Experimental group: 20 participants with burning mouth were submitted to transcutaneous electrical nerve stimulation using a device at a frequency of 50 Hertz with intensity modulated according to the patient's comfort ranging from 1 - 4 (pulse amplitude) with a duration of 25 minutes per session, with one session per week, totaling eight weeks of treatment at the end of the study. The device's self-adhesive electrodes were fixed on the participant's face, following the path of the trigeminal nerve branch on the side affected by pain and/or burning. Control group: 20 participants with burning mouth underwent low power laser therapy with punctual applications of infrared laser (L2) (power of 100mW, 808 nm, 6 joules of energy/60 seconds) in the affected region, keeping one centimeter of distance between the points, until the entire area of the complaint was irradiated. being held one session per week, totaling eight weeks of treatment at the end of the study.

Pain/burning was the primary outcome and was measured on a visual analogue scale (VAS; 0–10 cm). The patients were asked to choose a number from 0 to 10 on the scale to describe the level of intensity of their symptoms. Changes in unstimulated salivary flow, xerostomia, and dysgeusia were the secondary outcomes. Saliva was collected as described by Navazesh and Kumar and patients with an unstimulated salivary fow rate < 0.1 mL/min were classifed as having hyposalivation. Information on xerostomia was collected according to the method of Murray Thomson et al. using a standard question and the Xerostomia Inventory (XI). Patients answered the following question: “How often does your mouth feel dry?” (response options: “never,” “occasionally,” “frequently,” “always”). On each occasion, those who gave either of the latter two responses were categorized as xerostomic. The XI was used to obtain information on the severity of xerostomia symptoms, being an 11-item summated rating scale which combines the responses to 11 individual items into a single continuous scale score which represents the severity of chronic xerostomia, and higher scores represent more severe symptoms. The patients were asked to choose one of fve responses (“never,” scoring 1; “hardly ever,” 2; “occasionally,” 3; “fairly often,” 4; and “very often,” 5) to the following statements referring to the previous 4 weeks: “my mouth feels dry”; “my lips feel dry”; “I get up at night to drink”; “my mouth feels dry when eating a meal”; “I sip liquids to aid in swallowing food”; “I suck sweets or cough lollies to relieve dry mouth”; “my throat feels dry”; “the skin of my face feels dry”; “my eyes feel dry”; “my lips feel dry”; and “the inside of my nose feels dry.” Each individual’s responses was scored and summed to give a single XI score. Dysgeusia was analyzed using an adaptation of the method proposed by Muller et al., in which the four basic tastes at four diferent concentrations are tested. Urea was used as bitter taste as proposed by Henn et al. (sucrose solution: 0.4 g/mL, 0.2 g/mL, 0.1 g/mL, 0.005 g/mL; sodium chloride: 0.25 g/mL, 0.1 g/mL, 0.04 g/mL, 0.016 g/mL; citric acid: 0.3 g/mL, 0.165 g/mL, 0.09 g/mL, 0.05 g/mL; urea: 0.005 g/mL, 0.04 g/mL, 0.02 g/mL, 0.01 g/mL). Filter paper strips were impregnated with the test solutions as proposed by Kettenmann et al. The strips were placed in the center of the tongue and the patient was asked to identify the taste. The 10th percentile was used to defne the presence or absence of dysgeusia. The primary and secondary outcomes were collected at baseline (T0), after the 4th treatment session (T1), after the 8th treatment session (T2), and 30 days after the end of treatment (T3).

