It is a prospective, longitudinal study, considered a clinical trial and “blinded by double simulation” (Medronho et al., 2009). The TIBCO Statistica software (version 13.5.0.17) was used to calculate the sample size by testing hypotheses for comparing two independent groups with the Independent T-Test. The Wolf et al. (2008) study was used to estimate the variability of the method, selecting the articulatory diadochokinesis variable to compare the Speech Therapy Group (GTF) and Control Group (CG) groups. The values used were: mean difference of 2.0; grouped standard deviation of 0.66; α of 5%; 95% confidence level; e, K (test power) of 90%. A sample size of 21 patients was estimated for each group. 48 consecutive patients were included, 33 male and 15 female, aged between 18 and 70 years, diagnosed with AEC3, confirmed by genetic test or individual with ataxia, who has a first-degree relative with genetic test , which confirms the AEC3, being followed up at the General Neurology and Ataxias outpatient clinic at UNIFESP. The protocol was approved by the Ethics and Research Committee of UNIFESP (CEP 1206/2016). All signed a Free and Informed Consent Form - IC. The data collection period was from December 2017 to January 2019. Speech therapy evaluation and treatment took place at the General Neurology and Ataxias outpatient clinic. All participants were evaluated before and after the rehabilitation program and were submitted to the following protocol: assessment of motor function using the scales: SARA and ICARS, general physical examination, physical examination of the upper airways (oroscopy and rhinoscopy) and facial skeleton, evaluation of phonoarticulation , based on the Mayo Clinic protocol, nasofibrolaryngoscopy and videoendoscopy of swallowing, assessment of orofacial motricity, based on the AMIOFE-E protocol, performed the sleep examination (polysomnography) throughout the night to confirm the diagnosis of OSAS and lastly, responded to 6 protocols , namely: Epworth sleepiness scale, snoring analog scale, quality of life: Health Organization Quality of Life Assessment (WHOQoL Bref), Quality of Life in Swallowing Disorders - (SWAL-QoL), Living with Dysarthria (VcD) and Eating Assessment Tool -10 (EAT-10). The allocation was made by drawing lots prior to treatment and consisted of cards placed in a container with numbers 1 (25 cards) and others with number 2 (25 cards), in which 1 corresponded to the control group and 2 to the control group. speech therapy. Each patient selected a card and, depending on the number, was allocated to the corresponding group: • Control Group (CG): patients without speech therapy intervention, were just on the waiting list waiting for a place for speech therapy. • Speech Therapy Group (GTF): patients submitted to the rehabilitation program - A-TMOV (Myofunctional Orofacial and vocal therapy for Ataxia). Participants performed a physiological approach program, using exercise techniques; myofunctional orofacial, vocal effort and speech monitoring, once a week, for 60 minutes, totaling 14 visits, being the first and last meeting for evaluation and reassessment, with the same procedures, the evaluations were performed by an experienced speech therapist in the area , in which there was no knowledge in which group the patient belonged. The developed program consisted of exercises, to increase glottal coaptation without supraglottic interference, such as; sudden attacks on vowels and words, maximum phonation time on sustained vowels and automatic sequences, frequency and intensity variation, vocal projection, articulation sequences, articulatory diadochokinesis training, reading sections, isotonic and isometric exercise of myofunctional orofacial muscles and functions stomatognathic

