This is a two-arm, double-blind, randomized controlled study. In the experimental group: individuals living with HIV/AIDS will receive information about HPV and the HPV vaccine, prepared based on the Theory of Motivation for Protection (TMP) in a web intervention developed for mobile devices. In the control group: individuals living with HIV/AIDS will receive a synopsis of information from the page maintained by the Ministry of Health dedicated to informing the population about HPV and the HPV vaccine. The experimental and control intervention will be offered to a total sample with an estimated size of at least 396 participants in a randomized manner in a 1:1 ratio.

Trata-se de um estudo randomizado controlado de dois braços, duplo-cego. No grupo experimental: os indivíduos vivendo com HIV/AIDS receberão informações sobre o HPV e a vacina HPV, elaboradas sob as bases da Teoria da Motivação para Proteção (TMP) numa intervenção web desenvolvida para dispositivos móveis. No grupo controle: os indivíduos vivendo com HIV/AIDS receberão uma sinopse de informações da página mantida pelo Ministério da Saúde dedicada a informar a população sobre o HPV e a vacina HPV. A intervenção experimental e a controle serão oferecidas a uma amostra total com tamanho estimado de pelo menos 396 participantes de forma aleatorizada na relação de 1:1.

Background: Human papillomavirus (HPV) causes several cancers that disproportionally affect people living with HIV/AIDS (PLWH) yet there is a paucity of research on interventions to foster HPV vaccine use in this population. We sought to evaluate the efficacy of a web-based, tailored intervention (e-HPV) to promote HPV vaccination among PLWH. Methods: this is a randomized controlled trial with PLWH aged 18 to 45 years. Participants were recruited between January and June 2022 and randomized into two groups: experimental group (e-HPV), which received information about HPV and the HPV vaccine, based on the Protection Motivation Theory and control group, who received a clipping of information from the page maintained by the Ministry of Health dedicated to informing the population about HPV and the HPV vaccine. The primary and secondary outcomes were the percentage of PLWH willing to get HPV vaccine and HPV vaccine initiation (i.e., receipt of any doses by PLWH), respectively. Results: a total of 654 individuals were randomly allocated: 327 in the e-HPV and 327 in the control group. The average age was 29.7 years, the majority were men (71.4%), black or mixed race (63.2%). The intention to get vaccinated against HPV was approximately twice as high among participants in the e-HPV vs. control group (OR=2.0, 95% CI: 1.3–3.4; p<0.003), and HPV vaccine initiation was also significantly more common among participants in the e-HPV group (OR=2.1, 95% CI: 1.1-4.0; p=0.03). Belief in the effectiveness of the HPV vaccine, risk perception and the severity of an HPV infection were the reasons most reported by participants intending to get HPV vaccination. Conclusions: The intervention was acceptable and efficacious in increasing HPV vaccination among PLWH. Future studies are warranted to optimize and disseminate the e-HPV intervention to settings providing services to PLWH.

Métodos: trata-se de um ensaio clínico randomizado com PVHA de 18 a 45 anos. Os participantes foram recrutados entre janeiro e junho de 2022 e randomizados em dois grupos: grupo experimental (e-HPV), que recebeu informações sobre o HPV e a vacina contra o HPV, com base na Teoria da Motivação de Proteção e grupo controle, que recebeu um recorte de informações do página mantida pelo Ministério da Saúde dedicada a informar a população sobre o HPV e a vacina contra o HPV. Os resultados primários e secundários foram a percentagem de PVHS dispostas a receber a vacina contra o HPV e o início da vacina contra o HPV (ou seja, recebimento de quaisquer doses pelas PVHS), respectivamente. Resultados: foram alocados aleatoriamente 654 indivíduos: 327 no e-HPV e 327 no grupo controle. A média de idade foi de 29,7 anos, a maioria eram homens (71,4%), pretos ou pardos (63,2%). A intenção de se vacinar contra o HPV foi aproximadamente duas vezes maior entre os participantes do e-HPV versus grupo controle (OR=2,0, IC 95%: 1,3–3,4; p<0,003), e o início da vacina contra o HPV também foi significativamente mais comum entre participantes do grupo e-HPV (OR=2,1, IC 95%: 1,1-4,0; p=0,03). A crença na eficácia da vacina contra o HPV, a percepção de risco e a gravidade de uma infecção pelo HPV foram os motivos mais relatados pelos participantes que pretendiam se vacinar contra o HPV. Conclusões: A intervenção foi aceitável e eficaz no aumento da vacinação contra o HPV entre PVHS. Estudos futuros são necessários para otimizar e disseminar a intervenção e-HPV em ambientes que prestam serviços a PVH.
