It is a three-arm, single-blind, randomized controlled clinical trial. The study included a total of 30 children, randomly allocated into three groups: Control (n=10), SENA (n=10), and TAAC (n=10). Participant allocation to the groups occurred through age-matched randomization. The children were organized into pairs of the same age range, and within each pair, they were randomly allocated to one of the three groups by a simple draw conducted by the responsible researcher. Regarding masking (blinding), the study was conducted using a single-blind model. The participants were not aware of which intervention they were receiving (SENA, TAAC) or if they were in the control group. However, the researcher responsible for applying the interventions and assessments was aware of the group allocation due to the practical nature of the interventions, which prevented blinding of the assessor. The experimental groups (SENA and TAAC) underwent their respective interventions, while the Control group received no intervention during the study period, participating only in the assessments at the three established time points (T1, T2, and T3). Upon study completion, all participants in the Control group received appropriate intervention, in accordance with ethical principles. The intervention program comprised three assessment time points: Time 1 (T1, pre-intervention), Time 2 (T2, post-first phase), and Time 3 (T3, post-second phase). The assessments included audiometric thresholds, behavioral tests of auditory processing (GIN, PSI-MCI, TPD, and TDD), visual perception assessment (DTVP-2), the SAB Scale, and auditory evoked potentials (FFR and P300). Visuomotor Stimulation consisted of activities involving spatial orientation, balance, segmental awareness of limbs, and visuomotor coordination. The training consisted of ten sessions of 45 minutes each, once a week, in groups of five children. The Acustically Controlled Auditory Training (TAAC) consisted of auditory tasks with progressive complexity and gradual reduction of the signal-to-noise ratio. Conducted in an acoustic booth, TAAC used headphones for dichotic presentation, with specific rules for the starting ear (verbal: right; non-verbal: left). Progression between stages was based on performance (≥70% correct to advance; <30% to go back). Each phase comprised 10 weekly sessions of 45 minutes, totaling approximately 8 hours of sensory stimulation per phase. SENA (NeuroAuditory Stimulation System) intervention used musical stimuli (left ear) and speech/narrative stimuli (right ear), subjected to frequency modulation and a low-pass filter to prevent habituation. The intensity was individually calibrated based on audiometric thresholds, with an average of 60 dBNS. The sessions were conducted in a controlled environment, without concomitant activities. Trata-se de um estudo clínico randomizado controlado de três braços, simples-cego. O estudo incluiu um total de 30 crianças, randomicamente alocadas em três grupos: Controle (n=10), SENA (n=10) e TAAC (n=10). A alocação dos participantes aos grupos ocorreu por meio de randomização pareada por idade. As crianças foram organizadas em pares de mesma faixa etária e, dentro de cada par, alocadas aleatoriamente a um dos três grupos por sorteio simples, conduzido pela pesquisadora responsável. Quanto ao mascaramento, o estudo foi conduzido no modelo de simples-cego (single-blind). Os participantes não tinham conhecimento de qual intervenção estavam recebendo (SENA, TAAC) ou se estavam no grupo controle. A pesquisadora responsável pela aplicação das intervenções e avaliações, no entanto, tinha ciência da alocação dos grupos devido à natureza prática das intervenções, o que impossibilitou o cegamento do avaliador. Os grupos experimentais (SENA e TAAC) foram submetidos às suas respectivas intervenções, enquanto o grupo Controle não recebeu intervenção durante o período do estudo, participando apenas das avaliações nos três momentos estabelecidos (T1, T2 e T3). Ao término do estudo, todos os participantes do grupo Controle receberam intervenção adequada, conforme princípios éticos. O programa de intervenção compreendeu três momentos de avaliação: Tempo 1 (T1, pré-intervenção), Tempo 2 (T2, pós-primeira fase) e Tempo 3 (T3, pós-segunda fase). As avaliações incluíram limiares audiométricos, testes comportamentais do processamento auditivo (GIN, PSI-MCI, TPD e TDD), avaliação da percepção visual (DTVP-2), a Escala SAB e os potenciais evocados auditivos (FFR e P300). A Estimulação Visuomotora consistiu em atividades de orientação espacial, equilíbrio, consciência segmentária dos membros e coordenação visuomotora. O treinamento consistiu em dez sessões de 45 minutos cada, uma vez por semana, em grupos de cinco crianças. O Treinamento Auditivo Acusticamente Controlado (TAAC) consistiu em tarefas auditivas com complexidade progressiva, com redução gradual da relação sinal-ruído. Realizado em cabina acústica, o TAAC utilizou fones para apresentação dicótica, com regras específicas para orelha de início (verbal: direita; não-verbal: esquerda). A progressão entre as etapas era baseada no desempenho (≥70% de acertos para avançar; <30% para retornar). Cada fase compreendeu 10 sessões semanais de 45 minutos, totalizando aproximadamente 8 horas de estimulação sensorial por fase. A intervenção SENA (Sistema de Estimulação NeuroAuditiva) utilizou estímulos musicais (orelha esquerda) e de fala/narrativa (orelha direita), submetidos a modulação de frequência e filtro passa-baixo para evitar habituação. A intensidade foi calibrada individualmente com base