Trata-se de um estudo prospectivo, longitudinal, considerado ensaio clínico e “cego por dupla simulação” (Medronho et al., 2009). O software TIBCO Statistica (version 13.5.0.17) foi usado para calcular o tamanho da amostra por meio de teste de hipóteses de comparação de dois grupos independentes com o Teste-T Independente. Como estimativa de variabilidade do método foi utilizado o estudo de Wolf et al., (2008), selecionando- se a variável da diadococinesia articulatória para comparação dos grupos Grupo Terapia Fonoaudiológica (GTF) e Grupo Controle (GC). Os valores utilizados foram: diferença média de 2,0; desvio padrão agrupado de 0,66; α de 5%; nivel de confiança de 95%; e, K (poder do teste) de 90%. Estimou-se um tamanho da amostra de 21 pacientes para cada grupo. Foram incluídos 48 pacientes consecutivos, sendo 33 do sexo masculino e 15 do sexo feminino, com faixa etária entre 18 e 70 anos, com diagnóstico de AEC3, confirmado por teste genético ou indivíduo com ataxia, que tenha um parente de primeiro grau com teste genético, que confirme o AEC3, em acompanhamento no ambulatório de neurologia Geral e Ataxias da UNIFESP. O protocolo foi aprovado pelo Comitê de Ética e Pesquisa da UNIFESP (CEP 1206/2016). Todos assinaram um Termo de Consentimento Livre e Esclarecido – TCLE. O período da coleta de dados foi de dezembro de 2017 à janeiro de 2019. A avaliação e tratamento fonoaudiológico ocorreram no ambulatório de Neurologia Geral e Ataxias. Todos os participantes foram avaliados pré e pós programa de reabilitação e foram submetidos ao seguinte protocolo: avaliação da função motora pelas escalas: SARA e ICARS, exame físico geral, exame físico da VAS (oroscopia e rinoscopia) e do esqueleto facial, avaliação da fonoarticulação, baseado no protocolo Mayo Clinic, nasofibrolaringoscopia e videoendoscopia da deglutição, avaliação da motricidade orofacial, baseado no protocolo AMIOFE-E, realizaram o exame do sono ( polissonografia) durante a noite toda para confirmação diagnóstica da SAOS e por último, responderam a 6 protocolos, sendo eles: escala de sonolência de Epworth, escala analógica do ronco, qualidade de vida: Health Organization Quality of Life Assessment (WHOQoL Bref), o Quality of Life in Swallowing Disorders – Qualidade de Vida em Disfagia - (SWAL-QoL), Vivendo com Disartria (VcD) e Eating Assessment Tool -10 (EAT-10). A alocação foi realizada por meio de sorteio prévio ao tratamento e consistiu de fichas colocadas num recipiente com os números 1 (25 fichas) e outras com o número 2 (25 fichas), nas quais o 1 correspondia ao grupo controle e 2 ao grupo de terapia fonoaudiológica. Cada paciente sorteava uma ficha e dependendo do número era alocado para o grupo correspondente: • Grupo Controle (GC): pacientes sem intervenção fonoaudiológica, apenas estavam na fila de espera aguardando vaga para fonoterapia. • Grupo Terapia Fonoaudiológica (GTF): pacientes submetidos ao programa de reabilitação - A-TMOV ( Terapia Miofuncional Orofacial e vocal para Ataxia). Os participantes realizaram um programa de abordagem fisiológica, com o uso de técnicas de exercícios; orofacial miofuncional, de esforço vocal e monitoramento da fala, uma vez por semana, durante 60 minutos, totalizando 14 atendimentos, sendo o primeiro e último encontro para avaliação e reavaliação, com os mesmos procedimentos, as avaliações foram realizadas por uma fonoaudióloga experiente na área, na qual não havia conhecimento em qual grupo que o paciente pertencia. O programa desenvolvido foi constituído de exercícios, para aumento de coaptação glótica sem interferência supraglótica, tais como; ataques bruscos em vogais e palavras, tempo máximo de fonação em vogais sustentadas e sequências automáticas, variação de frequência e intensidade, projeção vocal, sequências articulatórias, treino da diadococinesia articulatória, trechos de leituras, exercício isotônico e isométrico da musculatura orofacial miofuncional e das funções estomatognáticas