Os pacientes foram solicitados a escolher um número de 0 a 10 na escala para descrever o nível de intensidade de seus sintomas. Alterações no fluxo salivar não estimulado, xerostomia e disgeusia foram os desfechos secundários. A saliva foi coletada conforme descrito por Navazesh e Kumar e pacientes com fluxo salivar não estimulado < 0,1 mL/min foram classificados como tendo hipossalivação. As informações sobre a xerostomia foram coletadas segundo o método de Murray Thomson et al. utilizando uma pergunta padrão e o Inventário de Xerostomia (IX). Os pacientes responderam à seguinte pergunta: “Com que frequência você sente a boca seca?” (opções de resposta: “nunca”, “ocasionalmente”, “frequentemente”, “sempre”). Em cada ocasião, aqueles que deram uma das duas últimas respostas foram categorizados como xerostômicos. O IX foi utilizado para obter informações sobre a gravidade dos sintomas de xerostomia, sendo uma escala de avaliação resumida de 11 itens que combina as respostas a 11 itens individuais em uma única pontuação de escala contínua que representa a gravidade da xerostomia crônica, e pontuações mais altas representam sintomas mais graves. Os pacientes foram solicitados a escolher uma entre cinco respostas (“nunca”, pontuação 1; “quase nunca”, 2; “ocasionalmente”, 3; “ frequentemente”, 4; e “muito frequentemente”, 5) para as seguintes afirmações: referindo-se às 4 semanas anteriores: “sinto a boca seca”; “meus lábios estão secos”; “Levanto à noite para beber”; “minha boca fica seca quando faço uma refeição”; “Tomo um gole de líquido para ajudar na deglutição”; “chupo doce ou pirulito para para aliviar a boca seca”; “minha garganta está seca”; “a pele do meu rosto parece seca”; “meus olhos estão secos”; “meus lábios estão secos”; e “o interior do meu nariz parece seco”. As respostas de cada indivíduo foram pontuadas e somadas para dar uma única pontuação ao IX. A disgeusia foi analisada por meio de uma adaptação do método proposto por Muller et al., no qual são testados os quatro sabores básicos em quatro concentrações diferentes. A ureia foi utilizada como sabor amargo conforme proposto por Henn et al. (solução de sacarose: 0,4 g/mL, 0,2 g/mL, 0,1 g/mL, 0,005 g/mL; cloreto de sódio: 0,25 g/mL, 0,1 g/mL, 0,04 g/mL, 0,016 g/mL; ácido cítrico: 0,3 g/mL, 0,165 g/mL, 0,09 g/mL, 0,05 g/mL; ureia: 0,005 g/mL, 0,04 g/mL, 0,02 g/mL, 0,01 g/mL). Introduction: The management of patients with chronic burning mouth is a challenge in the dental clinic. Objective To compare the efect of photobiomodulation (PBM) and transcutaneous electrical nerve stimulation (TENS) in the treatment of burning mouth. Materials and methods Randomized clinical trial of 25 patients with burning mouth treated by TENS (n=12) and PBM (n=13). The patients were treated weekly for 8 weeks. Two-factor ANOVA was used to determine whether the two interventions promoted signifcant diferences in symptoms (measured with a visual analogue scale), unstimulated salivary fow, xerostomia, and dysgeusia between T0 (baseline), T1 (after the 4th treatment session), T2 (after the 8th treatment session), and T3 (30 days after the end of treatment). Results Intragroup comparison of VAS scores for pain showed a signifcant diference between T0xT1, T0xT2, and T0xT3 in the TENS group and between T0xT2 and T0xT3 in the PBM group (p˂0.001). Intergroup comparison of VAS scores for pain between T2xT3 showed a better response to PBM than to TENS (p=0.003). Patients of the TENS group showed an increase in salivary fow between T1 and T2 (p=0.052). There were no expressive variations in xerostomia or dysgeusia in the two groups analyzed. Conclusion TENS and PBM were efective in reducing the symptoms of burning mouth during and after treatment. The PBM group showed a better response during follow-up.

Introdução: O manejo de pacientes com ardor bucal crônico é um desafio na clínica odontológica. Objectivo: Comparar o efeito da Terapia a Laser de Baixa Intensidade (LLLT) e da Estimulação Elétrica Nervosa Transcutânea (TENS) no tratamento do ardor bucal. Materiais e métodos: Ensaio clínico randomizado constituído por 25 pacientes com ardor bucal que foram tratados por TENS (n=12) e por LLLT (n=13). O tratamento foi realizado semanalmente por 8 semanas. ANOVA dois fatores foi usado para verificar se existia diferença significativa entre os tempos T0 (baseline), T1 (após a 4ª sessão de tratamento), T2 (após a 8ª sessão de tratamento) e T3 (30 dias após o término do tratamento) em relação aos sintomas, analisados por meio da Escala Visual Analógica, fluxo salivar não estimulado, xerostomia e disgeusia com as intervenções de TENS e LLLT. Resultados: A TENS e a LLLT foram eficazes na redução dos sintomas relatados pelos pacientes (p˂0,001). Observou-se entre os tempos T2 e T3 que o grupo LLLT apresentou uma melhor resposta quando comparado ao TENS (p=0,003). Os pacientes do grupo TENS apresentaram aumento do fluxo salivar entre os tempos T1 e T2 (p=0,052). Não foram observadas variações expressivas no que se refere a xerostomia e a disgeusia nos dois grupos analisados. Conclusão: A TENS e a LLLT foram eficazes na redução dos sintomas do ardor bucal durante e após o término do tratamento, sendo que o grupo LLLT apresentou uma melhor resposta durante o acompanhamento. Clinical relevance: TENS mostrou-se uma terapia segura e eficaz para o ardor bucal.