nos limiares audiométricos, com média de 60 dBNS. As sessões eram realizadas em ambiente controlado, sem atividades concomitantes. The study began with 31 recruited participants. However, one participant passed away before the start of the procedures, preventing their inclusion in the assessments and interventions. Thus, 30 participants were effectively allocated to the three groups through age-matched randomization, distributed equally among the Control (n = 10), SENA (n = 10), and TAAC (n = 10) groups. All 30 participants who began the study completed the assessments at all three time points (T1, T2, and T3) and, in the SENA and TAAC groups, completed both phases of intervention (20 sessions in total). There were no dropouts, exclusions, or additional losses throughout the study. The analyses of the primary and secondary outcomes demonstrated distinct effects among the SENA, TAAC, and combined interventions. In the FFR, a decrease in Wave A was observed in the combined groups (TAAC+SENA and SENA+TAAC) compared to the control group. Waves D, E, and F showed increases in all intervention groups relative to the control, whereas Wave O increased in the TAAC and combined groups, with no difference for SENA alone. Wave V decreased in the combined groups, and all intervention groups demonstrated an additive effect on the VA wave. For the P300, protocols that included TAAC, either alone or combined, reduced latency in both ears. SENA alone did not produce significant changes. Amplitude showed no relevant differences among groups. In the APD behavioral tests, significant differences were observed in PSI, TPD, and TPD-N for groups receiving TAAC alone or combined. The GIN and TDD tests did not show significant changes, indicating no measurable effect of the interventions on these aspects. The visual perception analysis showed significant effects in the groups receiving SENA alone or combined, varying according to the subtest: PVG, IVM, and PMR. TAAC alone did not present significant differences compared to the control group. The SAB Scale indicated that combined interventions significantly improved most items across all evaluated domains (hearing, comprehension, learning, and attention). Isolated interventions resulted in more restricted gains, with specific effects on certain items: TAAC (items 1, 6, and 11) and SENA (item 4). Data were expressed as arithmetic means and analyzed using Two-Way Analysis of Variance with Fisher’s post hoc test (LSD), considering p ≤ 0,05 as statistically significant

A Escala SAB indicou que as intervenções combinadas melhoraram significativamente a maioria dos itens em todos os domínios avaliados (audição, compreensão, aprendizagem e atenção). This study investigated the efficacy of two therapeutic procedures, Acoustically Controlled Auditory Training (ACAT) and NeuroAuditory Stimulation System (SENA), analyzed individually and in combination, to verify the application order on the stimulation effects.Thirty children, aged 8 to 11 years, equally distributed between sexes and matched by age, participated. All were evaluated before and after the interventions through pure tone audiometry, behavioral tests of auditory processing, dichotic listening (DLT), monotic listening (PSI), temporal resolution (GIN) and temporal ordering (TPD), short (FFR) and long (P300) auditory evoked potentials, in addition to the Visual Perception Assessment (DTVP-2) and the SAB Scale. In the isolated intervention, ACAT was effective in electrophysiological procedures (FFR and P300), auditory behavioral tests (PSI and TPD), and the SAB Scale. SENA protocol, a passive auditory stimulation, showed positive effects on the FFR, the PSI test, and item 4 of the SAB Scale. The combined interventions demonstrated greater efficacy, especially when ACAT was administered before SENA. The results reinforce the combined use of the protocols as a promising approach for auditory and cognitive rehabilitation, promoting behavioral and neurophysiological advances, besides expanding the understanding of the mechanisms of neural plasticity and its implications for the learning process

Este estudo investigou a eficácia de dois procedimentos terapêuticos, o Treinamento Auditivo Acusticamente Controlado (TAAC) e o Sistema de Estimulação NeuroAuditiva (SENA), analisados individualmente e combinados entre si, de forma a verificar a influência da ordem de aplicação nos efeitos da estimulação. Participaram 30 crianças igualmente distribuídas entre os sexos e pareadas por idade de 8 a 11 anos. Todas foram avaliadas antes e depois das intervenções por meio de audiometria tonal, testes comportamentais de processamento auditivo, escuta dicótica (TDD), escuta monótica (PSI), resolução temporal (GIN) e ordenação temporal (TPD), potenciais evocados auditivos de curta (FFR) e longa latência (P300), além da Avaliação da Percepção Visual (DTVP-2) e da Escala SAB. Na intervenção isolada, o TAAC foi eficaz nos procedimentos eletrofisiológicos (FFR e P300), nos testes comportamentais auditivos (PSI e TPD) e na Escala SAB. Já o protocolo SENA, de estimulação auditiva passiva, apresentou efeito positivo no FFR, no teste PSI e no item 4 da Escala SAB. As intervenções combinadas mostraram maior eficácia, especialmente quando o TAAC foi aplicado antes do SENA. Os resultados reforçam o uso combinado dos protocolos como abordagem promissora para a reabilitação auditiva e cognitiva, promovendo avanços comportamentais e neurofisiológicos, além de ampliar a compreensão sobre os mecanismos de plasticidade neural e suas implicações no processo de aprendizagem