Spinocerebellar ataxia type 3 (SCA3) is a degenerative disease that is inherited in an autosomal dominant pattern. It is caused by a mutation located on the chromosome 14q resulting in abnormal CAG triplet repeats. It affects different population groups with an estimated prevalence of 1:100,000 in Brazil. This disease usually manifests between 30 and 50 years of age and first symptoms include ataxia, gait disturbance and abnormal ocular motricity. The degenerative process underlying SCA3 affects many different regions and/or functions of the central nervous system (CNS) and the peripheral nervous system (PNS) including areas and pathways that are involved in motor speech and swallowing. With progression of the disease, individuals with SCA3 exhibit communication and swallowing deterioration. Speech-language interventions for speech, voice and swallowing problems are important because patients with difficulty swallowing are at risk of repeated aspiration and dysarthria affects normal socialization. Objectives: To evaluate the impact of a speech therapy rehabilitation program on phonoarticulation, voice, swallowing and quality of life (QoL) of patients with SCA3. Methods: All participants were randomly assigned to two groups, an intervention group receiving speech therapy (STG) and a control group (CG). The intervention comprised a 12-session speech therapy rehabilitation program consisting of oral, pharyngeal and laryngeal strengthening exercises—so-called ATAXIA – Myofunctional Orofacial and Vocal Therapy (A-MOVT). All participants underwent pre- and post-intervention clinical evaluations using a phonoarticulation assessment tool developed by the Mayo Clinic; nasofibrolaryngoscopy; videoendoscopic swallowing study; and orofacial motricity assessment based on Expanded Protocol of Orofacial Myofunctional Evaluation with Scores (OMES). They were also assessed by four different QoL instruments: The World Health Organization's Quality of Life (WHOQoL-Bref); Living with Dysarthria, (LwD); Quality of Life in Swallowing Disorders (SWAL-QoL); and EAT-10 Food Assessment Tool. Results: The study sample included 48 patients with SCA3 (STG = 25; CG = 23), 33 (69%) females and 15 (31%) males; mean age 47.1±11.4 years; mean age at symptom onset 36.9±11.3 years; and disease duration 11.9±13.3 years. The International Cooperative Ataxia Rating Scale (ICARS) scores were 32.4±20.2 and the Scale for the Assessment and Rating of Ataxia (SARA) scores were 11.8±8.0. At the end of the three-month intervention, the STG showed significant improvements in dysphagia (0.56±0.87 [pre-] vs. 0.00±0.00 [post-]; p<0.001); dysarthria (1.92±0.27 [pre-] vs. 0.84±0.62 [post-], p<0.001); and orofacial motricity (153.00±16.12 [pre-] vs. 205.44±27.55 [post-], p<0.001). There were significant changes in the QoL in the STG compared to the CG when assessed by the LwD (179.12±62.55 vs. 129.88±51.42, p<0.001), SWAL-QoL (79.04±13.97 vs. 82.87±11.91, p=0.010) and EAT-10 (5.16±7.55 vs. 2.08±3.85, p=0.018) and there was an additional effect of speech therapy on clinical parameters regarding the Epworth sleepiness scale (before 12.30 ± 5.5 and after 10.15 ± 6.64, p <0.001) and the snoring intensity (before 1.54 ± 1.07 and after 1.15 ± 1.05, p <0.001) and polysomnographic as to the reduction of the Hypopnea Apnea Index (AHI pre 16.57 ± 20.58 vs. after 11.50 ± 10.98, p = 0.013, sleep efficiency (pre 73.52 ± 10.69 vs. after 75.43 ± 9.26, p <0.001, Arousal index 22.22 ± 12.72 vs. 20.88 ± 5.57, pp <0.001), minimal oxyhemoglobin saturation (SpO2 pre 86.62 ± 7.59 and after SpO2 88.00 ± 7.04, p <0.001). Conclusions: Patients with SCA3 should receive continuous speech therapy as part of the A-MOVT program since rehabilitation therapy improves difficulty swallowing and dysarthria.

