The participants of this study were obtained through a convenience sample from a database of the Center for Obesity Studies of the Psychiatry Institute of the Hospital das Clínicas, Faculty of Medicine, USP. Patients who underwent bariatric surgery for more than 2 years at this Institute were selected through the medical records, in order to identify those who had regained weight. Of the total of 142 patients, 33 were identified who presented weight regain after bariatric surgery, and the criterion for weight regain adopted was a 15% increase in total weight from the lowest weight achieved (nadir). After excluding 13 participants unable to participate, the remaining 20 were randomly divided into two groups using the excel tool (=random between), which were blind as to which group would participate. The first (Group 1 – Intervention) was initially composed of 10 participants, underwent 8 sessions of Cognitive-Behavioral Therapy in an online group (CBGT/O). The second (Group 2 – Control), initially composed of 10 patients, attended three online lectures (CG/O). Participants in both groups were evaluated before (T0) and after the intervention (T1).

Os participantes deste estudo foram obtidos por meio de uma amostra por conveniência a partir de um banco de dados do Núcleo de Estudos da Obesidade do Instituto de Psiquiatria do Hospital das Clínicas da Faculdade de Medicina da USP. Foram selecionados por meio dos prontuários, pacientes que realizaram a cirurgia bariátrica há mais de 2 anos neste Instituto visando identificar os que tiveram reganho de peso. Do total de 142 pacientes, foram identificados 33 que apresentaram reganho de peso pós cirurgia bariátrica, sendo que o critério para reganho de peso adotado foi o de aumento de 15% do peso total a partir do menor peso atingido (nadir). Após a exclusão de 13 participantes impossibilitados de participar, os 20 restantes foram divididos aleatoriamente em dois grupos através da ferramenta do excel (=aleatorioentre), os quais eram cegos a respeito de qual grupo participariam. O primeiro (Grupo 1 – Intervenção) foi composto inicialmente por 10 participantes, passou por 8 sessões de Terapia Cognitivo-Comportamental em grupo on-line (TCCG/O). O segundo (Grupo 2 – Controle), composto inicialmente por 10 pacientes, assistiu três palestras on-line (GC/O). Os participantes de ambos os grupos foram avaliados, antes (T0) e após a intervenção (T1).

The sample is predominantly composed of female patients (93%), with a mean age of 53 years, ranging from 34 to 70 years, married (43%), with years of schooling between 8 and 12 years (57%) and who work (64%).The mean time after bariatric surgery was 96 months (~8 years), ranging from 52 to 109 months (~4 to 9 years). The mean BMI was equal to 35.4 kg/m2, ranging from 26.8 to 45.6 kg/m2.The mean weight regain was equal to 33%, with half of the patients having at least 32% regain. We have no evidence of significant differences in demographic and clinical characteristics between groups.

In the second evaluation (T1) there is a decrease of, on average, 3.36 points (CI(95%)=[-6.8; 0.089], p=0.0554) and 3.36 points (CI(95%)=[-5.43; -1.29 ], p=0.0039), respectively, in the mean levels of depression and binge eating, compared to the first assessment (T0), regardless of the group. We have no evidence of the effect of TCCG/O on levels of depression (p=0.93) and binge eating (p=0.72), regardless of the time of assessment. It was not possible to detect difference in the level of anxiety (p=0.72) between the groups, regardless of the moment of evaluation, and it was also not possible to detect difference in the level of anxiety (p=0.17), between the moments of evaluation, regardless of the group. In the TCCG/O group, only two patients gained weight compared to T0, with half of the patients maintaining or losing weight (median=0%). In the control group, all patients gained or maintained weight compared to T0, with half of the patients gaining at least 0.65% (<1%) of their weight at T0 (median=-0.65%). However, it was not possible to detect a significant difference for weight variation between groups (p=0.10).

OBJECTIVE: to investigate the effects of cognitive-behavioral group therapy (CBT) through online intervention with patients who regained weight two years after bariatric surgery. METHOD: a convenience sample was obtained from a University Bariatric Surgery Center. Patients who underwent bariatric surgery more than 2 years ago at this institute were selected through the medical records, in order to verify those who had regained weight. Of the total of 142 patients, 33 were identified who had regained 15% of the total weight from the lowest weight achieved after the surgical procedure (nadir). Thirteen participants who were unwilling to participate in the intervention were excluded. The 20 patients who met the weight regain criterion adopted in the study were randomly divided into two groups: Group 1 (Intervention), underwent 8 sessions of online Cognitive-Behavioral Group Therapy (TCCG/O) and Group 2 (Control), watched three online lectures (GC/O). The participants of both groups were evaluated before (T0) and after the intervention (T1). An assessment of symptoms of depression (BDI-II), anxiety (GAD-7), binge eating (BES) and body mass variations was performed. RESULTS: there was a significant reduction in symptoms of depression and binge eating in both groups, but no variations in body mass and anxiety symptoms were identified. CONCLUSION: this study indicated the feasibility of two short-term intervention modalities using the online cognitive-behavioral approach in patients who underwent bariatric surgery and regained weight.

Foram selecionados por meio dos prontuários, pacientes que realizaram a cirurgia bariátrica há mais de 2 anos neste instituto visando verificar os que tiveram reganho de peso. Do total de 142 pacientes, foram identificados 33 que apresentaram reganho de 15% do peso total a partir do menor peso atingido após o procedimento cirúrgico (nadir). Foram excluídos 13 participantes que não apresentaram disponibilidade de participar da intervenção. Os 20 pacientes que apresentavam o critério de reganho de peso adotado no estudo foram divididos aleatoriamente em dois grupos: O Grupo 1 (Intervenção), passou por 8 sessões de Terapia Cognitivo-Comportamental em grupo on-line (TCCG/O) e o Grupo 2 (Controle), assistiu três palestras on-line (GC/O). CONCLUSÃO: este estudo indicou a viabilidade de duas modalidades de intervenção de curto prazo pela abordagem cognitivo-comportamental on-line em pacientes que realizaram a cirurgia bariátrica e apresentaram reganho de peso.
