This is a three-arm randomized controlled clinical trial with a 6-month evaluation period. Simple randomization was performed using an electronically generated table, dividing the patients into: G1 - group that underwent intra-articular injection (IIA) with 3ml (60mg) of Triamcinolone Hexacetonide (20mg/ml) followed by a program of progressive resistance exercises (PERP) (n=18 performed /52 expected); G2 - group that performed IIA with 3ml of saline solution followed by the same PRSP (n=23 performed /52 predicted) and G3 - group that performed IAI placebo, followed by the same PRSP (n=18 performed /52 predicted). Allocation secrecy was performed using opaque and sealed envelopes. IAI was performed by a single rheumatologist on the most symptomatic knee, with the patient in the supine position and with the lower limb in extension. The needle entry access was two centimeters from the superolateral angle of the patella, submitted to eversion, with a 40x8mm needle. In a first syringe, we used 2% lidocaine without vasoconstrictor only for anesthesia of the path, if there is difficulty in the intra-articular approach to the knee. After the needle was inserted into the intra-articular environment, arthrocentesis was systematically performed in the knees with joint effusion, and then injected the medication. However, in the placebo group IIA, arthrocentesis was not performed and no medication was injected. All material used in the IIA procedure was sterile and handled away from the patient's visual reach. All patients were instructed to rest for 48 hours with a bandage applied to the knee and use dipyrone 750mg every 8 hours, on demand. Patients in the 3 groups were submitted to a PRSP consisting of a 10-minute stationary bicycle warm-up and four lower limb muscle strengthening exercises that were started one week after the IIA. The evaluator of the primary and secondary outcomes was blinded to the treatment group, as well as the patient and the physical therapist who applied the PERP were blinded to the IIA performed. The IIA was performed only once, right at the beginning of the intervention and the PRSP had a frequency and twice a week being performed for 24 weeks.

Trata-se de um ensaio clinico controlado e randomizado de três braços, com período de avaliação de 6 meses. Foi realizada aleatorização simples por meio de uma tábua gerada eletronicamente dividindo os pacientes em: G1 - grupo que realizou injeção intra-articular (IIA) com 3ml (60mg) de Hexacetonide de Triancinolona (20mg/ml) seguido de um Programa de exercícios resistidos progressivos (PERP) (n=18 realizados /52 previstos); G2 - grupo que realizou IIA com 3ml de solução salina seguido do mesmo PERP (n= 23 realizados /52 previstos) e G3 - grupo que realizou a IIA placebo, seguido do mesmo PERP (n=18 realizados /52 previstos). O sigilo de alocação foi realizado por meio de envelopes opacos e selados. A IIA foi realizada por um único médico reumatologista no joelho mais sintomático, com o paciente em decúbito dorsal e com o membro inferior em extensão. O acesso de entrada da agulha foi a dois centímetros do ângulo súpero-lateral da patela, submetida à eversão, com agulha de 40x8mm. Em uma primeira seringa, foi utilizada lidocaína a 2% sem vasoconstrictor apenas para anestesia do trajeto, caso exista dificuldade na abordagem intra-articular do joelho. Após a entrada da agulha no ambiente intra-articular, foi realizada sistematicamente artrocentese nos joelhos com derrame articular, e em seguida, injetada a medicação. Entretanto, no grupo IIA placebo não foi realizada a artrocentese e não foi injetada qualquer medicação. Todo o material utilizado no procedimento da IIA foi estéril e manipulado longe do alcance visual do paciente. Todos os pacientes foram orientados a fazer repouso articular de 48h com atadura no joelho aplicado e o uso de dipirona 750mg de 8/8h, sob demanda. Os pacientes dos 3 grupos foram submetidos a um PERP composto por aquecimento em bicicleta estacionária por 10 minutos e quatro exercícios de fortalecimento muscular dos membros inferiores que foi iniciado após uma semana da realização da IIA. O avaliador dos desfechos primário e secundários foi cego para o grupo de tratamento, assim como, o paciente e a fisioterapeuta que aplicou o PERP foram cegos quanto a IIA realizada. A IIA foi realizada uma única vez, logo no início da intervenção e o PERP tinha uma frequência e 2 vezes por semana sendo realizado por 24 semanas.

