This is a randomized, controlled, parallel and open clinical trial, which was carried out simple randomization, through the website www.random.org. by another researcher not linked to this project. Experimental group: 25 children between 4 and 6 years, 11 months and 29 days, who received an instructional therapeutic toy session before the intramuscular vaccine and used the virtual reality glasses during the vaccine (After placing the virtual reality glasses on the participant, and after 60 seconds of the video, the vaccine was administered, and the child remained with the glasses for another 40 seconds after the vaccine, until completing 2 minutes of the video). Control group: 25 children between 4 and 6 years, 11 months and 29 days old, who received an instructional therapeutic play session and the vaccine before the intramuscular vaccine and the vaccine without using virtual reality glasses. The intervention in the experimental group lasted 15 minutes and in the control group 10 minutes.

The approach of the two groups took place through the initial reception of the child and the guardian in the vaccination units by the Researcher, later the eligibility criteria were applied, and if the child fit the study criteria, the objectives and method were explained, invited to participate in the study and asked to read and sign the Free and Informed Consent Form and the Free and Informed Assent Form, when applicable. After signing these, the child and guardian were taken to a free room in the Unit and an instructional therapeutic play session took place. During the instructional therapeutic toy session, the intervention group was also taught how to put on the immersive virtual reality glasses. After clarifying the doubts, the child was invited to perform the intramuscular vaccine on a doll, if he accepted. After the instructional therapeutic toy session, the child and guardian were taken to the vaccination room and the application of the study instrument began, by checking the behavioral variables, as well as characterizing the child and the procedure. The child was positioned comfortably and with easy access for the intramuscular vaccine. The Quest 2 Meta® immersive virtual reality glasses were placed, the video started and the time was set to a stopwatch by the researcher in the intervention group. After 60 seconds, the intramuscular vaccine was administered by the Nursing Technician. The intramuscular vaccine lasted approximately 20 seconds. After removing the needle, the child remained wearing immersive virtual reality glasses for another 40 seconds until completing 2 minutes. When removing the immersive virtual reality glasses, the behavioral variables were applied again and the Pain Face Scale – Revised by the Researcher was used. Subsequently, the researcher asked the child four questions: Would you recommend that friends take the vaccine using immersive virtual reality glasses? Did you enjoy getting the vaccine using the immersive virtual reality glasses? Would you like to use immersive virtual reality glasses in other vaccines? What was the pain presented during the intramuscular procedure using the Faces Pain Scale – Revised? And, the guardian was asked two questions: Did you identify a difference in the child's perception of pain when compared to the memory of previous intramuscular vaccines? What was the child's pain during the intramuscular vaccine using the Faces of Pain Scale – Revised. The video used in the study is available free of charge on YouTube via the link: https://www.youtube.com/watch?v=m2L-G1tbQ2k. It is worth noting that in cases where the child needed to receive more than one vaccine, it was requested and emphasized to the child and the guardian to evaluate the pain only from the first intramuscular vaccine, as there was no interruption of the process between vaccines. Furthermore, the child was given a certificate of courage and a balloon at the end of the vaccine. In the control group, the same process was carried out, but without the use of immersive virtual reality glasses. However, after finishing, in addition to giving the child a certificate of courage and the balloon, the child was allowed to use the immersive virtual reality glasses, if they wanted. The planned initial sample was 25 participants for the intervention group and 25 for the control group, totaling 50 participants. After randomization, two children were excluded from the intervention group, as they did not accept using the immersive virtual reality glasses, and they presented fear and anxiety related to not knowing the device. In the end, 23 children remained in the intervention group and 25 children in the control group.

