  Viscoelastic is used in eye surgeries, mainly in cataract surgeries. Medilon is a viscoelastic manufactured by Mediphacos Indústrias Médicas SA and has been registered with ANVISA (National Health Surveillance Agency) since 2005. This study has as its primary objective to document the safety and performance of Medilon and the secondary objectives are: to document intraocular pressure , endothelial cell density and intraocular inflammation after the surgical procedure; document usability by the surgeon. For this, this monocular clinical investigation will be carried out in 15 participants, a unicentric, prospective, controlled, non-randomized study, using the viscoelastic under study, Medilon, to be carried out at Oculare - Hospital de Oftalmologia. The surgical procedure consists of anesthetic preparation determined by the anesthesiologist, usually with sedation and anesthetic block. Followed by asepsis and preparation of the surgical field. Initially, 2 clear cornea incisions are performed, followed by the introduction of viscoelastic solution to form the anterior chamber, and capsulotomy. The lens extraction procedure will be performed with a phacoemulsifier, with emulsification techniques and parameters determined by the surgeon's preference and experience. After cleaning the lens capsular bag, the Intraocular Lens will be introduced into the eye, through an injector containing the intraocular lens and viscoelastic solution, and positioned in the capsular bag. In the surgical procedure, only Medilon viscoelastic solution will be used. The surgical procedure will be performed by the ophthalmologist (Dr. Frederico Bicalho) who is a sub-specialist in cataract surgery. Finished with cleaning of the anterior chamber and dressing.

  O viscoelástico é utilizado em cirurgias oculares, principalmente, em cirurgias de catarata. O Medilon é um viscoelástico manufaturado pela Mediphacos Indústrias Médicas S.A. e tem registro na ANVISA (Agência Nacional de Vigilância Sanitária) desde 2005. Este estudo tem como objetivo primário documentar a segurança e o desempenho do Medilon e os objetivos secundários são: documentar a pressão intraocular, a densidade de células endoteliais e inflamação intraocular após o procedimento cirúrgico; documentar a usabilidade pelo cirurgião. Para isto, será realizada esta investigação clínica monocular em 15 participantes, um estudo unicêntrico, prospectivo, controlado, não randomizado, utilizando o viscoelástico em estudo, o Medilon, a ser realizado na Oculare - Hospital de Oftalmologia. O ato cirúrgico consiste no preparo anestésico determinado pelo médico anestesista, sendo usualmente realizado uma sedação e bloqueio anestésico. Seguido de assepsia e preparo do campo cirúrgico. Inicialmente, 2 incisões são realizadas do tipo clear cornea, seguido da introdução de solução viscoelástica para formação da câmara anterior, e capsulotomia. O procedimento de extração do cristalino será realizado com facoemulsificador, com técnicas de emulsificação e parâmetros determinados pela preferência e experiência do cirurgião. Após limpeza do saco capsular do cristalino, a Lente Intraocular será introduzida no olho, por um injetor contendo a lente intraocular e solução viscoelástica, e posicionada no saco capsular. No procedimento cirúrgico somente será utilizada a solução viscoelástica Medilon. O procedimento cirúrgico será realizado pelo médico oftalmologista (Dr. Frederico Bicalho) subespecialista em cirurgia de catarata. Finalizado com limpeza da câmara anterior e curativo.

• Inclusion criteria:

  Patients who had a clinical indication for cataract capsular extraction surgery with phacoemulsification with intraocular lens implantation in the posterior chamber. Adult patients 21 years of age or older. Patient with the ability to provide informed consent in the Free and Informed Consent Form approved by the Ethics Committee. Patients willing and able to return for all scheduled follow-up exams for 30 days after surgery.

  Pacientes que tiveram indicação clínica para cirurgia de extração capsular da catarata com facoemulsificação com implante de Lente intraocular na câmara posterior. Pacientes adultos com 21 anos de idade ou mais. Paciente com capacidade de fornecer consentimento informado no Termo de Consentimento Livre e Esclarecido aprovado pelo Comitê de Ética. Pacientes dispostos e aptos a retornarem para todos os exames de acompanhamento programados por 30 dias após a cirurgia.

  Mono vision patient. Patient with any anterior segment pathology (examples: keratoconus, dystrophies or significant corneal scarring, guttata, inflammation, edema, dystrophy, etc.) in the eye to be operated. Patient with anterior segment pathology likely to increase the risk of an adverse outcome for phacoemulsification cataract capsular extraction surgery with posterior chamber Intraocular lens implantation (eg pseudoexfoliation syndrome, synechiae, iris atrophy, inadequate dilation, shallow anterior chamber, traumatic cataract, lens subluxation) in the eye to be operated. Patient with any condition that prevents reliable specular microscopy in the eye to be operated on. Patient with uncontrolled glaucoma or glaucoma under treatment. Patient with a history of retinal detachment. Patient with diabetic retinopathy (proliferative or non-proliferative). Clinically significant changes in the pigment epithelium retinal and macula. Severe recurrent anterior or posterior segment inflammation of unknown etiology. Patient with immunodeficiency (infectious, neoplastic or medicated). Patient with chronic use of systemic steroids or immunosuppressive drugs. Patient who participated in any clinical investigation in the eye to be operated within 30 days prior to entry into this study.

  Paciente mono visão. Paciente portador de qualquer patologia do segmento anterior (exemplos: ceratocone, distrofias ou cicatriz corneana significativa, guttata, inflamação, edema, distrofia, etc.) no olho a ser operado. Paciente com patologia do segmento anterior que provavelmente aumenta o risco de um resultado adverso para cirurgia de extração capsular da catarata com facoemulsificação com implante de Lente Intraocular na câmara posterior (por exemplo, síndrome de pseudo exfoliação, sinéquias, atrofia da íris, dilatação inadequada, câmara anterior rasa, catarata traumática, subluxação de lente) no olho a ser operado. Paciente com qualquer condição que impeça a microscopia especular confiável no olho a ser operado. Paciente com glaucoma não controlado ou glaucoma em tratamento. Paciente com história descolamento de retina. Paciente com retinopatia diabética (proliferativa ou não proliferativa). Alterações clinicamente significativas do epitélio pigmentar retiniano e mácula. Inflamação recorrente grave do segmento anterior ou posterior de etiologia desconhecida. Paciente com imunodeficiência (infecciosa, neoplásica ou medicamentosa). Paciente em uso crônico de esteroides sistêmicos ou medicamentos imunossupressores. Paciente que participou de qualquer investigação clínica no olho a ser operado dentro de 30 dias antes da entrada neste estudo.

  Intraocular pressure will be evaluated at different times throughout the study, if it is greater than or equal to 30 mmHg postoperatively, it will be considered as the primary outcome.

  Throughout the study, the density of endothelial cells will be monitored in the postoperative period compared to the preoperative period, as well as the presence of postoperative inflammation. During the surgical procedure, the usability of the product will be evaluated.
