This is a pilot double-blind, controlled, randomized clinical trial study, using parallel group. 90 people over 18 years of age, both sexes, who have emotional distress and sought psychotherapy during the COVID-19 period (N=90) will participate in this study. Participants will be divided into two groups: Experimental (N=45) and Control (N=45). Participants will receive weekly psychotherapeutic follow-up, uninterrupted for 6 months, lasting 50 minutes, starting in May 2021. The day and time of the sessions will be agreed in advance, according to the availability of the psychotherapist and patient. The psychotherapists who will assist the patients will be psychology students from the last semesters and will receive previous training in the use of the instruments and the performance of the procedures described below. Participants will be guided by the psychotherapist to complete the Outcome Questionnaire-45.2 (OQ®-45.2) – intended to assess the progress of psychotherapy during the psychological intervention –, before the beginning of the session, or the day before, in the first 10 sessions of care. . After the first 10 sessions, the OQ®-45.2 will be instructed to be filled in alternately at each session. The Assessment for Signal Clients (OQ®-ASC) instrument - designed to assess problems with therapeutic alliance, social support, motivation and life events - will be completed after 3 months of psychotherapy, at the end of the psychotherapeutic process or when the Outcome Questionnaire System (OQ®-Analyst) displays the Red alert – alert made from the results of the OQ®-45.2. The instruments will be available in printed and digital versions, depending on the participant's need and preference. After filling in the instruments, the OQ®-Analyst will be fed with the information. For patients in the Experimental Group, Monitoring Routines (MR) and Feedback Systems (FS) will be used, in which psychotherapists have incorporated into their clinical practice the dialogue with patients about the answers to their questionnaires. For patients in the Control Group, RMR and SF will not be performed. After 6 months of intervention, the analyzes will identify if there were significant changes between the first and the last application of the OQ®-45, in the psychotherapeutic process of both groups. Seeking to confirm the hypothesis that the psychotherapeutic process that uses adjuvant MR and FS has the best effect (more effectiveness) on people who experience emotional distress during the COVID-19 period than the psychotherapeutic process that does not use MR / FS . Also at the end of the study, the appropriate outcomes will be performed: referrals or discharge.

Este é um estudo piloto, duplo-cego, controlado e randomizado, de ensaio clínico, utilizando grupos paralelos. Participarão deste estudo 90 pessoas, maiores de 18 anos, ambos os sexos, que apresentam sofrimento emocional e buscaram psicoterapia durante o período da COVID-19 (N=90). Os participantes serão distribuídos em dois grupos: Experimental (N=45) e Controle (N=45). Os participantes receberão acompanhamento psicoterapêutico semanal, ininterruptos por 6 meses, com duração de 50 minutos, a partir de maio de 2021. O dia e horário das sessões serão previamente acordados, de acordo com a disponibilidade do psicoterapeuta e paciente. Os psicoterapeutas que atenderão os pacientes serão estudantes de psicologia dos últimos períodos e receberão treinamento prévio para uso dos instrumentos e realização dos procedimentos a seguir descritos. Os participantes serão orientados pelo psicoterapeuta a preencher o Outcome Questionnaire-45.2 (OQ®-45.2) – destinado a avaliar o progresso da psicoterapia durante a intervenção psicológica –, antes do início da sessão, ou no dia anterior, nas 10 primeiras sessões de atendimento. Após as 10 primeiras sessões, o OQ®-45.2 será orientado a ser preenchido alternadamente a cada sessão. O instrumento Assessment for Signal Clients (OQ®-ASC) – destinado a avaliar problemas com a aliança terapêutica, suporte social, motivação e eventos da vida - será preenchido após 3 meses de psicoterapia, ao final do processo psicoterapêutico ou quando o Outcome Questionnaire System (OQ®-Analyst) apresentar o alerta Vermelho – alerta realizado a partir dos resultados do OQ®-45.2. Os instrumentos serão disponibilizados em versão impressa e digital, a depender da necessidade e preferência do participante. Após o preenchimento dos instrumentos, o OQ®-Analyst será alimentado com as informações. Para os pacientes do Grupo Experimental, será utilizado Rotinas de Monitorização (RMR) e Sistemas de Feedback (SF) no qual os psicoterapeutas incorporaram em sua prática clínica o diálogo junto aos pacientes sobre as respostas de seus questionários. Para os pacientes do Grupo Controle não será realizado RMR e SF. Após 6 meses de intervenção, as análises identificarão se houve mudanças significativas entre a primeira e a última aplicação do OQ®-45, no processo psicoterapêutico de ambos os grupos. Buscando confirmar a hipótese de que o processo psicoterapêutico que faz uso adjuvante RMR e SF, tem o melhor efeito (mais eficácia) em pessoas que apresentam sofrimento emocional durante o período da COVID-19 que o processo psicoterapêutico que não faz uso de RMR / SF. Também ao final do estudo serão realizados os devidos desfechos: encaminhamentos ou alta.

The OQ®-45.2 includes 45 items designed to assess three subscales: 1) Symptoms of subjective discomfort (Cronbach's α = .91–.92); 2) Interpersonal relationships (α = .74); and 3) Social role performance (α = .70–.71). All three subescales were assessed as outcome measures, alongside the total score, composed by the combination of these subscales.

Objective: The aim of this study was to evaluate, through a randomized clinical trial, the use of outcome monitoring and feedback system routines in psychotherapy processes conducted in a university psychology clinic (UPC) with people who presented emotional distress during the COVID-19 pandemic. Method: Participants (N=90) were divided into two groups: Experimental and Control. For patients in the Experimental Group, routine outcome monitoring (ROM) and feedback system (FS) were used, in which psychotherapists incorporated into their clinical practice a dialogue with patients about their answers to questionnaires. Results: After 6 months of intervention, we identified that there were significant changes between the first and the last application of the Outcome Questionnaire-45.2 (OQ®-45.2) in the psychotherapeutic process (F(3, 85) = 4,267, p < 0.01). However, there are no differences between the Experimental Group and the Control Group (F(3, 85) = 0.417, p = 0.741). Conclusion: Although no significant differences were found, it is possible to conclude that psychotherapy was effective for both groups. The use of ROM and FS in a Brazilian UPC proved to be a challenge, but at the same time, it is a way to develop a culture of research that can help beginning psychotherapists to improve results.

Para os pacientes do Grupo Experimental foram utilizados monitoramento de resultados de rotina (ROM) e sistema de feedback (FS), nos quais os psicoterapeutas incorporaram em sua prática clínica um diálogo com os pacientes sobre suas respostas aos questionários. Resultados: Após 6 meses de intervenção, identificamos que ocorreram mudanças significativas entre a primeira e a última aplicação do Outcome Questionnaire-45.2 (OQ®-45.2) no processo psicoterapêutico (F(3, 85) = 4.267, p < 0,01). Porém, não há diferenças entre o Grupo Experimental e o Grupo Controle (F(3, 85) = 0,417, p = 0,741). Conclusão: Embora não tenham sido encontradas diferenças significativas, é possível concluir que a psicoterapia foi eficaz para ambos os grupos. A utilização de ROM e FS em uma UPC brasileira mostrou-se um desafio, mas, ao mesmo tempo, é uma forma de desenvolver uma cultura de pesquisa que pode ajudar psicoterapeutas iniciantes a melhorar os resultados.
