  This is a randomized controlled clinical trial with three arms (total sample= 54 individuals). Constraint-induced therapy experimental group: 18 individuals diagnosed with stroke and severe upper limb impairment will receive task-oriented intensive repetitive training with a behavioral approach. Constraint-induced therapy + brain-machine interface experimental group: 18 individuals diagnosed with stroke and severe upper limb impairment will receive task-oriented intensive repetitive training with a behavioral approach associated with the brain-machine interface + imagery. Control Group: 18 individuals, diagnosed with stroke and severe impairment of the upper limb, will receive intensive conventional occupational therapy training. The three groups will perform the interventions 3 hours/day, 5 days/week, for 15 consecutive days. The groups were randomly randomized by an independent investigator, and all assessments were performed by a blinded evaluator.

  Trata-se de um ensaio clínico randomizado controlado com três braços (amostra total= 54 indivíduos). Grupo experimental terapia por contensão induzida: 18 indivíduos, com diagnóstico de AVC e comprometimento severo do membro superior, receberão treino repetitivo intensivo direcionado a tarefa com abordagem comportamental. Grupo experimental terapia por contensão induzida + interface cérebro máquina: 18 indivíduos, com diagnóstico de AVC e comprometimento severo do membro superior, receberão treino repetitivo intensivo direcionado a tarefa com abordagem comportamental associado à interface cérebro máquina + imagética. Grupo Controle: 18 indivíduos, com diagnóstico de AVC e comprometimento severo do membro superior, receberão treino de terapia ocupacional convencional intensiva. Os três grupos realizarão as intervenções 3 horas/dia, 5 dias/semana, por 15 dias consecutivos. Os grupos foram randomizados de maneira aleatória por um pesquisador independente, assim como todas as avaliações foram realizadas por um avaliador cego.

  Stroke more than six months before the beginning of the protocol; age between 18 and 80 years; hemiparesis as a sequela of stroke; score on the Fugl Meyer scale for the upper extremity ≤53 points (moderate and severe injuries), but present at least 30º of shoulder flexion and/or abduction and 10º of elbow extension (from 90º flexion), as well as, greater impairment in the hand most affected by the stroke, evidenced by the deficit in the active extension of the fingers of the hand most affected by the stroke (towel test: should not be able to pick up a face towel from a table and release it more than three times in a minute ); finger flexor tone of the most affected hand <3, assessed by the Modified Ashworth scale; ability to understand simple commands and instructions for assessments and intervention protocols; be available to attend assessments and therapy for 15 consecutive days and remain 3 hours/day in treatment

  Acidente Vascular Cerebral há mais de seis meses ao início do protocolo; idade entre 18 e 80 anos; hemiparesia como sequela do AVC; pontuação na escala de Fugl Meyer para extremidade superior ≤53 pontos (lesões moderadas e severas), porém apresentar no mínimo 30º de flexão e/ou abdução de ombro e 10º de extensão de cotovelo (a partir da flexão de 90º), assim como, maior comprometimento na mão mais afetada pelo AVC, evidenciado pelo déficit na extensão ativa de dedos da mão mais afetada pelo AVC (teste da toalha: não deverá ser capaz de pegar uma toalha de rosto de uma mesa e soltar mais que três vezes em um minuto); tônus em flexores de dedos da mão mais afetada <3, avaliado pela escala de Ashworth Modificada; habilidade em compreender comandos simples e instruções para as avaliações e os protocolos de intervenção; ter disponibilidade em comparecer para as avaliações e terapia 15 dias consecutivos e permanecer 3 horas/dia em tratamento

  With the primary outcome, it is intended to evaluate the improvement in the function and structure of the upper limb affected by stroke through the Fugl Meyer scale that has the domains of upper extremity (36 points), wrist (10 points), hand (14 points), coordination and movement speed (6 points), totaling 66 points with the inclusion of the reflex evaluation, and the minimum clinically significant change of this scale is 4.25-7.25 points, therefore it is expected that the experimental groups present a value minimum difference with the control group of 4.25 points

  With the secundary outcome, it is intended to evaluate the improvement of activities performed with the upper limb affected by stroke through the Graded Wolf Motor Function Test, which has 10 activities in which the patient must perform as quickly as possible and with the best quality possible for up to 120 seconds, at the same time the evaluator is analyzing the quality of movement during the activities performed and scoring through the functional ability scale, which varies from 0 to 5 points, with 0 - when the individual refuses to perform the activity, 1 - when trying to perform but not completing the activity within the time, 2 - performing the activity with an inadequate movement pattern, primitive patterns, compensation in the requested movement or with the help of the least affected (poor) upper limb, 3 - performing the activity slower or with compensation to perform the requested movement, but in other body structures not directly involved in the requested activity (regular ), 4 - almost normal movement, only slower or with some imprecision (almost normal) and 5 - normal movement. It is expected that the experimental groups show a significant difference in relation to the control group (there are no clinical parameters such as minimal detectable change, nor clinically significant for this instrument)

  Com o desfecho secundário pretende-se avaliar a melhora das atividades realizadas com o membro superior afetado pelo AVC através do Graded Wolf Motor Function Test que possui 10 atividades em que o paciente deve executar o mais rápido possível e com a melhor qualidade que conseguir por até 120 segundos, ao mesmo tempo o avaliador está analisando a a qualidade do movimento durante as atividades realizadas e pontuando por meio da escala de habilidade funcional, que varia de 0 à 5 pontos, sendo 0 - quando o indivíduo se nega a realizar a atividade, 1 - quando tenta realizar mas não completa a atividade dentro do tempo, 2 - realiza a atividade com um padrão de movimento inadequado, padrões primitivos, compensações no movimento solicitado ou com o auxílio do membro superior menos afetado (pobre), 3 - realiza a atividade de maneira mais lenta ou com compensações para realizar o movimento solicitado, porém em outras estruturas corporais não diretamente envolvidas na atividade solicitada (regular), 4 - movimento quase normal, somente mais lento ou com alguma imprecisão (quase normal) e 5 - movimento normal. Espera-se que os grupos experimentais apresentem diferença significativa em relação ao grupo controle (não há parâmetros clínicos como mínima mudança detectável, nem clinicamente significativa para este instrumento)
