  Thirty selected subjects will be divided into 2 groups of 15 individuals each by random sequence using Research Randomizer software. The experimental group (15 subjects) will undergo bimaxilar orthognathic surgery with mentoplasty. In the post-operative period will be used anti-inflammatory (standardized) and also will be applied to low-power laser therapy. Anti-inflammatories will be used intravenously during the hospital stay (3 days) and orally in the postoperative period (5 days). Low power laser therapy will be performed in the immediate postoperative period in the post-anesthetic recovery, and in the hospital bed with 24, 48 hours, only in an extra oral form, as there is a difficulty of mandibular movements. The extra-oral points will be arranged in the mandibular branch and body with 8 points on each side, 2 points in each preauricular region, digastric and submandibular lymph nodes, bilaterally in the region of 9 points, upper and lower lip 4 points each and paranasal region 3 points each side. After hospital discharge, the applications will occur intra and extra-oral, twice a week until 8 weeks, totaling 19 sessions. The intra-oral points will be arranged bilaterally in the groove bottom region near the incision of the maxilla and mandible with a total of 8 points in the maxilla bilaterally and 12 points bilaterally in the mandible. The equipment will be previously calibrated, with wavelength of 808nm (infra-red), power of 100 mW, energy of 3.0 J / point of irradiation, irradiation time of 30 seconds per point, with energy density of 100 J / cm2 during, with beam area of ??0.03 cm2. The application method will be used punctually, in contact with the skin or mucosa. The control group (15 subjects) will undergo bimaxillary orthognathic surgery with mentoplasty with the use of standardized anti-inflammatory drugs and placebo protocol of laser therapy in the same times and periods of the experimental group.

  Trinta sujeitos selecionados serão divididos em 2 grupos de 15 indivíduos, cada um por seqüência aleatória usando o software Research Randomizer. O grupo experimental (15 indivíduos) passará por cirurgia ortognática bimaxilar com mentoplastia. No período pós-operatório será utilizado anti-inflamatório (padronizado) e também será aplicado a terapia a laser de baixa potência. Antiinflamatórios serão utilizados por via intravenosa durante a internação (3 dias) e por via oral no pós-operatório (5 dias). A terapia com laser de baixa potência será realizada no pós-operatório imediato na recuperação pós-anestésica e no leito hospitalar com 24, 48 horas, apenas de forma oral extra, pois há dificuldade de movimentos mandibulares. Os pontos extra-orais serão dispostos em ramo e corpo mandibular com 8 pontos em cada lado, 2 pontos em cada região pré-auricular, linfonodos digástrico e submandibular. Na região do mento bilateralmente em 09 pontos, lábio superior e inferior 4 pontos cada e região paranasal 3 pontos cada lado. Após a alta hospitalar, as aplicações ocorrerão intra e extra-oral, duas vezes por semana até 8 semanas, totalizando 19 sessões. Os pontos intra-orais serão organizados bilateralmente na região inferior do sulco, perto da incisão da maxila e da mandíbula com um total de 8 pontos na maxila bilateralmente e 12 pontos bilateralmente na mandíbula. O equipamento será previamente calibrado, com um comprimento de onda de 808nm (infravermelho), potência de 100 mW, energia de 3,0 J / ponto de irradiação, tempo de irradiação de 30 segundos por ponto, com densidade de energia de 100 J / cm2 durante área de feixe de 0,03 cm2. O método de aplicação será usado pontualmente, em contato com a pele ou mucosa.   Return of mandibular movements more quickly, in patients submitted to bimaxilar orthognathic surgery with mentoplasty, after phototherapy with low power laser. The mandibular movements will be measured in millimeters during passive opening, passive right and left laterality and active protrusion. These measurements will be performed with the aid of a digital caliper in the preoperative period and in the postoperative period of 7, 14, 30 and 60 days. Statistical tests will be applied to compare the groups.

Measurements of edema in millimeters with the use of a malleable ruler in the immediate postoperative period, 7, 14, 30 and 60 days, based on cephalometric points, will be evaluated: these are the distances between the corner of the eye and the gonion, between the tragus and the labial commissure and between the tragus and the pogonium of each patient. The right and left sides of both groups will also be evaluated, being the sum of the sides considered.   Menor quantidade de Edema, em pacientes submetidos à cirurgia ortognática bimaxilar com mentoplastia, após a fototerapia com laser de baixa potência. Serão avaliadas as medidas do edema em milímetros com a utilização de uma régua maleável no pós-operatório imediato, 7, 14, 30 e 60 dias, baseadas em pontos cefalométricos, sendo estas: as distâncias entre o canto do olho e o gônion, entre o tragus e a comissura labial e entre o tragus e o pogônio de cada paciente. Também serão avaliados os lados direito e esquerdo de ambos os grupos, sendo considerada as somatórias dos lados. Esses valores serão anotados em ficha padrão, serão aplicados testes estatísticos para comparação entre os grupos.
