A prospective, randomized, controlled, double-blind clinical trial with 3 parallel groups. Participants will be randomly allocated in a 1:1:1 ratio using computer-generated randomization sequences (Random Allocation Software, version 1.0.0) by an independent researcher. Allocation will be concealed using opaque, sealed, and sequentially numbered envelopes. Participants will be distributed into three groups of 15 individuals each: Group 1 – active tDCS combined with active FES (tDCS-A/FES-A); Group 2 – active tDCS combined with placebo FES (tDCS-A/FES-P); Group 3 – placebo tDCS combined with active FES (tDCS-P/FES-A). Active tDCS will be applied using the NeuroConn DC-Stimulator (NeuroConn GmbH), using 35 cm² (5 × 7 cm) rubber electrodes positioned on the presumed leg area in the affected hemisphere, laterally and very close to the Cz region (international 10–20 system), with the cathode in the contralateral supraorbital region. Stimulation will be performed at an intensity of 2 mA for 30 minutes, with 5-second ramp-up and ramp-down cycles. In the tDCS placebo group, the electrodes will be positioned identically, with a current of 2 mA applied only for the first 30 seconds, followed by a reduction to 0 mA. FES will be applied during gait training on the ground, using the Dualpex 071 electrostimulator (Quark®), with 3 × 3 cm self-adhesive electrodes positioned over the motor point of the tibialis anterior muscle. In the active FES groups, alternating current with a frequency of 50 Hz, pulse width of 400 μs, on/off times synchronized to the swing phase of gait, and sufficient intensity to promote painless muscle contraction will be used. Stimulation will begin at the start of the swing phase and be interrupted upon heel contact with the ground. In the placebo FES group, minimum intensity will be applied only for sensory perception for 1 minute, being periodically adjusted to zero, without inducing muscle contraction. The interventions will last 30 minutes per session, performed five times a week, for two consecutive weeks. The blinding involved the participants, stage assessors, and the researcher responsible for the statistical analysis.

Os participantes serão distribuídos em três grupos, com 15 indivíduos cada: Grupo 1 – tDCS ativa associada à FES ativa (tDCS-A/FES-A); Grupo 2 – tDCS ativa associada à FES placebo (tDCS-A/FES-P); Grupo 3 – tDCS placebo associada à FES ativa (tDCS-P/FES-A). A tDCS ativa será aplicada por meio do estimulador NeuroConn DC-Stimulator (NeuroConn GmbH), utilizando eletrodos de borracha de 35 cm² (5 × 7 cm), posicionados em cima da área presumida da perna no hemisfério acometido, lateralmente e bem próximo a região Cz (sistema internacional 10–20) e o cátodo na região supraorbital contralateral. A estimulação será realizada com intensidade de 2 mA, durante 30 minutos, com rampas de subida e descida de 5 segundos. No grupo tDCS placebo, os eletrodos serão posicionados de forma idêntica, com aplicação de corrente de 2 mA apenas nos primeiros 30 segundos, seguida de redução para 0 mA. A FES será aplicada durante o treino de marcha em solo, utilizando o eletroestimulador Dualpex 071 (Quark®), com eletrodos autoadesivos de 3 × 3 cm posicionados sobre o ponto motor do músculo tibial anterior. Nos grupos FES ativa, será utilizada corrente alternada com frequência de 50 Hz, largura de pulso de 400 μs, tempos on/off sincronizados à fase de balanço da marcha e intensidade suficiente para promover contração muscular não dolorosa. A estimulação será iniciada no início da fase de balanço e interrompida ao contato do calcanhar com o solo. No grupo FES placebo, será aplicada intensidade mínima apenas para percepção sensorial por 1 minuto, sendo posteriormente ajustada para zero, sem indução de contração muscular. As intervenções terão duração de 30 minutos por sessão, realizadas cinco vezes por semana, durante duas semanas consecutivas. O mascaramento envolverá participantes, avaliadores dos desfechos e pesquisador responsável pela análise estatística.

The tDCS group showed statistical improvement in the parameters of the FMA-LE instruments [3.9 (1.9 – 5.9), p <0.01], TUG [- 2.7 (-5.54 – - 0.14), p = 0.04], and mobility domain of SIS 3.0 [5.77 (0.79 – 10.74), p = 0.03]. In the FES group, a significant improvement was observed in the 6MWT [31.7 (8.98 – 54.41), p = 0.01]; TUG [- 2.71 (-4.61 – -0.80), p <0.01]; 10mWT [0.13 (0.03 – 0.22), p = 0.01]; mini-BESTest [2.50 (0.85–4.19), p = 0.01] and mobility domain [5.77 (0.79–10.74), p = 0.04]. Finally, in the tDCS/FES group, statistical improvement was identified only for the FMA-LE [2.4 (0.2–4.6), p = 0.03] and mini-BESTest [1.50 (0.09–2.90), p = 0.04]. Values expressed as median (interquartile range). No statistical difference was observed between the groups in the post-intervention evaluation, indicating that there was no potentiating effect on improving the outcome results due to the combination of the two neuromodulation techniques.

