  Two interventions were performed on each patient, one in a conventional way (control group) and another guided (test group), 10 participants in each group, and each of these interventions used a surgical guide, in the control group this surgical guide was made in an analog way, and in the test group this guide was prototyped, a dichotomous record was made after to check if the guides are well adapted to start the surgical procedures. In the control group, an incision was made over the ridge crest and in the teeth adjacent to the edentulous region, one tooth for distal and one for mesial, after the incision the flap was prepared with full-thickness and detached both on the buccal and lingual surfaces until exposed 10mm in height of bone tissue, and then with the guide already checked in terms of adaptation, it was put in position again and the drilling was started with the following sequence of drills: spear, 2mm diameter drill, conical drill 3,5mm and 4,3mm tapered drill, installing the implant later and completing the proposed treatment. On the side equivalent to the test group, the measurement of the mucosa was taken into account since there was no flap making, making it impossible to directly view the bone. With the guide in position, a reduction of 3.5 / 2Ø or 4.3 / 2Ø was put through the guide depending on the thickness of the implant, and then through the reduction, the spear drill was used to drill the cortical, followed by the drill 2mm in diameter, drilling up to the planned size of the implant, adding the thickness of the mucosa, right after the reduction was removed and in cases of 3.5mm thick implants, the last drilling was performed through the guide without the reduction, already in cases of 4.3mm thick implants, a new 4.3 / 3.5 reduction was used, then the 3.5mm drill was used, and the reduction was removed, followed by the final drilling with the 4.3mm drill, delimiting the implant installation.

  Foi realizado duas intervenções em cada paciente, uma de maneira convencional (grupo controle) e outra guiada (grupo teste), sendo 10 participantes em cada grupo, e cada uma dessas intervenções utilizaram um guia cirúrgico, no grupo controle esse guia cirúrgico foi confeccionado de maneira analógica, e no grupo teste este guia foi prototipado, após a confecção foi realizado um registro dicotômico para checar se os guias estão bem adaptados para então dar início aos procedimentos cirúrgicos. No grupo controle foi feita uma incisão sobre a crista do rebordo e nos dentes adjacentes a região edêntula, um dente para distal e um para mesial, após a incisão o retalho foi preparado com espessura total, e descolado tanto na face vestibular quanto na lingual até que se exponha 10mm em altura de tecido ósseo, e então com o guia já verificado em termos de adaptação, foi posto em posição novamente e se iniciou a perfuração com a seguinte sequência de brocas: lança, broca de 2mm de diâmetro, broca cônica 3,5mm e broca cônica 4,3mm, instalando o implante posteriormente e concluindo o tratamento proposto. No lado equivalente ao grupo teste a medida da mucosa foi levada em consideração já que não houve confecção de retalho, impossibilitando a visão direta sobre o osso. Com o guia em posição foi posto através do guia uma redução de 3,5/2Ø ou 4,3/2Ø a de depender da espessura do implante, e então através da redução foi utilizada a broca lança para perfurar a cortical, seguida da broca de 2mm de diâmetro perfurando até o tamanho planejado do implante adicionando a espessura da mucosa, logo após a redução foi removida e em casos de implantes de 3,5mm de espessura foi realizada a última perfuração através do guia sem a redução, já em casos de implantes de 4,3mm de espessura, foi posta uma nova redução de 4,3/3,5 então a broca 3,5mm foi utilizada, e a redução removida, seguida da perfuração final com a broca de 4,3mm delimitando o caminho de instalação do implante.

  Good oral hygiene with visible plaque index less than or equal to 20%; age greather than or equal to 18 years; sufficient bone availability for implant installation, with minimum height and thickness equal to 10 and 5mm, respectively; loss on both sides of the mandible of posterior teeth; sufficient interocclusal height to adapt the prototyped surgical guide and perform the perforations.

  History of periodontal surgical procedures in the operated region; systemic alterations that could compromise the surgical procedures or healing; presence of inflammation or infection involving the region to be operated; bone pathology in the implant site; smokers, people with bruxism, alcoholics, drug addicts, diabetics, pregnant women or those who wish to become pregnant in the next year, who have a history of radiotherapy treatment in the head and neck region, who take medications that may interfere with bone remodeling or that have pathologies that affects bone metabolism.

  With the results of this research, it is expected that flapless guided surgery will facilitate the installation of implants in the ideal position and reduce surgical time and postoperative morbidity when using specific virtual planning software for making surgical guides that makes possible to perform surgery for implant installation in a less invasive manner, the validation of the proposed method will be done based on statistical analysis, in which the confidence level will be established at 95%.

  Com os resultados dessa pesquisa espera-se que a cirurgia guiada sem retalho facilite a instalação de implantes na posição ideal e reduza o tempo cirúrgico e a morbidade pós operatória quando se utiliza softwares específicos de planejamento virtual para a confecção de guias cirúrgicos no qual possibilitam executar a cirurgia de instalação de implantes de maneira menos invasiva, a validação do método proposto será feito a partir de análises estatísticas, em que o nível de confiança será estabelecido em 95%.

  The validation of free software for planning and making a prototyped surgical guide will facilitate student's understanding and performance of the procedure, for this to happen the virtual planning will be done by the students through specific software, and then corrected together with the advisor professor to validate what was done and make adjustments if necessary.
