This is a randomized, prospective, longitudinal, single-blind clinical study (the patient did not know if he was receiving the studied intervention protocol). The elderly were randomized, through the website www.randomization.com, to participate in one of two groups: intervention, which performed training with a handgrip, or a control group, which was not submitted to training with a handgrip and performed only a few stretches. A sample of 18 individuals per group was considered. Thus, 36 elderly were recruited and evaluated. The assessments carried out were: assessment of the five frailty criteria (assessment of handgrip strength, gait speed, unintentional weight loss, level of physical activity and report of exhaustion), assessment of heart rate variability (Polar V800) and blood pressure at rest and during 3 minutes of isometric handgrip exercise at 30% of maximum voluntary contraction force. After the evaluations, the elderly were randomized to one of two groups. After randomization, the elderly will be submitted to a strength training protocol (handgrip) or stretching (control). Intervention group: 18 elderly people participated in the Isometric Handgrip Training protocol with Handgrip. The elderly participants used the handgrip (rubber device) to carry out the protocol. The elderly person was seated and with the dominant upper limb, he interspersed 5 seconds of handgrip maintained using his maximum voluntary contraction and 5 seconds of rest. 32 repetitions were performed, totaling 5 minutes of duration, every day, for 30 consecutive days. Control group: 18 elderly people who participated in a stretching protocol, in which they were instructed to perform a series of active movements on the dominant upper limb, consisting of the following movements: wrist flexion, fingers and wrist extension; flexion and extension of the 2nd to 5th finger of the hand; with 10 repetitions each. Ending with stretching of the wrist extensor and flexor muscles, with maintenance of 12 seconds each. The elderly performed this protocol for a total of 5 minutes. All volunteers in this study received explanations about the respective protocol through explanatory videos and a folder with illustrations and instructions about it, as well as having their doubts clarified

Trata-se de um estudo clínico randomizado, prospectivo, longitudinal e unicego (o paciente não sabia se estava recebendo o protocolo de intervenção estudado). O idoso foi randomizado, através do site www.randomization.com, para participar de um dos dois grupos: intervenção, que realizava treinamento com handgrip, ou grupo controle, que não era submetido a treinamento com handgrip e realizava apenas alguns alongamentos. Foi considerada uma casuística de 18 indivíduos por grupo. Desta forma, 36 idosos foram recrutados e avaliados. As avaliações realizadas foram: avaliação dos cinco critérios da fragilidade (avaliação da força de preensão manual, velocidade da marcha, perda de peso não intencional, nível de atividade física e relato de exaustão), avaliação da variabilidade da frequência cardíaca (Polar V800) e da pressão arterial em repouso e durante 3 minutos de exercício isométrico de preensão manual a 30% da força de contração voluntária máxima. Após as avaliações, os idosos foram randomizados para um dos dois grupos. Após randomização, os idosos serão submetidos ao protocolo de treinamento força (preensão manual) ou de alongamento (controle). Grupo intervenção: 18 idosos participaram do protocolo de Treinamento Isométrico de Preensão Manual com Handgrip os idosos participantes utilizaram o handgrip (aparelho de borracha) para a execução do protocolo. O idoso estava sentado e com o membro superior dominante ele intercalava 5 segundos de preensão manual mantida utilizando sua contração voluntária máxima e 5 segundas de descanso. Foram realizadas 32 repetições, totalizando 5 minutos de duração, todos os dias, durante 30 dias consecutivos. Grupo controle: 18 idosos que participaram de um protocolo de alongamentos, no qual eram orientados a executarem uma série de movimentos ativos no membro superior dominante, composto pelos seguintes movimentos: flexão de punho, dedos e extensão de punho; flexão e extensão do 2º ao 5º dedo da mão; com 10 repetições cada. Finalizando com alongamento dos músculos extensores e flexores de punho, com manutenção de 12 segundos cada. Os idosos realizavam este protocolo totalizando 5 minutos de duração. Todos os voluntários deste estudo receberam explicações sobre o respectivo protocolo através de vídeos explicativos e um folder com ilustrações e instruções sobre o mesmo, bem como ter suas dúvidas esclarecidas

509 elderly people from the Integrated Health Center of the city of São Paulo were screened, but 451 elderly people were not able to participate in the study due to difficulties with traveling to the location of the evaluations, refused to participate in the research or had difficulties caused by the COVID pandemic -19. Of the 58 seniors selected, 22 were excluded for not completing the assessments, presenting collection errors in heart rate variability signals or completing isometric training with handgrip, totaling 36 elderly participants in the study who completed one of the protocols (training or control) and performed the final evaluations