Both genders; Age equal to or greater than 60 years; Diagnosis of Osteoarthritis of the knee according to American College of Rheumatology criteria (Altman et al. 2000), diagnosed by a rheumatologist; Kellgren/Lawrence radiographic classification (0-IV) between II and III (Kellgren, Lawrence, 1957); Pain between 4 and 8 centimeters on the 10 centimeter numerical pain scale in at least one knee; Symptom duration for more than 3 months; Stable medication for knee osteoarthritis in the last 3 months; Agreement and signature of the free and informed consent form for this study.

They were diagnosed with inflammatory arthritis, gout, pseudogout, fibromyalgia, psychopathies and decompensated cardiopathies or neuropathies that affected the lower limbs; Diabetes mellitus or decompensated systemic arterial hypertension; Physiotherapy or acupuncture in the last 3 months; Initiated or changed regular physical activity in the last three months; Started or changed use of walking aids in the last three months; Skin lesion that prevented intra-articular injection in the knee to be studied; intra-articular injection with corticosteroids or hyaluronic acid derivative in the studied knee in the last 3 months and in any other joint in the last month; Previous or scheduled surgery for the next 6 months; Severe clotting disorder; Suspected bacterial infection of any nature; Difficulty in locomotion and understanding/taking the tests; litigation.

Clinical and demographic characteristics demonstrated the homogeneity of the sample for all parameters evaluated: age, gender, education, duration of illness, BMI, ethnicity, marital status, chronic fall, presence of comorbidities and radiological grade K/L II and III. As well as for all primary outcomes (END for pain at rest and movement) and secondary (edema, WOMAC pain, stiffness, function and total, SF-36, TUGT, 6MWT, 1RM, SPPB, Likert scale. The IIA CE + PERP group had a sample with a mean age ± SD of 70.5 ± 5.5; 93.3% were female, mean ± SD of years studied of 6.7 ± 3.4; mean BMI ± SD of 30.95 ± 5.42; 60% of white ethnicity; 60% married; 26.7% were chronic fallers; comorbidities (66.7% - SAH, 53.3% - DM and 53.3% DLP); 53.3% had radiological grade K/L II and 46.7% K/L III. For the primary outcome, mean ± SD for END pain at rest was 2.8 ± 3.1 and pain at movement was 7.7 ± 1.7. For the secondary outcomes, the mean ± SD for WOMAC pain, stiffness, function and total was 11.07 ± 3.6; 4.4 ± 1.6; 39.1±14.1 and 54.6±18.4, respectively. The mean ± SD for each SF-36 subscale showed 34.0 ± 22.8 for functional capacity, 41.7 ± 34.9 for limitations due to physical aspects, 36.8 ± 15.9 for pain, 56.7 ± 19.0 for general health status, 52.3 ± 21.5 for vitality, 50.8 ± 26.1 for social aspects, 36.2 ± 45.0 for emotional aspects and 56.8 ± 22.0 for health mental. For the evaluation of edema, the knee circumference was evaluated and presented a mean ± SD of 38.6 ± 4.4 centimeters. The TUGT showed a mean ± SD of 9.12 ± 3.79. The 6-minute walk test showed a mean±SD of 368.1 ± 108.2. SPPB had a mean ± SD of 10.0 ± 1.41. The 1 RM was performed for 4 muscle groups, with a mean ± SD of 18.1 ± 13.2 for the knee extensor, 15.0 ± 8.8 for the knee flexor, 22.9 ± 15.7 for the knee adductor. hip and 33.2 ± 17.4 for hip abductor. The IIA SS + PERP group had a sample with a mean age ± SD of 71.3 ± 5.6; 86.7% were female, mean ± SD of years studied of 6.7 ± 3.1; mean BMI ± SD of 29.31 ± 3.89; 80% of white ethnicity; 66.7%% married; 13.3% were chronic fallers; comorbidities (66.7% - SAH, 53.3% - DM and 66.7% DLP); 40.0% had radiological grade K/L II and 60.0% K/L III. For the primary outcome, mean ± SD for END pain at rest was 4.2 ± 3.4 and pain at movement was 6.1 ± 3.1. For secondary outcomes, mean ± SD for WOMAC pain, stiffness, function and total of 9.53 ± 5.01; 3.73 ± 2.28; 33.3 ± 15.2 and 46.6 ± 21.3, respectively. The mean ± SD for each SF-36 subscale showed 47.3 ± 22.2 for functional capacity, 45.0 ± 43.5 for limitations due to physical aspects, 41.7 ± 23.5 for pain, 51.5 ± 21.3 for general health status, 59.0 ± 26.4 for vitality, 65.0 ± 31.1 for social aspects, 42.2 ± 46.2 for emotional aspects and 57.9 ± 25.8 for health mental. For the evaluation of edema, the knee circumference was