This is a 3-arm, double-blind, randomized clinical study. Sites requiring both inlay and onlay reconstruction were enrolled in the study. In Control Group (CG, n=8), an anorganic xenograft was used in the inlay site and a 1:1 mix of anorganic xenograft and an autograft was positioned in the onlay site. In Test Group 1 (TG1, n=8), both inlay and onlay sites were grafted with the anorganic xenograft associated with the micrografts derived from periosteum. In Test Group 2 (TG2, n=8), both inlay and onlay sites were grafted with the anorganic xenograft associated with the micrografts derived from bone. Six months after the grafting procedures, cone beam computed tomography scans were performed and bone biopsy samples were obtained during implant placement surgery for future rehabilitation. The bone specimens were analyzed histomorphometrically, by measuring the levels of vital mineralized tissue (VMT), non vital mineralized tissue (NVMT) and non mineralized tissue (NMT). Immunohistochemically, the levels of VEGF were evaluated. The patients receiving the different treatments were assigned to groups randomly using the website Randomization.com (http://www.randomization.com), and the random assignment was placed in a sealed opaque envelope. Allocations of the procedure was revealed to investigator (L.A.M.C) according to the sequence of the enrollment. The patients were blinded to the planned treatment and all the analyses were performed by a blind operator (double-blinded study)

Trata-se de um estudo clínico randomizado de 3 braços, duplo-cego. Os locais com demanda para reconstrução Inlay e Onlay foram inscritos no estudo. No grupo controle (GC, n = 8), um xenoenxerto anorgânico foi usado no local do incrustação e uma mistura 1: 1 de xenoenxerto anorgânico e um autoenxerto foi posicionado no local da Onlay. No grupo de teste 1 (GT1, n = 8), os locais de inlay e onlay foram enxertados com o xenoenxerto anorgânico associado aos microxertos derivados do periósteo. No grupo de teste 2 (GT2, n = 8), os locais de inlay e onlay foram enxertados com o xenoenxerto anorgânico associado aos microxertos derivados do osso. Seis meses após os procedimentos de enxerto, foram realizadas exames de tomografia computadorizada de feixe cônico e as amostras de biópsia óssea foram obtidas durante a cirurgia de colocação de implantes para futura reabilitação. As amostras ósseas foram analisadas histomorfometricamente, medindo os níveis de tecido mineralizado vital (TMV), tecido mineralizado não vital (TMNV) e tecido não mineralizado (TNM). Imunohistoquimicamente, os níveis de VEGF foram avaliados. Os pacientes que receberam os diferentes tratamentos foram distribuídos em grupos aleatoriamente por meio do site Randomization.com (http://www.randomization.com), e a alocação aleatória foi colocada em um envelope opaco lacrado. As alocações do procedimento foram reveladas ao investigador (L.A.M.C.) de acordo com a sequência da inscrição. Os pacientes foram cegados para o tratamento planejado e todas as análises foram realizadas por um operador cegado (estudo duplo cego)

Patients of both genders; partially edentulous for more than 1 year; with absence of two teeth at least and at most five at the posterior maxilla (unilateral or bilateral); requiring implants in the posterior maxillary region with no more than 4 mm of remaining alveolar ridge; with need for maxillary sinus floor augmentation combined with onlay vertical bone augmentation in one or both sides; all patients should have between 18 and 65 years of age, with adequate oral health

This study had 19 individuals (6 men and 13 women). Control Group (CG) had 3 men and 3 women, and a mean and standard deviation ages of 52.17 and 8.42, respectively. Test Group 1 (TG1) had 2 men and 5 women, and a mean and standard deviation ages of 59.57 and 11.63, respectively. Test Group 2 (TG2) had 2 men and 4 women, and a mean and standard deviation ages of 60.50 and 14.22, respectively. For the primary outcome newly formed bone percentage (i.e. vital mineralized tissue percentage), in the inlay augmentations, there was no difference (p<0.05) in the means between CG, GT1 and GT2 (31.13%, 31.75% and 31.71%, respectively). However, for onlay augmentations, there was a statistical difference in the means between CG, GT1 and GT2 (46.19%, 43.26% and 32.74%, respectively). For the secondary outcome in the inlay augmentation, there was no difference (p<0.05) in the means between CG, GT1 and GT2 (8.95mm, 8.80mm and 8.65mm, respectively). For onlay augmentation there was also no difference (p<0.05) in the means between CG, GT1 and GT2 (1.72mm, 1.79mm and 1.52mm, respectively)