As a primary outcome, the heart rate variability of the elderly in the pre- and post-intervention moments of the fragile training (TF) and fragile control (FC) groups is presented. There is a significant reduction in the LF index (nu), (TF: 59.69±8.20 vs. CF: 52.84±5.29; p≤0.001) and in cardiac autonomic balance (TF: 1.61 ±0.58 vs. FC: 1.16±0.24; p≤0.002). In addition to a significant increase in the HF (nu) index (TF: 40.25±8.19 vs. CF: 46.92±5.31; p≤0.002) only in the TF group. There was no significant difference in the CF group in terms of time domain, frequency or non-linear analysis. Regarding the results of hemodynamic variables, the main effect was the reduction in systolic blood pressure, which occurred only in the TF group (Pre: 124± 12.35 vs. Post: 120± 12.83; p=0.044). However, regarding diastolic blood pressure, there was a statistical tendency towards reduction (Pre:75±4.85 vs. Post: 73±5.66; p=0.059). Regarding the control group, there was no change in systolic blood pressure (Pre: 120± 12.62 vs. Post: 121± 11.63; p=0.180) nor in diastolic blood pressure (Pre: 76±7.57 vs. Post : 77±6.62; p=0.317). There were no significant changes to heart rate in either group

Introduction: Based on the premise that physical training can provide benefits to the elderly, being able to reduce cardiac and cardiac sympathetic activity and blood pressure and increase cardiac parasympathetic activity, making it pertinent to understand the effects caused by isometric training with handgrip, never studied in this fragile population, to consider whether this type of exercise when performed by the elderly is efficient and safe from an autonomic and hemodynamic point of view. Objective: To evaluate the effect of isometric handgrip training on heart rate variability (HRV) and blood pressure (BP) in frail elderly people. Methods: After screening 509 elderly people from the Integrated Health Center in the city of São Paulo, 36 elderly people (33 women and 3 men) with a mean age of 69±7.36 years, classified as independent elderly according to the criteria of Fried et al. (2001). The elderly were randomized to the isometric training with handgrip (IHT) or control group, for a period of 30 consecutive days. Before and after this period, the RR interval was evaluated using the polar V800 and the HRV was observed using the Kubios HRV Standard. Blood pressure was measured in the remainder, by the auscultatory method. Results: There was a significant reduction after isometric training in the LF (nu) index (p<0.001) and in the autonomic balance (p<0.001), in addition to a significant increase in the HF (nu) index (p<0.001). In the control group, there was no change in HRV in any of the parameters. There was a reduction in SBP only in the TF group (p=0.044), while in the control group there were no changes. There were no changes for DBP or HR in any of the groups, although DBP in the TF group had a tendency to decrease (p=0.059). Conclusion: There was an improvement in HRV after isometric training with a handgrip, with a reduction in sympathetic modulation and an increase in cardiac parasympathetic modulation. Furthermore, isometric training was associated with SBP, which was moderately correlated with increased cardiac parasympathetic modulation in these patients

Introdução: Partindo da premissa que a o treinamento físico pode proporcionar benefícios aos idosos sendo capaz de reduzir a atividade simpática cardíaca, frequência cardíaca e pressão arterial e aumentar a atividade parassimpática cardíaca, torna-se pertinente entender os efeitos crônicos promovidos pelo treinamento isométrico com handgrip, nunca estudado nesta população frágil, para considerar se essa modalidade de exercício quando executada por idosos frágeis é eficiente e segura do ponto de vista autonômico e hemodinâmico. Objetivo: Avaliar o efeito do treinamento isométrico com handgrip sobre a variabilidade da frequência cardíaca (VFC) e pressão arterial (PA) em idosos frágeis. Métodos: Após triagem de 509 idosos do Centro Integrado de Saúde da cidade de São Paulo foram avaliados 36 idosos (33 mulheres e 3 homens) com idade média de 69±7,36 anos, classificados como idosos frágeis segundos os critérios de Fried et al (2001). Os idosos foram randomizados para o grupo treinamento isométrico com handgrip (TIH) ou grupo controle, por um período de 30 dias consecutivos. Antes e após este período, foram avaliados o intervalo RR pelo polar V800 e analisada a VFC pelo Kubios HRV Standard. A pressão arterial foi medida no repouso, pelo método auscutatório. Resultados: Houve uma redução significativa após treinamento isométrica no índice LF (nu) (p<0,001) e no balanço autonômico (p<0,001), além de um aumento significativo no índice HF (nu) (p<0,001). No grupo controle não houve alteração na VFC em nenhum dos parâmetros. Houve redução da PAS apenas no grupo TF (p=0,044), enquanto no grupo controle não houve alterações. Não houve alterações significativas para PAD nem FC em nenhum dos grupos, embora a PAD no grupo TF tenha tido uma tendência a redução (p=0,059). Conclusão: Houve melhora na VFC após o treinamento isométrico com handgrip, com redução na modulação simpática e aumento da modulação parassimpática cardíaca. Além do mais, o treinamento isométrico reduziu a PAS, o que foi moderadamente correlacionado com o aumento da modulação parassimpática cardíaca destes pacientes
