  A checklist will be applied for patients eligible for teaching to assess the suitability for ventilatory weaning. Patients who are able to start the weaning process are called to perform the spontaneous breathing test. The fact that it is not useful is installed using a T-tube technique in which patients are disconnected and the oxygen source at 5 L / min. The duration of the test is initially 30 minutes and may extend for 120 minutes. When the test is applied, the checklist will be applied to verify that the man shows no sign of failure and can not be displayed for extubation. Participants who are apt for extubation are randomized, by simple draw, into two groups: (1) Intervention group: composed of 150 patients. For this reason, it is necessary to pass the tracheal extubation for 1 hour. (2) Control group: 150 patients. Extubation will be performed after successful spontaneous breathing testing.

  Para os pacientes elegíveis para o estudo será aplicado um checklist para avaliar a aptidão para o desmame ventilatório. Os pacientes considerados aptos para iniciar o processo de desmame, serão colocados para realizar o teste de respiração espontânea. O teste de respiração espontânea será realizado utilizando-se a técnica de tubo T, no qual os pacientes serão desconectados do ventilador mecânico e conectados a uma fonte de oxigênio a 5 L/min. A duração do teste será inicialmente de 30 minutos podendo se estender a 120 minutos. Quando concluído o teste, será aplicado o checklist para verificar se o paciente não apresenta nenhum sinal de falha ao teste e esteja apto para ser extubado. Os participantes que estiverem aptos para a extubação serão randomizados, por meio de sorteio simples, em dois grupos: (1)Grupo intervenção: composto por 150 pacientes. Após passar no teste de respiração espontânea, os pacientes serão reconectados ao ventilador mecânico para repouso durante 1 hora para então proceder a extubação traqueal. (2)Grupo controle: 150 pacientes. A extubação será realizada imediatamente após o sucesso no teste de respiração espontânea.

  Mechanical ventilation for more than 21 days; Patients with Glasgow Scale <8; Patients with no indication of cardiopulmonary resuscitation or reintubation; Formal indication of non-invasive ventilation after extubation due to hypercapnia during the spontaneous breathing test; Patients with spindle trauma, neuromuscular diseases. Failure to obtain the informed consent term or withdrawal of study participation at any time.

  Evaluate the rate of tracheal reintubation within 48 hours after extubation has been performed. The percetual of reintubated patients will be verified in both the intervention group and the control group, and a comparison of the rates between groups will be performed.