Purpose: To evaluate the regenerative potential of micrografts, derived from periosteum or bone tissue, added to an anorganic xenograft in reconstruction of the posterior maxilla, requiring an inlay and onlay augmentation. Materials and Methods: After clinical selection and the analysis of CBCT scans, 24 posterior maxillary sites, in 19 patients, were treated by using Barbell Technique. Sites requiring both inlay and onlay reconstruction were enrolled in the study. In the Control Group (CG, n=8), a xenograft was used in the inlay site and for the onlay site, a 1:1 mix of xenograft and an autograft was used. In Test Group 1 (TG1, n=8), both inlay and onlay sites were grafted with the xenograft associated with the micrografts derived from periosteum. In Test Group 2 (TG2, n=8), both inlay and onlay sites were grafted with the xenograft associated with the micrografts derived from bone. Six months after the procedures, CBCT scans were obtained, and bone biopsy samples were harvested during implant placement surgery. The bone specimens were analyzed histomorphometrically, by measuring the percentages of vital mineralized tissue (VMT), non vital mineralized tissue (NVMT) and non mineralized tissue (NMT). Immunohistochemically, the levels of VEGF were categorized by a score approach. Results: Histomorphometric analysis revealed, for the inlay grafts, no significant difference among the groups for VMT, NVMT and NMT. However, for onlay grafts, CG achieved a higher amount of VMT in comparison with TG2, and the opposite occurred for NMT values. In this regard, no statistical difference was observed between CG and TG1. Concerning immunohistochemistry, the VEGF values for CG and TG1 were slightly higher than those obtained by TG2 for both inlay and onlay grafts, but without statistical significance. CBCT analysis showed a similar level of gain for all groups, for both inlay and onlay bone augmentation sites. Clinically, one implant (in CG) within a total of 50 implants installed, had early failure and was replaced after 3 months. All patients received implant supported prosthesis. Conclusion: This study indicated that the clinical use of micrograft derived from periosteum may have some potential to increase bone formation in onlay reconstructions, unlike the micrograft derived from bone tissue

Objetivo: Avaliar o potencial regenerativo de microenxertos, derivados de periósteo ou tecido ósseo, adicionados a um xenoenxerto inorgânico na reconstrução da maxila posterior, necessitando de aumento inlay e onlay. Materiais e Métodos: Após seleção clínica e análise dos exames de tomografias computadorizadas cone beam (TCCB), 24 sítios maxilares posteriores, em 19 pacientes, foram tratados pela Barbell Technique. Locais que necessitavam de reconstrução inlay e onlay foram incluídos no estudo. No Grupo Controle (GC, n=8), foi utilizado xenoenxerto no sítio inlay e para o sítio onlay foi utilizada uma mistura 1:1 de xenoenxerto e enxerto autógeno. No Grupo Teste 1 (GT1, n=8), tanto os locais inlay quanto onlay foram enxertados com o xenoenxerto associado aos microenxertos derivados do periósteo. No Grupo de Teste 2 (TG2, n=8), tanto os locais inlay como onlay foram enxertados com o xenoenxerto associado aos microenxertos derivados de osso do túber. Seis meses após os procedimentos, foram obtidas TCCB e amostras de biópsia óssea foram coletadas durante a cirurgia de instalação do implante. Os espécimes ósseos foram analisados histomorfometricamente, medindo-se as porcentagens de tecido vital mineralizado (TVM), tecido não vital mineralizado (TNVM) e tecido não mineralizado (TNM). Imunohistoquimicamente, os níveis de VEGF foram categorizados por uma abordagem de pontuação. Resultados: A análise histomorfométrica não revelou, para os enxertos inlay, qualquer diferença significativa entre os grupos para TVM, TNVM e TNM. Porém, para os enxertos onlay, o GC obteve maior quantidade de TVM em comparação ao GT2, e o contrário ocorreu para os valores de TNM. Nesse aspecto, não foi observada diferença estatística entre GC e GT1. Em relação à imunohistoquímica, os valores de VEGF para GC e GT1 foram ligeiramente superiores aos obtidos pelo GT2 tanto para enxertos inlay quanto onlay, mas sem significância estatística. A análise da TCCB mostrou um nível semelhante de ganho para todos os grupos, tanto para locais de aumento ósseo inlay quanto onlay. Clinicamente, um implante (no GC), de um total de 50 implantes instalados, apresentou falha precoce e foi substituído após 3 meses. Todos os pacientes receberam prótese suportada por implante. Conclusão: Este estudo indicou que o uso clínico de microenxerto derivado de periósteo pode ter algum potencial para aumentar a formação óssea em reconstruções onlay, diferentemente do microenxerto derivado de tecido ósseo