A abordagem dos dois grupos aconteceu por meio do acolhimento inicial da criança e do responsável nas unidades de vacinação pelo Pesquisador, posteriormente foi aplicado os critérios de elegibilidade, e caso a criança se encaixasse nos critérios do estudo era explicado os objetivos e método, convidado a participar do estudo e solicitado a leitura e assinatura do Termo de Consentimento Livre e Esclarecido e Termo de Assentimento Livre e Esclarecido, quando aplicável. Após a assinatura destes a criança e o responsável eram encaminhados para uma sala livre da Unidade e realizado uma sessão de brinquedo terapêutico instrucional. Durante a sessão do brinquedo terapêutico instrucional também foi ensinado ao grupo intervenção como colocar o óculos de realidade virtual imersiva. Após esclarecer as dúvidas a criança era convidada a realizar a vacina intramuscular em um boneco, caso aceitasse. Após a sessão do brinquedo terapêutico instrucional a criança e o responsável eram encaminhados para sala de vacinação e iniciado a aplicação do instrumento do estudo, por meio da verificação das variáveis do comportamento, assim como era realizado a caracterização da criança e do procedimento. A criança era posicionada de forma confortável e com fácil acesso para realização da vacina intramuscular. Colocado o óculos de realidade virtual imersiva Quest 2 Meta®, iniciado o vídeo e o tempo em cronômetro pelo pesquisador no grupo intervenção. Após passar 60 segundo era realizado a vacina intramuscular pelo Técnico em Enfermagem. A vacina intramuscular durou aproximadamente 20 segundos. Após a retirada da agulha a criança permaneceu por mais 40 segundos com óculos de realidade virtual imersiva até completar 2 minutos. Ao retirar o óculos de realidade virtual imersiva foi aplicado novamente as variáveis do comportamento e utilizado a Escala de Faces de Dor – Revisada pelo Pesquisador. Posteriormente, o Pesquisador realizou quatro perguntas a criança: Você recomendaria para amigos tomar a vacina utilizando o óculos de realidade virtual imersiva? Você gostou de tomar a vacina usando o óculos de realidade virtual imersiva? Você gostaria de utilizar o óculos de realidade virtual imersiva em outras vacinas? Qual foi a dor apresentada durante o procedimento intramuscular utilizando a Escala de Faces de Dor – Revisada? E, ao responsável foi realizado duas perguntas: Você identificou diferença na percepção dolorosa da criança quando comparada com lembrança de vacinas intramusculares anteriores? Qual foi a dor da criança durante a vacina intramuscular utilizando a Escala de Faces de Dor – Revisada. O vídeo utilizado no estudo encontra-se disponível de forma gratuita no Youtube pelo link: https://www.youtube.com/watch?v=m2L-G1tbQ2k. Cabe ressaltar, que nos casos em que a criança necessitou receber mais de uma vacina, foi solicitado e frisado para a criança e o responsável avaliar a dor apenas da primeira vacina intramuscular, pois não houve o interrompimento do processo entre as vacinas. Ainda, foi dado a criança um certificado de coragem e um balão ao término da vacina. No grupo controle foi realizado o mesmo processo, porém sem a colocação do óculos de realidade virtual imersiva. Porém, após o término além de dar a criança um certificado de coragem e o balão, foi deixado que a criança usasse o óculos de realidade virtual imersiva, caso quisesse. A amostra inicial planejada foi de 25 participantes para o grupo intervenção e 25 para o grupo controle, totalizando 50 participantes. Após a randomização foram excluídas duas crianças do grupo intervenção, pois não aceitaram utilizar o óculos de realidade virtual imersiva, a qual apresentaram medo e ansiedade relacionado ao fato de não conhecer o dispositivo. Ao final permaneceu 23 crianças grupo intervenção e 25 crianças do grupo controle.

Primary outcome: the presence of pain was verified as children in the intervention group reported not feeling pain in 17 (74%) vaccine applications, while the control group reported not feeling pain in 6 (24%) applications, thus achieving statistical significance ( p value < 0.001; Relative Risk 0.52; confidence interval 0.35-0.72) between using immersive virtual reality and the traditional clinical method. And, when carrying out the association and reliability analysis between the variables, the agreement coefficient of Cohen's Kappa test showed that there was a moderate agreement of 0.749 between the children's pain report and that of the responsible researcher, which was congruent when carried out Cohen's Kappa test regarding the child's report of pain and the guardian's perception, which obtained a moderate agreement of 0.706. However, when performing Cohen's Kappa test between the observations of the researcher and those responsible, it demonstrated that there was a strong agreement of 0.957. Secondary outcomes were obtained through the children's behavior before and during intramuscular vaccination. However, during intramuscular vaccination the most evident behaviors in the control group compared to the intervention group were: whining 88%, crying 68%, acting nervously 92%, screaming 64%, struggling 76%, requiring restraint 88 %, protested 92%, spoke during the procedure 76% and requested information during intramuscular vaccination 68%. Since, in the secondary outcome, confidence intervals, amplitude, variance and standard deviation were not calculated.

