A clinical, interventional, prospective, single-center, randomized, single-blind study was carried out to evaluate the effect of neuromuscular electrical stimulation with the IBRAMED NEURODYN™ II device on the hemodynamics of the lower limbs and on the systemic fibrinolytic activity of 10 healthy individuals. The experiment was divided into 6 moments. a) M0 – Positioning for rest, b) M1 – Pre-stimulation moment; c) M2 – Moment of electrical stimulation, lasting 20 minutes, d) M3 – Moment immediately after M2, e) M4 – 120 minutes after the end of M2 and f) M5 – 240 minutes after the end of M2. At moment 0, the eligible participant, called healthy, positioned himself in a controlled office (20-25 degrees Celsius, with relative air humidity greater than 20%), sitting, with his feet at the same level as his hips, being authorized knee flexion and hip external rotation. He could wear shorts and should go without socks, for proper measurement of parameters. He then remained like this for 30 minutes of rest. Right before the start of electrical stimulation, the tolerability score that would be used was again explained to the individual, which was used as a possible criterion for discontinuity (if a score between 4 and 5 during exposure). Then, at moment 2, there was electrical stimulation. This was done by positioning the electrodes with the aid of a strap and transducer gel on the skin of the posterior and proximal part of the leg. Electrodes were placed on both lower limbs, but only the randomized side was stimulated. This positioning allowed the electrodes to apply a stimulus to the common peroneal nerve. Stimulation of this nerve by the device caused the muscles to contract isometrically. Contraction of the calf muscles boosted blood flow to the lower limbs. After the end of exposure, at moments 3, 4 and 5, ultrasound evaluations were carried out again and new blood samples were collected for laboratory tests. During the 4 hours after exposure, the participant maintained the previously described resting position. The sonographer did not know which lower limb the electrical stimulation was applied to. The lower limb subjected to the experiment was selected by simple randomization of 1:1 between the right and left. Randomization was carried out using a specialized program (https://www.randomizer.org/), which generated a list defining which limb was allocated to electrical stimulation. This list was not communicated to the researcher. The electrodes that performed the electrical stimulation were installed on the randomized lower limb. Electrodes were applied to the other limb, but were not stimulated. The lower limbs were covered with a sheet. A non-research employee performed the implant. Blinding was done by instructing the participant not to reveal which limb the experiment was carried out on and by applying electrodes to both lower limbs. The outcome assessor was blind, that is, he did not have access to information about which limb was stimulated.

Foi realizado um estudo clínico, intervencionista, prospectivo, unicêntrico, randomizado, unicego para avaliar o efeito da estimulação elétrica neuromuscular com o aparelho IBRAMED NEURODYN™ II sobre a hemodinâmica dos membros inferiores e sobre a atividade fibrinolítica sistêmica de 10 indivíduos saudáveis. O experimento foi dividido em 6 momentos. a) M0 – Posicionamento para repouso, b) M1 – Momento pré-estimulação; c) M2 – Momento da eletroestimulação, com duração de 20 minutos, d) M3 – Momento logo após M2, e) M4 – 120 minutos após fim de M2 e f) M5 – 240 minutos após fim de M2. No momento 0, o participante elegível, chamado de saudável, posicionou-se em um consultório controlado (20-25 graus Celsius, com umidade relativa do ar maior que 20%), sentado, com os pés no mesmo nível do quadril, sendo autorizados flexão do joelho e rotação externa do quadril. Poderia usar shorts e deveria ficar sem meias, para adequada medição dos parâmetros. Permaneceu assim, então, por 30 minutos de repouso. Logo antes do início da eletroestimulação, foi novamente explicado ao indivíduo o score de tolerabilidade que seria utilizado, o qual foi utilizado como possível critério de descontinuidade (se pontuar entre 4 e 5 durante a exposição). Em seguida, no momento 2, houve a eletroestimulação. Esta foi feita através do posicionamento dos eletrodos com auxílio de cinta e gel transdutor na pele da parte posterior e proximal da perna. Foram posicionados eletrodos em ambos os membros inferiores, mas apenas o lado randomizado foi estimulado. Este posicionamento permitiu que os eletrodos aplicassem um estímulo sobre o nervo fibular comum. A estimulação deste nervo pelo dispositivo fez com que os músculos se contraíssem isometricamente. A contração dos músculos da panturrilha impulsionou o fluxo sanguíneo dos membros inferiores. Após o fim da exposição, nos momentos 3, 4 e 5, foram realizadas novamente as avaliações ultrassonográficas e novas coletas de sangue para exames laboratoriais. Durante as 4 horas após a exposição, o participante manteve a posição de repouso previamente descrita. O ultrassonografista não soube em qual membro inferior foi aplicado a eletroestimulação. O membro inferior submetido ao experimento foi selecionado por randomização simples de 1:1 entre o direito e o esquerdo. A randomização foi feita por programa especializado (https://www.randomizer.org/), que gerou uma lista com a definição de qual membro foi alocado para eletroestimulação. Esta lista não foi informada ao pesquisador. Os eletrodos que realizaram a eletroestimulação foram instalados no membro inferior randomizado. No outro membro, foram aplicados eletrodos, mas que não foram estimulados. Os membros inferiores foram cobertos por um lençol. Um funcionário não-pesquisador realizou o implante. O cegamento se deu por orientação ao participante de não revelar em qual membro foi realizado o experimento e pela aplicação de eletrodos em ambos os membros inferiores. O avaliador dos desfechos foi cego, ou seja, não teve acesso a informação de qual membro foi estimulado.

