This is a two-arm, double-blind, randomized controlled clinical study. Experimental group: 25 individuals with stroke will receive intervention with Radio Electric Asymmetric Conveyer (REAC). Control group: 25 individuals with stroke will receive intervention with Radio Electric Asymmetric Conveyer placebo (device turned off). The experimental and placebo interventions will be offered in a randomized manner. It will be a single intervention session with REAC lasting approximately 5 to 10 seconds. After the intervention (experimental or placebo) participants will be reassessed regarding outcomes. Randomization will be carried out in blocks through the generation of random numbers in Microsoft Excel software. To do this, 6 blocks will be determined and then the blocks will be stratified in any column of the software until reaching 50 numbers that represent the participants. In the column next to the block stratification, random numbers will be generated using the command =RANDOM. The generated random numbers will be classified from largest to smallest and, in this way, the stratified blocks will also be automatically randomized. The allocation order for each block will then be placed according to the randomization order generated in another column of the software. The random order obtained will be assigned an identification number (1001, 1002, 1003...) sequentially and recorded on the outside of opaque envelopes (LIM & IN, 2019). The corresponding group will be placed inside the envelope, which will later be sealed and delivered to the participant immediately after the initial assessment and then sent to carry out the experiment. The entire randomization process (generation of random numbers in blocks, sealing of envelopes and distribution) will be carried out by a person not involved in the study's evaluation and intervention procedures.

Trata-se de um estudo clínico randomizado controlado de dois braços, duplo-cego. Grupo experimental: 25 indivíduos portadores de acidente vascular encefálico receberão intervenção com Radio Electric Asymmetric Conveyer (REAC). Grupo controle: 25 indivíduos portadores de acidente vascular encefálico receberão intervenção com Radio Electric Asymmetric Conveyer placebo (aparelho desligado). As intervenções experimental e placebo serão oferecidas de forma aleatorizada. Será uma única sessão de intervenção com REAC com duração de aproximadamente 5 a 10 segundos. Após a intervenção (experimental ou placebo) os participantes serão reavaliados quanto aos desfechos. A randomização será realizada em blocos através da geração de números aleatórios no software Microsoft Excel. Para isso, serão determinados 6 blocos e em seguida realizada a estratificação dos blocos em uma coluna qualquer do software até atingir os 50 números que representam os participantes. Na coluna ao lado da estratificação dos blocos serão gerados os números randômicos através do comando =ALEATÓRIO. Os números randômicos gerados serão classificados do maior para o menor e, dessa forma, automaticamente os blocos estratificados serão também randomizados. Em seguida serão colocados a ordem de alocação de cada bloco conforme a ordem de randomização gerada em uma outra coluna do software. Para a ordem randômica obtida será designado um número de identificação (1001, 1002, 1003...) de forma sequencial e registrada na parte externa de envelopes opacos (LIM & IN, 2019). No interior do envelope será colocado o grupo correspondente, posteriormente será selado e entregue ao participante logo após a avaliação inicial e, em seguida encaminhado para a realização do experimento. Todo o processo randomização (geração dos números randômicos em blocos, selagem dos envelopes e distribuição) será realizada por uma pessoa não envolvida no procedimentos de avaliação e intervenção do estudo.

Male or female; aged 18 and over; diagnosis of right or left hemispheric stroke of ischemic or hemorrhagic origin; physiotherapeutic diagnosis of right or left hemiparesis due to stroke; chronic cerebrovascular injury (injury time greater than 6 months); absence of obvious orthopedic or neurological disorders not resulting from the stroke that could interfere with balance; absence of rheumatic disorders with joint involvement of the lower limbs; have not participated in other clinical trials related to the use of transcranial stimulation within one month; use of medications that may interfere with the assessment of study efficacy, such as opioids, barbiturates, benzodiazepines and antipsychotics; ability to understand and execute simple verbal commands; ability to remain upright for at least 60 seconds without the need for human support or an assistive device; ability to walk without human assistance, being allowed to use an auxiliary device

At baseline, the experimental (n = 27) and control (n = 26) groups had comparable clinical and demographic characteristics. The median age was 60.5 years (Q1–Q3: 52.75–68.00) in the experimental group and 56.0 years (Q1–Q3: 46.5–69.0) in the control group. Median heights were similar between the groups: 1.64 m (1.57–1.68) in the experimental group and 1.65 m (1.57–1.74) in the control group. Weight was slightly higher in the control group [80.4 kg (73.3–84.4)] compared to the experimental group [73.1 kg (67.2–73.1)]. The time following a stroke varied considerably among participants, with a median of 37.0 months (20.3–119.8) in the experimental group and 24.0 months (24.0–59.0) in the control group. The sex distribution was balanced, with 14 men and 13 women in the experimental group and 14 men and 12 women in the control group. Regarding the type of stroke, both groups showed a predominance of ischemic cases: 18 ischemic and 9 hemorrhagic in the experimental group; 18 ischemic and 8 hemorrhagic in the control group. The dominant side was predominantly the right in both groups (experimental: 26; control: 25). The paretic side was similarly distributed, with a slight left predominance in both groups (experimental: 12 on the right and 15 on the left; control: 11 on the right and 15 on the left). Most participants in both groups were in rehabilitation at the time of the study (experimental: 15 yes / 12 no; control: 15 yes / 11 no)

During the intervention period, mild adverse events were observed in both groups. In the experimental group, the most frequently reported events were dry mouth (7 participants) and fatigue (3 participants), followed by somnolence (3), sweating (3), agitation (2) and headache (1). In the control group, the most common adverse events were somnolence (3 participants), dry mouth (2), sweating (2) and fatigue (1); no cases of headache or agitation were reported in this group. None of the reported events required interruption of the intervention, being considered mild and self-limiting

In intra- and intergroup comparisons, it was observed that, in general, postural and functional performance measures showed stability or small variations over time in both groups. Regarding the load on the lower limbs, the average values remained similar before and after the intervention, both in the experimental group (paretic: 44.0% → 44.5%; non-paretic: 56.0% → 55.5%) and in the control group (paretic: 43.3% → 43.4%; non-paretic: 56.7% → 56.6%). In the center of pressure (CoP) posturographic variables, the experimental group showed slight reductions in oscillation measurements, such as in the total area of the center of pressure (CoP) (492.18 mm² → 361.97 mm²) and in the right and left lateral areas (center of pressure (CoP) D: 184.39 mm² → 145.57 mm²; center of pressure (CoP) E: 377.54 mm² → 355.92 mm²). In the control group, the values also decreased slightly (total center of pressure (CoP): 208.38 mm² → 196.92 mm²). The oscillation velocity of the center of pressure (CoP) (total, anteroposterior, and mediolateral) followed a similar trend of slight reduction in both groups, with slightly lower final values in the control group. The center of pressure (CoP) variability measurements (VAR X and VAR Y) showed slight oscillations in both groups, without significant changes. Regarding functional clinical parameters, the Berg Balance Scale (BBS) remained stable in both groups, with similar medians before and after the test (experimental: 50.0 → 50.5; control: 52.0 → 53.0). The Timed Up and Go (TUG) test showed a slight improvement in the experimental group (22.33 s → 20.64 s), while the control group showed a slight worsening (17.53 s → 18.38 s). The mean center of pressure (CoP) position (BAR X and BAR Y) showed small variations over time in both groups, without clinically relevant changes
