Evaluation of the effectiveness of different anesthetic techniques during the cosmetic application of botulinum toxin in the upper third of the face through a Randomized Clinical Trial. Patient satisfaction and preference for the anesthetic technique used will be assessed, as well as safety after the application of each technique. Initially patients will be randomized (block randomization) into 3 groups with a split-face design, in which each patient will be their own control group. In each group, the patient will previously have their face divided in the midline, marking the points where the toxin will be applied, these being equally distributed and receiving exactly the same amount of toxin (IU) on each side of the face. Each hemiface will receive the following techniques: unilateral vibratory anesthesia (G1), or unilateral topical anesthesia (G2), or vibratory anesthesia x topical anesthesia (G3). In groups where unilateral applications are made, placebo ointment in the same color and consistency as the anesthetic ointment will be applied to the other side. The side to receive the interventions will also be determined randomly (simple randomization). Immediately after carrying out the cosmetic procedure, a questionnaire will be administered. The main outcomes investigated through the questionnaire will be: In the first part, the level of discomfort measured using the visual analogue scale (VAS); in the second part, preference and satisfaction with the anesthetic technique used; and in the third part, patients will be able to express the feelings and/or perceptions they may have about each anesthetic method used.

Trata-se de um estudo clínico randomizado controlado de três braços, duplo-cego. Busca avaliar a eficácia de diferentes técnicas anestésicas durante a aplicação cosmética de toxina botulínica, no terço superior da face. Será avaliada a satisfação e preferência do paciente pela técnica anestésica empregada, e a segurança após a aplicação de cada técnica. Inicialmente a amostra de 100 pacientes serão randomizados (randomização em blocos) em 3 grupos com um desenho de face dividida, em que cada paciente será o seu próprio grupo controle. Em cada grupo o paciente previamente terá sua face dividida na linha média, feita a marcação dos pontos onde será aplicada a toxina, sendo estes igualmente distribuídos e recebendo exatamente a mesma quantidade de toxina (UI) em cada um dos lados da face. Cada hemiface receberá as seguintes técnicas: anestesia vibratória unilateral (G1), ou anestesia tópica unilateral (G2), ou anestesia vibratória x anestesia tópica (G3). Nos grupos que forem feitas aplicações unilaterais, no outro lado será aplicado pomada placebo na mesma cor e consistência da pomada anestésica. O lado a receber as intervenções também será determinado randomicamente (randomização simples). Imediatamente após a realização do procedimento cosmético, será aplicado um questionário. Os desfechos principais investigados através do questionário serão: Na primeira parte o nível de incômodo medido por meio da escala visual analógica (EVA); na segunda parte a preferência e satisfação pela técnica anestésica usada; e na terceira parte, os pacientes poderão expor os sentimentos e/ou percepções que pudessem ter sobre cada método anestésico utilizado.

Inclusion criteria:

Female patients, aged over 18 years, systemically healthy, with no previous history of botulinum toxin application in the upper third of the face and who presented dynamic wrinkles in the upper third of the face.

Exclusion criteria:

Patients with abusive use of alcohol or illicit drugs, moderate smokers (> 10 cigarettes/day), history of facial surgery, pregnant or breastfeeding, pre-existing disorders that affect neuromuscular function (such as myasthenia gravis, motor neurone disease), infections locals, undergoing remote or recent (< 2 years) laser, radiotherapy or chemotherapy treatment in the orofacial region, who continuously take medications that may affect the metabolism of inflammatory mediators, and who are hypersensitive or allergic to botulinum toxin.

Pacientes com fizerem uso abusivo de álcool ou drogas ilícitas, tabagistas moderados (> 10 cigarros/dia) histórico de cirurgias faciais, grávidas ou amamentando, desordens pré-existentes que afetam a função neuromuscular (como miastenia grave, doença do neurônio motor), infecções locais, em tratamento de laser, radioterapia ou quimioterapia remota ou recente (< 2 anos) na região orofacial, que façam ingestão contínua de medicamentos que possam afetar o metabolismo de mediadores inflamatórios, e que tenham hipersensibilidade ou alergia à toxina botulínica.

After this, the paired sample t-test will be used for comparative analysis within each randomized group. A significance level of 5% will be adopted for all tests.

The study sample consisted of women who sought facial aesthetic treatment in a postgraduate specialization course in the city of São Luís, Brazil. The study included female volunteers over 18 years of age who were in good general health, had not previously received botulinum toxin treatment in the upper face, and presented expression wrinkles in the upper third of the face.

= 0.532), but vibration and topical prevailed more than placebo (P = 0.398). Regarding
the recommendation and reuse of the technique, only G3 showed differences in
response frequencies (P = 0.010), with the highest recommendation being on the side
applied with topical anesthetic. When analyzing each group, we found that the
discomfort was alleviated: G1 (vibration in 75% of cases), G2 (topical anesthetic in
71.9% of cases) and G3 (topical anesthetic in 55.6% of cases). Topical anesthetic was
the most effective technique recommended by patients. However, the vibratory stimulus was also effective in reducing pain in 75% of cases of unilateral vibration, 44.4%
vibration versus placebo, and in the total sample it was one of those that received the

Pain in multiple punctures for the administration of Botulinum Toxin type A (TxB-A) in the face is a persistent challenge, as infiltrative anesthesia is not viable. Strategies such as topical anesthetic creams have been explored, but adverse effects, high cost and longer clinical time make them less attractive. The application of vibratory stimuli during the procedure appears as a faster and more economical alternative, although with less scientific basis. The objective of the study was to evaluate the level of pain, satisfaction and patient preference for different anesthetic techniques during the cosmetic application of TxB-A in the upper third of the face, through a Randomized Clinical Trial. O controle da dor durante as múltiplas punções para a administração de Toxina Botulínica do tipo A (TxB-A) em procedimentos cosméticos faciais é um desafio para o profissional. Diversas estratégias têm sido exploradas, como cremes anestésicos tópicos, porém os efeitos adversos, alto custo e tempo clínico maior tornam esta opção pouco atrativa. A aplicação de estímulos vibratórios durante o procedimento surge como alternativa mais rápida e econômica. Entretanto, há escassez de evidências na literatura sobre ensaios clínicos que avaliem o efeito analgésico do estímulo vibratório. Dessa forma, o objetivo do estudo foi avaliar o nível de dor, satisfação e preferência pelo paciente de diferentes técnicas anestésicas durante a aplicação cosmética da TxB-A no terço superior da face, através de um ensaio clínico randomizado, duplo cego, tipo face dividida. Foram incluídas no estudo mulheres com idade igual ou maior de 18 anos que passaram por procedimento estético para redução de rugas facial. As pacientes foram randomizados em 3 grupos com um desenho de face dividida e recebendo exatamente a mesma quantidade de toxina (UI), sendo o paciente seu próprio grupo controle. Os grupos foram: (G1) anestesia vibratória x placebo, (G2) anestesia tópica x placebo, e (G3) anestesia vibratória x anestesia tópica. O placebo tinha a mesma cor e consistência da pomada anestésica.
