  This will be a clinical, longitudinal, prospective, analytical, controlled, randomized, 1:1 allocation (with a sample size of 243 patients), double-blind trial to be carried out in some hospitals in the city of Salvador, Bahia, Brazil. Patients will be followed up for a period of 28 days. They will receive standard hyperprotein normocaloric supplement (control) or supplement enriched with immunonutrients (experimental) for a period of 10 days (5 days preoperatively and 5 days postoperatively) and will be submitted to collection of blood samples on D0 and on D10. Post-surgical and infectious complications of the control and experimental groups will be analyzed, as well as length of stay and hospital costs. Serum samples will be sent to analyze certain laboratory tests, such as blood count, albumin, C-reactive protein (CRP), CD4 and CD8 lymphocytes, interleukins 2, 4, 6, 8 and 10, in addition to tumor necrosis factor alpha (TNF-α ). The results generated may reveal a way to prevent serious intercurrences in the perioperative period and will guide the establishment of clinical protocols.

  Tratar-se-á de um ensaio clínico, longitudinal, prospectivo, analítico, controlado, randomizado, alocação 1:1 (com valor amostras de 243 pacientes), duplo-cego a ser realizado em alguns hospitais na cidade de Salvador, Bahia, Brasil. Os pacientes serão acompanhados até um período de 28 dias. Eles receberão suplemento normocalórico hiperproteico padrão (controle) ou suplemento enriquecido com imunonutrientes (experimental) por um período de 10 dias (5 dias no pré-operatório e 5 dias no pós-operatório) e serão submetidos a coleta de amostras de sangue no D0 e no D10. Serão analisados as complicações pós-cirúrgicas e infecciosas dos grupos controle e experimental, bem como tempo de internamento e custos hospitalares. As amostras séricas serão encaminhadas para analisar determinados exames laboratoriais, como hemograma, albumina, proteína C reativa (PCR), linfócitos CD4 e CD8, interleucinas 2, 4, 6, 8 e 10, além de fator de necrose tumoral alfa (TNF-α). Os resultados gerados poderão revelar uma forma de prevenção de intercorrências graves no perioperatório e guiarão o estabelecimento de protocolos clínicos.

  The main outcome of the study will be the determination of the use of INO in the perioperative period in patients with any malignant neoplasm of the GI tract, which can reduce the risk of surgical and infectious complications, resulting in better clinical outcomes.

  The secondary outcomes of the study will be to analyze whether the use of oral immunonutrients in the perioperative period of patients with gastrointestinal cancer can cause a decrease in mortality, length of hospital stay and hospital costs. In addition, serum levels of inflammatory markers will be analyzed (Leucometry, CRP, IL-2, IL-4, IL-6, IL-8, IL-10, INF-gamma and TNF-α) in patients with breast cancer. treated gastrointestinal tract, in order to demonstrate whether the use of INO can reduce such levels. With this, the study will discuss whether the use of INO can positively impact the levels of CD4 and CD8 lymphocytes, serum albumin.

  Os desfechos secundários do estudo serão analisar se o emprego de imunonutrientes orais no perioperatório de pacientes portadores de câncer do trato gastrointestinal pode causar diminuição de mortalidade, de tempo de internamento hospitalar e dos custos hospitalares. Além disso, serão analisados os níveis séricos de marcadores inflamatórios (Leucometria, PCR, IL-2, IL-4, IL-6, IL-8, IL-10, INF-gama e TNF-α) em pacientes portadores de câncer do trato gastrointestinal tratados, a fim de evidenciar se o emprego de INO pode reduzir tais níveis. Com isso, o estudo irá discutir se o uso de INO pode impactar positivamente nos níveis de linfócitos CD4 e CD8, de albumina sérica.