A Ataxia Espinocerebelar tipo 3 (AEC3) é uma doença degenerativa, de herança autossômica dominante, cuja mutação no cromossomo 14q resulta na repetição anormal do tripleto cromossômico (CAG). Ela afeta diferentes populações e a prevalência estimada no Brasil é de 1:100.000. A doença manifesta-se geralmente entre os 30 e 50 anos e os sintomas iniciais incluem ataxia, alterações de marcha e distúrbios da motricidade ocular. O processo degenerativo acomete diferentes regiões e/ou funções do sistema nervoso central e periférico, entre elas, áreas e vias responsáveis pelo controle motor da fonoarticulação e da deglutição. Com o desenvolvimento da doença, restringe-se progressivamente a capacidade de comunicação e deglutição. A intervenção fonoaudiológica nas alterações de fala, voz e deglutição é importante, pois, na maioria dos casos, a dificuldade de deglutir pode causar pneumonia aspirativa de repetição e a disartria dificulta a sociabilização. Objetivo: Avaliar os efeitos de um programa de exercícios fonoterápicos, realizado em pacientes com AEC3 nos parâmetros da fonoarticulação, voz, deglutição e qualidade de vida. Método: Os pacientes foram divididos aleatoriamente em dois grupos: Grupo controle, em que os pacientes não foram submetidos à terapia fonoaudiológica e Grupo Fonoterapia, em que os pacientes foram submetidos à fonoterapia por 12 sessões. Todos os participantes foram avaliados pré e pós programa de reabilitação e foram submetidos ao seguinte protocolo; avaliação da fonoarticulação baseado no protocolo Mayo Clinic, nasofibrolaringoscopia, videoendoscopia da deglutição, avaliação da motricidade orofacial, baseado no protocolo “Avaliação Miofuncional Orofacial com Escores expandido” (AMIOFE-E) e por último responderam a 5 protocolos, sendo eles: qualidade de vida: “Health Organization Quality of Life Assessment” (WHOQoL BREF), o “Quality of Life in Swallowing Disorders” (SWAL-QoL), Vivendo com Disartria (VcD) e “Eat Assessment Tool” (EAT 10). Resultados: Participaram 48 pacientes com AEC3, sendo 33 (69%) do sexo feminino e 15 (31%) do sexo masculino, com idade média de 47,1±11,4 anos. A idade média de manifestação dos sintomas foi de 36,9±11,3 anos, o tempo de doença foi de 11,9±13,3 anos, escala International Cooperative Ataxia Rating Scale (ICARS) 32,4±20,2 e escala Scale for the Assessment and Rating of Ataxia (SARA) 11,8±8,0. Dos 48 pacientes da amostra, 23 foram do grupo controle e 25 do grupo terapia fonoaudiológica. Após três meses de treinamento com um programa de exercícios musculares na cavidade oral, orofaringe e laringe, de pacientes com AEC3 mostrou redução do grau de disfagia (Grau Geral pré 0,56±0,87 vs. pós 0,00±0,00; p<0,001) e disfonia (Grau Geral no pré 1,92±0,27 e pós 0,84±0,62, p<0,001), do fortalecimento da musculatura da motricidade orofacial miofuncional (pré 153,00±16,12 e pós 205,44±27,55, p<0,001), da qualidade de vida, mensurados pelos protocolos VcD (score total pré 179,12±62,55 vs. pós fonoterapia 129,88±51,42, p<0,001), SWAL-Qol (79,04±13,97 vs. 82,87±11,91, p=0.010) e EAT 10 (pré 5,16±7,55 vs. pós 2,08±3,85, p=0,018). e houve efeito adicional da terapia fonoaudiológica nos parâmetros clínicos quanto a escala de sonolência de Epworth (pré 12,30±5,5 e pós 10,15±6,64, p<0,001) e a intensidade do ronco (pré 1,54±1,07 e pós 1,15±1,05, p<0,001) e polissonográficos quanto a redução do Índice de Apneia Hipopneia (IAH pré 16,57±20,58 vs. pós 11,50±10,98, p=0,013, eficiência do sono (pré 73,52±10,69 vs. pós 75,43±9,26, p<0,001, Índice de despertares 22,22±12,72 vs. 20,88±5,57, p p<0,001), saturação minima da oxihemoglobina (SpO2 pré 86,62±7,59 e pós SpO2 88,00±7,04, p<0,001). Conclusão: Nos pacientes com AEC3, a reabilitação fonoaudiológica deve ser indicada e mantida continuamente, porque melhora os parâmetros relacionados a disartrofonia, disfagia e impacto na qualidade de vida.
