Interventions

• Interventions:

  This is a randomized, triple-blind, controlled clinical trial with patients who underwent surgical treatment after elbow fractures. Patients will be divided into 2 groups: the intervention group with 25 participants of both sexes (therapeutic ultrasound and protocol of the Physiotherapy service) and the control group with 25 participants (sham therapeutic ultrasound and protocol of the Physiotherapy service). The ultrasound equipment that will be used is the Sonopulse III from Ibramed, with an effective radiation area of ​​7 square centimeters and a non-uniformity rate of 4, with a frequency of 3 megahertz, continuous mode. The therapeutic ultrasound intervention will be performed twice a week for 5 consecutive weeks, with a total study time of 10 weeks. In the intervention group with 25 participants, the therapeutic ultrasound will be turned on with the abovementioned parameters and at an intensity of 1 watt per square centimeter with a total emission power of 7 watts. In the control group with 25 participants, therapeutic ultrasound will be turned on at 0.0 watts per square centimeter. Ultrasound will be applied in circular movements in the cubital fossa, anterior region of the elbow (estimated area 56 square centimeters) for 8 minutes, using aqueous gel as a form of coupling and in the posterior region (35 square centimeters) for 5 minutes. The patient will be instructed not to report any sensation to the physiotherapist, except pain or excessive heat. Two experienced physiotherapists will participate in the intervention and the entire protocol will be carried out at the Physiotherapy outpatient clinic of the University Hospital of Juiz de Fora individually. The physiotherapy intervention protocol used in both groups is described by Standard Operating Procedure 046 of the Physiotherapy service of the Multidisciplinary Unit of the Federal University Hospital of Juiz de Fora, and is available attached on the REBEC platform. The outcome evaluator, the data analyst and the patients will be blinded to the allocated groups, as will the two physiotherapists who will participate in the intervention. A researcher not involved in the assessment, intervention, and data analysis will enter the parameters into the therapeutic ultrasound equipment and then cover the screen with paper and tape so that the physiotherapist cannot see them. The physiotherapist will start the ultrasound application by pressing enter and will finish after hearing the equipment's end-of-intervention sound. Patient randomization will be performed using an algorithm developed for the statistical software R Core Team 2021, Vienna, Austria and performed by one of the researchers not involved in recruitment, treatment, and assessment, in addition to maintaining allocation confidentiality until the end of the study. The collected data will be stored in opaque envelopes, duly numbered, ensuring masking and confidentiality of the information. Adverse events during the intervention period will be recorded throughout the study, as well as treatment adherence, in addition to the level of patient satisfaction at the end of the study. Elbow range of motion and functionality will be assessed, as well as pain and palmar grip strength (baseline, tenth week post-operatively, fourteenth week post-operatively and at follow-up after 6 months)

  Trata-se de um ensaio clínico controlado aleatorizado, triplo cego, com pacientes que foram submetidos a abordagem cirúrgica após fraturas de cotovelo. Os pacientes serão alocados em 2 grupos, sendo o grupo intervenção 25 participantes de ambos os sexos (ultrassom terapêutico e protocolo do serviço de Fisioterapia) e o grupo controle 25 participantes (ultrassom terapêutico sham e protocolo do serviço de Fisioterapia). O equipamento de ultrassom que será utilizado é o Sonopulse III da Ibramed, com área de radiação efetiva de 7 centímetros quadrados e taxa de não uniformidade de 4, com frequência de 3 mega hertz , modo contínuo. A intervenção com ultrassom terapêutico será realizada 2 vezes por semana por 5 semanas consecutivas, sendo o tempo total do estudo de 10 semanas. No grupo intervenção com 25 participantes, o ultrassom terapêutico estará ligado nos parâmetros mencionados anteriormente e na intensidade de 1 watt por centímetro quadrado e com potência total de emissão de 7 watts. No grupo controle com 25 participantes, o ultrassom terapêutico estará ligado com uma intensidade de 0.0 watt por centímetro quadrado. O ultrassom será aplicado em movimentos circulares na fossa cubital, região anterior do cotovelo (área estimada 56 centímetros quadrados por um período de 8 minutos, sendo utilizado gel aquoso como forma de acoplamento e na região posterior (35 centímetros quadrados) por 5 minutos. O paciente será instruído a não relatar qualquer sensação ao fisioterapeuta, exceto dor ou calor excessivo. Dois fisioterapeutas experientes participarão da intervenção e todo o protocolo será realizado no ambulatório de Fisioterapia do Hospital Universitário de Juiz de Fora de forma individual. O protocolo de intervenção fisioterapêutica utilizado nos dois grupos é descrito pelo Procedimento Operacional Padrão 046 do serviço de Fisioterapia da Unidade Multiprofissional do Hospital da Universidade Federal de Juiz de Fora, encontra- se disponível em anexo na plataforma REBEC . O avaliador dos desfechos, o analista de dados e os pacientes estarão cegos quanto aos grupos alocados, assim como os dois fisioterapeutas que participarão da intervenção. Um pesquisador não envolvido com a avaliação, intervenção e análise dos dados vai inserir os parâmetros no equipamento de ultrassom terapêutico e depois ocluirá a tela com um papel e fita adesiva para que o fisioterapeuta não consiga visualizar. O fisioterapeuta iniciará a aplicação do ultrassom pressionando o enter e terminará após ouvir o som de término da intervenção do equipamento. A randomização dos pacientes será realizada por meio de algoritmo desenvolvido para o software estatístico R Core Team 2021, Viena, Áustria e realizada por um dos pesquisadores não envolvidos com o recrutamento, tratamento e avaliação, além do mesmo manter o sigilo de alocação até o final do estudo. Os dados coletados serão armazenados em envelopes opacos, devidamente numerados, garantindo o mascaramento e a confidencialidade das informações. Eventos adversos durante o período de intervenção serão registrados ao longo do estudo, bem como a adesão do tratamento, além do nível de satisfação do paciente ao final do estudo. Serão avaliadas amplitude de movimento e funcionalidade do cotovelo, além da dor e força de preensão palmar (linha de base, décima semana do pós -operatório, décima quarta semana do pós operatório e no acompanhamento após 6 meses)

  Individuals aged 18 to 70 years; of both sexes; referred for physiotherapy treatment from the sixth week of surgical intervention after elbow fractures; with limited range of motion greater than or equal to twenty degrees of extension deficit; or less than or equal to one hundred and twenty degrees of elbow flexion; elbow fractures involving the distal segment of the humerus; proximal segment of the ulna; fracture of the neck and head of the radius according to Mason's Classification; elbow fractures and dislocations such as Hotchkiss's terrible triad; Monteggia lesion; transolecranon fracture and dislocation

  Patients with bilateral elbow fractures or whose indicated treatment is total elbow arthroplasty; pregnant women; neurological, rheumatic or psychiatric diseases that compromise rehabilitation; previous history of elbow stiffness; previous surgical procedure on the elbow; existence of Kirschner wires or cerclage wires in the proximal ulna and distal humerus; Mini-Mental State Examination score of less than 24 points for those with higher education and 18 points with low to moderate level of education