evaluated and presented a mean ± SD of 38.8 ± 3.7 centimeters. The TUGT showed a mean ± SD of 9.43 ± 5.03. The 6-minute walk test showed a mean±SD of 392.5±99.1. SPPB had a mean ± SD of 9.07 ± 2.4. The 1 RM was performed for 4 muscle groups, with a mean ± SD of 14.4 ± 5.9 for the knee extensor, 11.9 ± 5.4 for the knee flexor, 19.3 ± 8.7 for the knee adductor. hip and 30.8 ± 11.0 for hip abductor. Group IIA placebo + PERP had a sample with mean age ± SD was 71.9 ± 7.4; 73.3% were female, mean ± SD of years studied of 7.7 ± 3.2; mean BMI ± SD of 29.31 ± 4.61; 86.7% of white ethnicity; 60.0%% married; 13.3% were chronic fallers; comorbidities (80.0% - SAH, 26.7% - DM and 46.7% DLP); 53.3% had radiological grade K/L II and 46.7% K/L III. As características clínicas e demográficas demonstraram a homogeneidade da amostra para todos os parâmetros avaliados: idade, gênero, escolaridade, tempo de doença, IMC, etnia, estado civil, caidor crônico, presença de comorbidades e graduação radiológica K/L II e III. Assim como, para todos os desfechos primários (END para dor ao repouso e ao movimento) e secundários (edema, WOMAC dor, rigidez, função e total, SF-36, TUGT, TC6´, 1RM, SPPB, escala Likert. O grupo IIA CE + PERP apresentou uma amostra com idade média ± DP foi de 70,5 ± 5,5; 93,3% do gênero feminino, média ± DP de anos estudados de 6,7 ± 3,4; média de IMC ± DP de 30,95 ± 5,42; 60% de etnia branca; 60% casados; 26,7% eram caidores crônicos; comorbidades (66,7% - HAS, 53,3% - DM e 53,3% DLP); 53,3% tinha graduação radiológica K/L II e 46,7% K/L III. Para o desfecho primário apresentou média ± DP para END dor ao repouso de 2,8 ± 3,1 e ao movimento de 7,7 ± 1,7. During this study, we had the COVID-19 pandemic, which made it difficult to include new patients and caused losses in the follow-up. A total of 96 individuals were eligible, 37 of whom were excluded (did not want to undergo IIA (n=13); did not meet the inclusion criteria (n=10); did not want to participate (n=6), other reasons (surgery, fx, travel ) (n=5) and pandemic (n=3)). Fifty-nine patients were randomized to one of 3 groups, 18 in the CE IIA group, 23 in the SS IIA group, and 18 in the placebo group IIA. Of these, there was 1 dropout in the placebo group IIA due to family reasons and 1 in the EC group IIA due to a personal problem, both at T6. The absences that we had during the evaluations carried out at T2, T6, T12 and T24 were all due to the impossibility of going to the place on that occasion, such as, for example, flu, tooth extraction, dermatological microsurgery, fall or other personal problems. Regarding the pandemic, we had losses throughout the study, with 14 dropouts in T6 and T12 and 29 dropouts in T24. Due to the difficulty of inclusion and also these absences in the reassessments after the beginning of the pandemic, we chose to perform the statistical analysis per protocol only with those who finished the PRSP in T12, that is, 45 participants, 15 for each group.

Durante a realização deste estudo tivemos a pandemia de COVID-19 o que dificultou a inclusão de novos pacientes e fez com que ocorressem perdas no seguimento. Foram elegíveis, 96 indivíduos, sendo 37 excluídos (não quiseram realizar a IIA (n=13); não preencheram os critérios de inclusão (n=10); não quiseram participar (n=6), outros motivos (cirurgia, fx, viagem) (n=5) e pandemia (n=3)). Cinquenta e nove pacientes foram randomizados para um dos 3 grupos, sendo 18 no grupo IIA CE, 23 no grupo IIA SS e 18 no grupo IIA placebo. Desses, houve 1 desistência no grupo IIA placebo por motivo familiar e 1 no grupo IIA CE por problema pessoal, ambos no T6. As faltas que tivemos ao longo das avaliações realizadas nos tempos T2, T6, T12 e T24 foram todas por impossibilidade de ir até o local naquela ocasião, como por exemplo, gripe, extração de dente, microcirurgia dermatológica, queda ou outros problemas pessoais. COm relação a pandemia tivemos perdas ao longo do estudo, sendo 14 desistências em T6 e T12 e 29 desistências em T24. Devido à dificuldade de inclusão e também essas faltas nas reavaliações após o início da pandemia, optamos por realizar a análise estatística por protocolo somente com os que terminaram o PERP em T12, ou seja, 45 participantes, sendo 15 para cada grupo.

