). However, we found a significant difference at T6m in favor of Group B (PRP) (p value = 0.0199, with difference). The collected data show that PRP treatment presents superiority in terms of improvement in the WORC questionnaire at 6 months, but not at 6 weeks, according to the Mann-Whitney test. Ultrasonographic measurement of Supraspinatus tendon rupture size reduction: During the patient follow-up, due to the COVID-19 pandemic, we lost some ultrasound examination data. In Group B - PRP, 3 ultrasound examinations were not performed at T6s (6 weeks), and in Group A - Placebo, 2 ultrasound examinations were not performed at T6w (6 weeks) and 1 ultrasound examination was not performed at T6m (6 months). However, this incident did not hinder the calculation by time points, according to the described significance tests. In Group B (PRP), we analyzed the improvement in the tendon rupture size using the Wilcoxon test. We found a significant positive difference in the T0-T6w, T0-T6m, and T6w-T6m time points. The results were as follows: T0-T6w: p value < 0.0001 (with difference), T0-T6m: p value < 0.0001 (with difference), T6w-T6m: p value = 0.0015 (with difference). The collected data demonstrate a significant improvement in the size of the ruptures in Group B (PRP) in the first 6 weeks after treatment, which further increased at 6 months. In Group A (Placebo), we analyzed the improvement in the tendon rupture size using the Wilcoxon test and did not find a significant positive difference in the T0-T6w and T6w-T6m time points. However, we found a difference between the T0-T6m time points. The results were as follows: T0-T6w: p value = 0.1953 (without difference), T0-T6m: p value = 0.0398 (with difference), T6w-T6m: p value = 0.3652 (without difference). The collected data demonstrate a significant reduction in the size of the Supraspinatus tendon rupture in Group B (PRP), which increased continuously over 6 months. On the other hand, there was no significant reduction in Group A (Placebo) in the first 6 weeks after treatment, but a reduction was observed at 6 months. However, the p values in all three phases were lower than the calculated p value for Group B (PRP). When comparing the two groups (A and B) using the Mann-Whitney test, we did not find a statistically significant difference in the tendon rupture size at T0 (p value = 0.0754, without difference). However, a significant difference was found at T6w (p value = 0.0119, with difference) and T6m (p value = 0.0002, with difference). The collected data show that PRP treatment presents superiority in terms of improvement in the size of the tendon rupture at 6 weeks and 6 months, according to the Mann-Whitney test.

No Grupo B (PRP), analisamos a evolução da melhora do tamanho da rotura do tendão utilizando o teste de Wilcoxon. Encontramos uma diferença positiva significativa nos tempos T0-T6s, T0-T6m e T6s-T6m. Os resultados foram os seguintes: T0 - T6s: valor de p < 0,0001 (com diferença), T0 - T6m: valor de p < 0,0001 (com diferença) e T6s - T6m: valor de p = 0,0015 (com diferença). Os dados coletados mostram uma melhora significativa no tamanho das roturas no Grupo B (PRP) nas primeiras 6 semanas após o tratamento, que aumentou em 6 meses. Results summary:

The aim of this study was to compare the effects of barbotage of the supraspinatus tendon (ST) partial rupture with or without Platelet Rich Plasma (PRP) using the Western Ontario Rotator Cuff Index score, and ultrasound (US) images, at 6 weeks and 6 months after treatment. Patients showed clinical improvement in both groups, and no significant difference in the score at 6 weeks was found. However, at 6 months a significant improvement was observed in the PRP group (p=0,019). Supraspinatus Tendon rupture decreased in both groups after treatment, and there was a significant enhancement at 6 weeks and 6 months in the PRP group. In conclusion, US guided barbotage technique associated with PRP or saline solution are effective treatments for clinical improvement and decrease in the size of the partial ST tear, with better results with PRP in clinical improvement at 6 months. The association of PRP with barbotage is superior in reducing the size of the ST tear at 6 weeks and 6 months, reaching complete healing in 79,3% of the tears.

O objetivo deste estudo foi comparar os efeitos da barbotagem da ruptura parcial do tendão supraespinhal (ST) com ou sem Plasma Rico em Plaquetas (PRP) usando o escore Western Ontario Rotator Cuff Index e imagens de ultrassom (US), em 6 semanas e 6 meses após o tratamento. Os pacientes apresentaram melhora clínica em ambos os grupos, e nenhuma diferença significativa na pontuação em 6 semanas foi encontrada. Entretanto, aos 6 meses foi observada melhora significativa no grupo PRP (p=0,019). A ruptura do tendão supraespinal diminuiu em ambos os grupos após o tratamento, e houve uma melhora significativa em 6 semanas e 6 meses no grupo PRP. Em conclusão, a técnica de barbotagem guiada por US associada ao PRP ou solução salina são tratamentos eficazes para melhora clínica e diminuição do tamanho da ruptura parcial do ST, com melhores resultados com PRP na melhora clínica em 6 meses. A associação de PRP com barbotagem é superior na redução do tamanho da rotura do ST em 6 semanas e 6 meses, atingindo cicatrização completa em 79,3% das roturas.