Outcome found 1: Of all the study variables, the vessel diameter was the one that proved to be most sensitive to electrical stimulation. There was an increase in diameter in all vessels analyzed, including at moments M4 and M5. The diameters varied without statistical difference between the stimulated and non-stimulated limb, which allowed joint analysis of the data and suggests a systemic effect of NMES. This occurred regardless of the fact that the variation in flow volume was not significant, which suggests that it was not just a dilation caused by the flow. The continuous increase in the diameter of the popliteal arteries (the largest median was found in M5) suggests a prolonged residual effect caused by NMES, which continued even after the end of stimulation. Flow velocities varied without statistical difference between limbs over time, except for the popliteal vein. Only the femoral vessels had significant changes in arterial and venous flow velocities. The velocities in the popliteal vessels varied positively, but not significantly. The clinical significance of increased flow favors the use of the technique in therapies that require increased flow, such as in the prevention of VTE and as a possible therapy for CVI and PAD. There was variation in both the stimulated and non-stimulated limbs, with no statistical difference between them over time. The variation in flow volume in the joint analysis of the limbs for the popliteal vein, between M4 and M1 and between M5 and M4, was statistically significant, which suggests a late effect of NMES.

The participants were 10 people who, initially, considered themselves healthy, invited at random. They underwent screening at the Vascular Laboratory of the Hospital das Clínicas of the Faculty of Medicine of Botucatu, at which time the anamnesis and physical examination were carried out, with the application of the eligibility criteria and presentation of the consent form.

The joint analysis of the moments to identify differences between the stimulated vessels and those that were not stimulated was only significant for the flow velocity of the popliteal vein, therefore, for this vein, it is possible to observe the difference in the median and velocity distribution flow between veins that have been stimulated and those that have not been stimulated. In the rest of the vessels and variables analyzed, this difference was not observed. All individuals gave the lowest score for discomfort and there were no adverse events, expected or not. There was no statistically significant change in the plasminogen activator inhibitor during the moments. There was only numerical variation, less than 2%. There was a numerical variation, with a continuous increase, of up to almost 6% in M4, but this did not constitute a statistically significant change. The variation in flow volume proved to be significant in the joint analysis of the limbs, at moment 4 for the popliteal vein, with a decrease of 23.94%, going from 12.135 ml/min to 9.23 ml/min as well as the interquartile median ( IQR) had a reduction of 40.22%, going from 11.98ml/min to 7.16ml/min. The diameter of the vessels proved to be the most sensitive to NMES in the joint analysis of the limbs. The variation for moment 3 and moment 5 were significant. The median and IQR variation were, respectively; 11.03% (0.72 to 0.805mm) and 36.61% (0.725 to 0.188mm) for moment 3 and from; 10.35% (0.72 to 0.8mm) and 44.61% (0.175 to 0.188mm) for moment 5. The diameter of the femoral vein changed significantly at all moments. The median and IQR varied, respectively; 38.28% (0.64 to 0.885mm) and 55.17% (0.29 to 0.45mm), for moment 3; 32.3% (0.64 to 0.845mm) and 36.89% (0.29 to 0.39mm), for moment 4; 39.06% (0.64 to 0.89mm), 88.62% (0.29 to 0.547), for moment 5. For the popliteal artery, only moment 3 was significant. The variation in the median and IQR of the diameter for this artery in the joint analysis of the limbs: 10.18% (0.54 to 0.595mm) and 6.89% (0.145 to 0.155mm). The median diameter of the popliteal vein was significantly increased in M3 and M4; 31% (0.5 to 0.655mm) and 43% (0.5 to 0.715mm) respectively. As for the interquartile median, moment 3 changed negatively by 9.52% (0.21 to 0.19mm) and moment 4 had an increase of 8.57% (0.21 to 0.228mm). Flow velocities varied in a statistically significant way only in the femoral vessels. In the popliteal vessels there was a variation in flow increment, but it was not statistically significant. The femoral artery showed significant increases in velocity at moments 4 and 5. The median and interquartile median varied respectively, 31.39% (287.635 to 377.92cm/s) and 51.71% (155.247 to 235.52cm/s) , for moment 4 of 46.09% (287.635 to 420.20cm/s) and 10.09% (155.24 to 170.91cm/s). The median femoral vein flow velocity increased significantly, varying 22.22% (239.99 to 293.61cm/s) at moment 4 and 33.63% (239.99 to 320.69cm/s) for the moment 5. At both moments the interquartile median varied negatively, 4.64% (199.61 to 190.34cm/s), at moment 4 and 24.92% (199.61 to 149.86cm/s), at moment moment 5.