No entanto, ao realizar o teste de Kappa de Cohen entre as observações do pesquisador e dos responsáveis, demostrou que houve uma concordância forte de 0,957. Desfecho secundário foram obtidos por meio do comportamento das crianças antes e durante a vacinação intramuscular. Vaccination is an effective, available and reliable intervention that prevents infectious and debilitating diseases. However, it occurs more frequently in childhood, which can contribute to the development of trauma. Therefore, non-pharmacological interventions can be used to try to avoid this type of reaction, as virtual reality is one of them, as it allows the child to be immersed in a virtual, three-dimensional environment that provides distraction and reduces pain perception. Check feasibility of a pilot study of a randomized clinical trial on the effectiveness of immersive virtual reality glasses in terms of children's pain perception associated with intramuscular vaccination in relation to the traditional method. Pilot study of a randomized, controlled, parallel and open clinical trial, carried out in four vaccination units in Southern Brazil, between August and November 2023, with children between four and six years, 11 months and 29 days. The intervention group used immersive virtual reality glasses during the intramuscular vaccine, while the control group received the traditional clinical method, with both groups receiving the instructional therapeutic toy to prepare children during the vaccine. The association between the variables was verified using Pearson's chi-square test (X2) or Fisher's exact test with Confidence Intervals (CI) of 95%, and a significance level of 5% (p-value≤0.05 ). Furthermore, the agreement between the pain reported by the child and the pain observed by the researcher and guardian was verified using Cohen's Kappa test. And, the criteria to test the feasibility of the protocol were: eligibility, recruitment, protocol fidelity; retention/friction; and, missing data, as well as the sample size was calculated. The study was approved by the Ethics Committee under opinion number 6,045,606. Of the 50 eligible participants, 48 were randomized and included in the final analysis, 23 children allocated to the intervention group and 25 to the control group. Furthermore, it was found that 17 (74%) children in the intervention group stated that they did not feel pain during the intramuscular vaccination using immersive virtual reality glasses, while only 6 (24%) children in the control group reported that they did not feel pain. However, there was no statistical difference when comparing the perception of severe pain reported by the children compared to the researcher's observation and the guardian's observation. Yelling and whining behavior were lower in the intervention group. In this pilot study there was a statistical difference between the groups in relation to pain reduction, since the control group had more severe pain, as well as the behaviors during the procedure were more intense.

A vacinação é uma intervenção efetiva, disponível e confiável que previne contra doenças infecciosas e debilitantes. No entanto, ocorre com maior frequência na infância, o qual pode contribuir para o desenvolvimento de traumas. Assim, intervenções não farmacológicas podem ser utilizadas para tentar evitar este tipo de reação, visto que a realidade virtual é uma delas, pois permite que a criança fique imersa em um ambiente virtual e tridimensional que proporciona distração e diminuição da percepção dolorosa.Verificar a viabilidade de um estudo piloto de um ensaio clínico randômico sobre a efetividade do óculos de realidade virtual imersiva quanto a percepção dolorosa da criança associada a vacinação intramuscular em relação ao método tradicional. Estudo piloto de ensaio clínico randômico, controlado, paralelo e aberto, realizado em quatros unidades de vacinação no Sul do Brasil, entre agosto e novembro de 2023, com crianças entre quatro e seis anos, 11 meses e 29 dias. O grupo intervenção utilizou o óculos de realidade virtual imersiva durante a vacina intramuscular, enquanto o grupo controle recebeu o método clínico tradicional, sendo que em ambos os grupos receberam o brinquedo terapêutico instrucional para o preparo da crianças durante a vacina. A associação entre as variáveis foi verificada por meio do teste do qui-quadrado (X2) de Pearson ou Exato de Fisher com Intervalos de Confiança (IC) de 95%, e nível de significância de 5% (p-valor≤0,05). Ainda, se verificou a concordância da dor relatada pela criança e a dor observada pelo pesquisador e responsável por meio do teste de Kappa de Cohen. E, os critérios para testar a viabilidade do protocolo foram: elegibilidade, recrutamento, fidelidade do protocolo; retenção/atrito; e, dados ausentes, assim como, foi calculado o tamanho amostral. O estudo foi aprovado no Comitê de Ética sob o número do parecer 6.045.606. Dos 50 participantes elegíveis, 48 foram randomizados e incluídos na análise final, 23 crianças alocadas no grupo intervenção e 25 para o grupo controle. Ainda, foi verificado que 17 (74%) crianças do grupo intervenção afirmaram não sentir durante a vacinação intramuscular utilizando o óculos de realidade virtual imersiva, enquanto apenas 6 (24%) crianças do grupo controle relatou não sentir dor. Porém, não houve diferença estatística quanto a comparação da percepção da dor forte relatada pelas crianças comparado com a observação do pesquisador e com a observação do responsável. O comportamento gritar e choramingar foram menores no grupo intervenção. Neste estudo piloto houve diferença estatística entre os grupos em relação a redução da dor, visto que, o grupo controle apresentou mais dor forte, assim como os comportamentos durante o procedimento foram mais intensos.