Introduction: Stroke is a neurological condition that can reduce functional mobility and affect various qualities of life. Central and peripheral neuromodulation strategies using electrical current have been used in isolation to promote functional mobility in patients. However, studies evaluating the combination of these techniques to improve mobility in individuals with classic stroke are scarce. Objective: To evaluate the effects of combining tDCS (Transcranial Direct Current Stimulation) with FES (Functional Electrical Stimulation) on functional gait recovery during the climactic stage of stroke. Methods: Through a randomized, controlled, triple-blind clinical trial, 30 individuals with modern stroke (> 6 months after injury) were allocated to one of three groups (n = 10 each): tDCS/FES, FES, and tDCS. tDCS (2 mA, anodal) was applied at Cz using the 10/20 electroencephalogram system. For FES (25Hz, intensity to generate maximum non-painful contraction), the electrodes were positioned in the region of the tibialis anterior muscle. All participants performed walking training on a flat surface for 30 min, 5x/week for 2 weeks, during the neuromodulation strategies. The following were assessed: functional capacity (6-minute walk test - 6MWT), functional mobility (Timed Up and Go - TUG and 10-meter walk test - 10MWT), balance (Mini-BESTest), lower limb motor function (Fugl-Meyer Scale for lower limbs: FMA-MI), functional independence (modified Barthel Index) and quality of life (domains: activities of daily living - ADL, mobility and participation of the Stroke Impact Scale 3.0 - SIS 3.0). For outcome analysis, one-way ANOVA and repeated-measures ANOVA tests were used for variables that met the assumptions of normality, and the Kruskal–Wallis and Wilcoxon tests for those that did not include a normal distribution, adopting a significance level of p < 0.05. Results: An intragroup analysis revealed that the combined application of tDCS and FES resulted in a significant increase in the FMA-MI and Mini-BESTest scores. The FES protocol, when applied alone, promoted an increase in performance in the 6MWT, TUG, 10MWT, and Mini-BESTest tests, in addition to the mobility domain of the SIS 3.0. In turn, tDCS alone provided gains in the FMA-MI, TUG, and the mobility and activities of daily living (ADL) domains of the SIS 3.0. No significant differences were observed between the groups after the intervention period. Conclusion: The simultaneous application of tDCS and FES did not result in additional benefits to the functional recovery of gait in individuals with dangerous stroke, when compared to the isolated application of each technique.

Métodos: através de um ensaio clíníco, randomizado, controlado, triplo cego, 30 indivíduos com AVC crônico (> 6 meses de lesão) foram alocados em um dos três grupos (n=10 cada): tDCS/FES, FES e tDCS. A tDCS (2mA, anódica) foi aplicada em Cz considerando o sistema 10/20 de eletroencefalograma. Para o FES (25Hz, intensidade para gerar máxima contração não dolorosa) os eletrodos foram posicionados na região do músculo tibial anterior. Todos os participantes realizaram treino de caminhada em superfície plana por 30 min, 5x/semana por 2 semanas, durante as estratégias de neuromodulação. Foram avaliados: a capacidade funcional (teste de caminhada de 6 mim -TC6min), mobilidade funcional (Timed Up and Go – TUG e Teste de caminhada de 10 metros - TC10m), Equilíbrio (Mini-BESTest), função motora de membros inferiores (Escala de Fugl-Meyer para membros inferiores: FMA-MI), independência funcional (Índice de Barthel modificado) e qualidade de vida (domínios: atividade de vida diária – AVD, mobilidade e participação da Stroke Impact Scale 3.0 – SIS 3.0). Para a análise dos desfechos, foram utilizados os testes ANOVA unidirecional e ANOVA de medidas repetidas para variáveis que atenderam aos pressupostos de normalidade, e os testes de Kruskal–Wallis e Wilcoxon para aquelas que não apresentaram distribuição normal, adotando-se nível de significância de p < 0,05. Resultados: A análise intragrupos revelou que a aplicação combinada de tDCS e FES resultou em aumento significativo nos escores do FMA-MI e do Mini-BESTest. O protocolo de FES, quando aplicado isoladamente, promoveu aumento no desempenho nos testes TC6min, TUG, TC10m e Mini-BESTest, além do domínio de mobilidade do SIS 3.0. Por sua vez, a tDCS isolada proporcionou ganhos no FMA-MI, no TUG e nos domínios de mobilidade e de atividades de vida diária (AVD) do SIS 3.0. Não foram observadas diferenças significativas entre os grupos após o período de intervenção. Conclusão: A aplicação simultânea de tDCS e FES não resultou em benefícios adicionais à recuperação funcional da marcha de indivíduos com AVC crônico, quando comparada à aplicação isolada de cada técnica.
