  Data collection on demographic information, previous diseases, use of medications and other clinical characteristics such as weight (Kg) and height (m). The assessment of respiratory muscle strength (manovacuometry), lung function (spirometry), functional capacity (6-minute walk test) and laboratory tests will be performed at the baseline (at the first outpatient visit after transplantation) and after six weeks of respiratory muscle training. Training Program: patients in the control group and the intervention group will perform the traning from home by twenty minutes on daily basis for six weeks, using Inspiratory Muscle Training Threshold IMT (Philips Respironics, Amsterdam, Netherlands). Whenever signs of tiredness are detected the patient may have a minute break. To the intervation group, the inspiratory load will be adjusted in 40% of the Maximal Respiratory Pressure previously measured, and the training load will be adjusted biweekly to keep the 40% of the Maximal Respiratory Pressure. The control group will be submitted to the same evaluations of the intervention group, however will perform the protocol using Trashold “sham”, with minimum load (9 cmH20), being maintained this load during all the training. The participation will be evaluated via telephone contact weekly. It is estimated that it will take 19 patients in the intervention group and 19 in the control group

  Coleta de dados sobre informações demográficas, doenças prévias, uso de medicações e outras características clínicas como peso (kg) e altura (m). As avaliações da força muscular respiratória (manovacuometria), da função pulmonar (espirometria), da capacidade funcional (teste de caminhada de 6 minutos) e exames laboratoriais serão realizadas na linha de base (na primeira consulta ambulatorial após o transplante) e após seis semanas de treinamento muscular respiratório. Programa de Treinamento: os pacientes do grupo controle e do grupo intervenção realizarão o treinamento em seu domicilio por 30 minutos, sete dias por semana, durante seis semanas, utilizando o Inspiratory Muscle Training Threshold IMT (Philips Respironics, Amsterdam, Holanda). O treinamento será limitado a 20 minutos para o primeiro dia, sendo que nos dias seguintes o tempo poderá ser aumentado até, no máximo, 30 minutos. O paciente poderá realizar um intervalo de um minuto durante o treinamento, sempre que necessário, se apresentar sinais de cansaço. Para o grupo intervenção, será ajustada a carga inspiratória em 40% da Pimáx previamente aferida, e as cargas de treinamento serão ajustadas quinzenalmente para manter os 40% da Pimáx. O grupo controle será submetido às mesmas avaliações do grupo intervenção, porém, realizará o protocolo utilizando Threshold sham, ou seja, com carga mínima (9 cmH2O), sendo mantida essa carga durante todo o período. A adesão será avaliada via contato telefônico, uma vez por semana. Estima-se que serão necessários 19 pacientes no grupo de intervenção e 19 no grupo de controle

  Renal transplant recipients up to six months post-transplant; of both sexes; aged between 6 and 18 years; under follow-up at the Nephrology Outpatient Clinic of Santo Antonio Children’s Hospital; and whose parents or guardians agree to participate by signing the written informed consent