A análise conjunta dos momentos para identificação de diferenças entre os vasos estimulados e aqueles que não foram estimulados somente foi significativo para a velocidade de fluxo da veia poplítea, sendo assim, para essa veia, é possível observar a diferença da mediana e da distribuição da velocidade fluxo entre as veias que foram estimuladas e aquelas que não foram estimuladas. A variação da mediana e da IQR foram, respetivamente; 11,03% (0,72 para 0,805mm) e 36,61% (0,725 para 0,188mm) para o momento 3 e de; 10,35% (0,72 para 0,8mm) e 44,61% (0,175 para 0,188mm) para o momento 5. O diâmetro da veia femoral modificou-se significativamente em todos os momentos. A mediana e a IQR variaram, respectivamente; 38,28% (0,64 para 0,885mm) e 55,17%(0,29 para 0,45mm), para o momento 3; 32,3%(0,64 para 0,845mm) e 36,89% (0,29 para 0,39mm), para o momento 4; 39,06%(0,64 para 0,89mm), 88,62% (0,29 para 0,547), para o momento 5. Para a artéria poplítea apenas o momento 3 foi significativo. A variação da mediana e da IQR do diâmetro para essa artéria na análise conjunta dos membros: 10,18% (0,54 para 0,595mm) e 6,89% (0,145 para 0,155mm). A veia poplítea teve a mediana do diâmetro significativamente aumentado em M3 e M4; 31% (0,5 para 0,655mm) e 43% (0,5 para 0,715mm) respectivamente. Quanto a mediana interquartil, o momento 3 variou negativamente em 9,52% (0,21 para 0,19mm) e no momento 4 teve 8,57% de aumento (0,21 para 0,228mm). As velocidades de fluxo variaram de forma estatisticamente significativa apenas nos vasos femorais. Nos vasos poplíteos houve variação de incremento de fluxo, mas foi não significativa estatisticamente. A artéria femoral apresentou aumentos significativos de velocidade nos momentos 4 e 5. A mediana e a mediana interquartil variaram respectivamente, 31,39% (287,635 para 377,92cm/s) e 51,71% (155,247 para 235,52cm/s), para o momento 4 de 46,09% (287,635 para 420,20cm/s) e 10,09% (155,24 para 170,91cm/s). A mediana da velocidade de fluxo da veia femoral aumentou significativamente, variando 22,22% (239,99 para 293,61cm/s), no momento 4 e 33,63% (239,99 para 320,69cm/s) para o momento 5. Em ambos os momentos a mediana interquartil variou negativamente, 4,64% (199,61 para 190,34cm/s), no momento 4 e 24,92% (199,61 para 149,86cm/s), no momento 5.

The prevalence of peripheral vascular diseases is on the rise, mainly due to the increase in their risk factors, such as the aging of the population, the three with greatest clinical significance being peripheral atherosclerotic obstructive disease (PAOD), chronic venous insufficiency (CVI) and venous thromboembolism (VTE). Muscle physical exercise stimulates circulatory development. But it is not always possible. A clinical, interventional, prospective, single-center, randomized, single-blind study was carried out to evaluate the effect of neuromuscular electrical stimulation in 10 healthy individuals, in which there was an increase in the velocity of arterial and venous blood flow in the lower limbs, an increase in the volume of venous flow in the popliteal vein, increased diameter of femoral and popliteal vessels in healthy individuals and non-significant increase in tissue plasminogen activator. The systemic effects of neuromuscular eletric stimulation (NMES) have also been demonstrated, in the absence of adverse events and significant discomfort.

A prevalência das doenças vasculares periféricas está em ascensão, principalmente devido ao aumento de seus fatores de risco, como o envelhecimento da população, sendo as três com maior significado clínico a doença aterosclerótica obstrutiva periférica (DAOP), a insuficiência venosa crônica (IVC) e o tromboembolismo venoso (TEV). O exercício físico muscular é um estímulo ao desenvolvimento circulatório. Mas nem sempre ele é possível. Foi realizado um estudo clínico, intervencionista, prospectivo, unicêntrico, randomizado, unicego para avaliar o efeito da eletroestimulação neuromuscular em 10 indivíduos saudáveis, em que houve aumento da velocidade de fluxo sanguíneo arterial e venoso de membros inferiores, aumento do volume de fluxo venoso da veia poplítea, aumento do diâmetro dos vasos femorais e poplíteos de indivíduos saudáveis e aumento não significativo do ativador do plasminogênio tecidual. Também se demonstrou os efeitos sistêmicos da estimulação elétrica neuromuscular (EENM), na ausência de eventos adversos e desconforto significativo.
