This is a prospective, randomized study that will be carried out at the Inpatient Unit of Hospital e Maternidade Galileo. Data collection will be carried out after approval of the project by the Research Ethics Committee, including 150 individuals admitted to the hospital after approval by the Research Ethics Committee and who meet the inclusion criteria, carried out on patients of both sexes, over 18 years of age, in the period between 10/01/2019 and 10/01/2022, randomized into two groups: flow incentive spirometry group (GF) and volume incentive spirometry group (GV). In the preoperative period, patients underwent physiotherapeutic evaluation and spirometry with the CpntecMed SP10 spirometer equipment, and were instructed on the surgical procedure, exercises and the respiratory incentive. After the surgical procedure, during the anesthetic recovery period, patients performed exercises such as standing up and sitting down in the chair, shoulder extension, flexion, adduction and abduction exercises with an elastic band, cycle ergometer, walking and incentive spirometry according to the randomization, under the supervision of a physiotherapist. Spirometry reassessments were performed in the immediate PO (iPO) and late PO. Participants who were allocated to the flow incentive group were instructed to perform maximum and slow inspirations, holding the spheres for as long as possible, following the following sequence of repetitions: 15 repetitions inhaling and holding the first sphere, 15 repetitions inhaling and holding two spheres, 15 repetitions inhaling and holding three spheres, totaling 3 sets of 15 repetitions with 10-second intervals between sets. Participants selected for the volume incentive group will be instructed to take slow, maximum breaths so that the first piston rises to the markings defined on the device and then raise the second piston to the markings for the generated tidal volume. 3 sets of 15 repetitions will be performed with 10-second intervals between sets.

Introduction: Currently, bariatric surgeries have been widely performed in Brazil using advanced techniques as one of the options for treating obesity. In this context, the role of early physiotherapy in reducing complications related to reduced lung volumes and capacities can be highlighted. Objective: To compare the spirometric variable maximum voluntary ventilation (MVV) between two physiotherapy intervention groups, flow and volume spirometers, in the postoperative period of bariatric surgery. Methodology: Prospective clinical study, conducted in patients of both sexes, over 18 years of age, randomized into two groups: flow incentive spirometry group (GF) and volume incentive spirometry group (GV). In the preoperative period (PRE), patients underwent physiotherapy evaluation and spirometry with the ContecMed® SP10 spirometer equipment, and were instructed on the surgical procedure, exercises and how to use the respiratory incentive. After the surgical procedure, during the anesthetic recovery period, the patients performed exercises such as standing up and sitting down in the chair, extension, flexion, adduction and abduction exercises of the shoulders with elastic bands, cycle ergometer, walking and incentive spirometer according to the randomization, under the supervision of the physiotherapist. Spirometry reassessments were performed in the immediate PO (POi) and in the first PO (PO1). Results: Of the 163 patients selected, 88 were included in the GF, with a mean age of 37.34 ± 10.32 years and 75% were female. The SG included 75 patients with a mean age of 34.76 ± 9.77 years and 85.3% were female. There was no significant difference between the groups regarding BMI (p = 0.104). The surgery and anesthesia times were significantly different between the groups, 103.30 ± 19.83 min. in GF and 110.20 ± 18.03 min. in GV (p = 0.026) and 135.80 ± 16.01 min. in GF and 149.10 ± 15.20 min in GV (p = 0.002), respectively. The length of hospital stay of patients showed a significant difference (p = 0.028), in GF (35.89 ± 1.47h) and in GV (35.41 ± 1.37h), however, without clinical relevance. There was a reduction in MVV (l / s) in both groups, in the PRE and POi moments, presenting values in PRE of 129.03 ± 23.32 l / s in GF and 134.65 ± 34.54 l / s in GV. At POi, the measured values were 72.73 ± 15.97 l/s in GF and 88.07 ± 25.60 l/s in GV (p<0.001). At PO1, there was an increase in MVV values in relation to POi and this increase was significantly greater in GV: 109.16±32.90 l/s compared to GF: 85.06±15.47 l/s (p=0.008). Conclusion: Patients who used volume boosters showed a significant recovery of MVV values at PO1, indicating the superiority of the volume booster in the postoperative period of bariatric surgery.