Desfecho 8: No SPPB não encontramos diferenças significativas no comportamento dos três grupos ao longo do tempo (p interação = 0,183). Background: Osteoarthritis (OA) is highly prevalent in the elderly. Both IIA with triancinolone hexacetonide (TH) and the prescription of resistance exercise are frequent practices that have been proven to be effective in the management of patients with knee OA. However, there are no studies that evaluate the effectiveness of the association of these two interventions in these patients. Objective: To evaluate the effectiveness of the association of IIA CE with a PPRE in improving pain, function, muscle strength and quality of life in elderly patients with knee OA. Methods: Elderly patients diagnosed with knee OA, both genders, with pain between 4 and 8 cm on a 10 cm numerical pain
scale, having symptoms for at least 3 months and stable medication for OA for 3 months were included. The elderly were randomized into 3 intervention groups: IIA CE + PPRE, IIA SS + PPRE and IIA placebo + PPRE. The PPRE was performed for 12 weeks, twice a week. Assessments of pain (NPS), function (WOMAC), quality of life (SF-36), and performance tests (6' walk test, TUGT, FTSS and SPPB) were performed at baseline, 2, 6, and 12 weeks after IIA by a blinded evaluator.
Results: Due to the epidemiological crisis (COVID-19), only 15 patients were allocated to each group. In the initial evaluation, the groups were homogeneous for all parameters. Intergroup analysis, using repeated measures ANOVA, found no statistically significant difference between groups over time for all variables of pain, function, quality of life, and functional tests. In the intragroup analysis, there was an improvement in the NPS for the 3 groups (p<0.001) over time, WOMAC (all domains (p<0.001)), 1RM (p<0.001), SPPB (<0.007) and SF- 36 for all domains except vitality. Conclusion: CE IIA added to a 12-week PPRE twice weekly was not effective in improving pain, function, and quality of life in elderly patients with knee OA.pt-br
Introdução: A osteoartrite (OA) tem uma alta prevalência nos idosos, sendo o joelho a articulação mais comum acometida nessa população. Tanto a IIA com hexacetonida de triancinolona (CE), como a prescrição de exercício resistido, são práticas frequentes e comprovadamente efetivas no manejo de pacientes com OA de joelhos. No entanto, não existem estudos que avaliem a efetividade da associação dessas duas intervenções nesses pacientes. Objetivo: Avaliar a efetividade da associação da IIA CE, a um programa de exercícios resistidos progressivos (PERP) na melhora da dor, função, força muscular e qualidade de vida em idosos com OA de joelhos. Métodos: Foram incluídos idosos com diagnóstico de OA de joelho, ambos os gêneros, com dor entre 4 e 8 cm em uma escala numérica de dor de 10 cm, ter sintomas há pelo menos 3 meses e medicação estável para OA há 3 meses. Os idosos foram randomizados em 3 grupos de intervenção: IIA CE + PERP, IIA SS + PERP e IIA placebo + PERP. O PERP foi realizado por 12 semanas, 2x por semana. As avaliações de dor (END), função (WOMAC), qualidade de vida (SF-36), e os testes de desempenho (teste de caminhada de 6’, TUGT, FTSS e SPPB) foram realizados no início do estudo, 2, 6 e 12 semanas após a IIA por um avaliador cego. Resultados: Devido à crise epidemiológica (COVID-19), apenas 15 pacientes foram alocados para cada grupo. Na avaliação inicial os grupos foram homogêneos para todos os parâmetros. A análise intergrupos, usando o ANOVA de medidas
repetidas, não encontramos diferença estatisticamente significante entre os grupos ao longo do tempo para todas as variáveis de dor, função, qualidade de vida e testes funcionais. Na análise intragrupos, houve melhora nas variáveis END dor para os 3 grupos (p<0,001) ao longo do tempo, WOMAC (todos os domínios (p<0,001)), 1RM (p<0,001), SPPB (<0,007) e SF-36 para todos os domínios, exceto vitalidade. Conclusão: A IIA CE adicionada a um PERP de 12 semanas, 2x por semana, não foi efetiva na melhora da dor, função e qualidade de vida em idosos com OA de joelhos.
