Interventions:

Control Group: 20 Participants in the control group will receive guidance routinely offered by the care team of the follow-up clinic, physicians, nurses, physiotherapists, etc. Experimental group: The experimental group, 20 participants, will perform the Tummy Time practice until 3 months of Corrected Age, age that is expected to acquire the motor skill of head control. Parents of premature newborns allocated to the experimental group will be guided by the same care team as the control group, with the aid of an illustrative guidebook on the practice of the method. The orientation will be to carry out the Tummy Time practice for 30 minutes daily that can be distributed throughout the day. This time is in accordance with what is recommended by the World Health Organization. Caregivers will be instructed to carry out activities with children alert and awake, and supervise them constantly. The child should be placed on a fixed and comfortable surface for the Tummy Time, caregivers should pay attention to signs of tiredness such as crying or resting their face on the surface, and note the time to end the practice before your baby gets tired . In the booklet, you will have illustrative guidelines on the exercises performed and the surface for performing the practices. The content of the booklet was based on guidelines from the American Association of Occupational Therapy used in the practices. Guidelines of the booklet: - To initially encourage caregivers, the following strategies will be adopted: When burping the baby, try to place him in your lap on your belly. Incorporate Tummy Time into activities you are already doing with your baby, such as drying the towel after a bath, changing a diaper or applying lotion. It's never too early to start reading to your baby, and Tummy Time is a great opportunity to tell stories; - Child's Tummy Time Positioning: Wrap a thin towel or blanket to make a pillow that will provide extra support during tummy time. Place the pillow under the baby's chest and place your arms over the roller, with your hands stretched out in front of the baby. Your baby's chin should always be positioned in front of the pillow so that the airway is not blocked. Make sure your baby distributes her weight evenly on both sides of her body while on her back to strengthen the muscles evenly; - Encouragement of activities and play: While your baby is playing on his stomach, hold a toy in front of his face to get his attention, this will encourage your baby to lift his head and reach out. Sit or lie down in front of your baby during Tummy Time for safety and supervision and to keep him or her entertained. During Tummy Time, arrange toys in a circle around the baby to promote reach in many different directions. Your baby will be encouraged to lift their head, reach out and play when they see your face and hear your voice. - Involve your whole family: Encourage active play to strengthen your muscles during Tummy time. Offer toys for the baby to look up, the sides, and try to pick them up. Place a plastic mirror in front of your baby so that he is interested in lifting his head to look at his own reflection. If she doesn't look at her reflection, try tapping the mirror with your finger. Use blankets or towels with different textures and colors so that your baby can experience different visual and touch sensations (for example, switching between a towel and a fleece blanket). Four evaluations will be carried out throughout the study, one pre-intervention (baseline) and three post-intervention. Grupo Controle: 20 participantes do grupo controle receberão orientações rotineiramente oferecidas pela equipe assistencial do ambulatório de seguimento, médicos, enfermeiros, fisioterapeutas, e etc. Habitualmente, estas orientações referem-se à manutenção do vínculo e marcos do desenvolvimento sem protocolos definidos. Grupo experimental: O grupo experimental, 20 participantes, realizará a prática Tummy Time até os 3 meses de Idade Corrigida, idade que espera-se a aquisição da habilidade motora do controle de cabeça. Os pais dos recém-nascidos prematuros alocados ao grupo experimental serão orientados pela mesma equipe assistencial do grupo controle, com o auxílio de uma cartilha ilustrativa de orientação sobre a prática do método. A orientação será a realização da prática Tummy Time por 30 minutos diários que poderá ser distribuído durante todo o dia. Este tempo está de acordo com o que é preconizado pela Organização Mundial de Saúde. Os cuidadores serão orientados a realizar as atividades com as crianças em alerta e acordado, e supervisiona-las constantemente. A criança deverá ser colocada em uma superfície fixa e confortável para a realização do Tummy Time, os cuidadores deverão prestar atenção aos sinais de cansaço como chorar ou descansar o rosto na superfície, e observar o momento de encerrar a prática antes que seu bebê fique cansado. Na cartilha terá orientações ilustrativas sobre exercícios realizados e superfície de realização das práticas. O conteúdo da cartilha foi baseado em orientações da Associação Americana de Terapia Ocupacional utilizadas nas práticas. Orientações da cartilha: - Para incentivar inicialmente os cuidadores serão adotadas as seguintes estratégias: Ao arrotar o bebê, tente colocá-lo em seu colo sobre a barriga. Incorpore o Tummy Time nas atividades que você já está fazendo com seu bebê, como secar a toalha após o banho, trocar fraldas ou aplicar loção. Nunca é muito cedo para começar a ler para seu bebê, e o Tummy Time é uma ótima oportunidade para contar histórias; - Posicionamento Tummy Time da criança: Enrole uma toalha fina ou cobertor para fazer um travesseiro que fornecerá suporte extra durante a hora da barriga. Coloque a almofada sob o peito do bebê e posicione seus braços sobre o rolo, com as mãos esticadas à frente dele. Inclusion criteria:

Participants will be selected according to the following criteria: (1) neonates born between 30 wk and 36 wk and 6 days, according to ultrasound; (2) adequate for gestational age; (3) Apgar ≥ 7 in the 5th minute; (4) birth weight less than 2500 g; (5) intrauterine growth restriction (6) absence of cardiorespiratory complications such as bronchopulmonary dysplasia and congenital heart diseases, neurological such as brain malformation, microcephaly, hydrocephalus, musculoskeletal disorders such as congenital clubfoot and congenital muscular torticollis, or auditory according to the little ear test; genetic syndromes, congenital infections (toxoplasmosis, syphilis, rubella, cytomegalovirus and herpes simplex) and visual changes according to the little eye test; (7) present poor or normal motor repertoire in the assessment of Generalized Movements (MG).

Exclusion criteria:

The following will be excluded: (1) babies diagnosed with grade III and IV intraventricular hemorrhage; (2) babies whose parents withdraw from participating (3) babies who do not attend 2 assessments to monitor primary and secondary outcomes, (4) parents do not answer the phone for 3 consecutive days to reschedule assessments or declare withdrawal from the intervention by of the guardians, (5) parents who do not fill out the field diary to monitor the practice of Tummy Time. After randomization, participants will be excluded if they present any health deterioration and need to be hospitalized, change state and/or country.

Serão excluídos: (1) bebês com diagnóstico de hemorragia intraventricular grau III e IV,; (2) bebês cujos pais desistirem de participar (3) bebês que não comparecerem a 2 avaliações para monitoramento dos desfechos primários e secundários, (4) pais não atenderem o telefone por 3 dias consecutivos para remarcação das avaliações ou declarar desistência da intervenção por parte dos responsáveis, (5) pais que não preencherem o diário de campo para monitoramento da prática do Tummy Time. Após a randomização serão excluídos os participantes que apresentarem algum agravamento da saúde e necessidade de internação hospitalar, mudança de estado e/ou país.

The PTNB whose parents consented to participate in the study will be randomized into two groups: control and experimental and will be monitored by a researcher responsible (P1) for teaching the parents of the experimental group the Tummy Time practice, and maintaining periodic contact with the parents of the group control. Randomization will be simple and the sequence will be obtained by generating random numbers by computer. This step will be conducted by a researcher not involved in the evaluations. This researcher will organize the allocation in opaque envelopes numbered according to the coding that will be created for the study groups (control and experimental). The evaluations of the groups will be carried out at different times so that information sharing and direct contact between participants does not occur. All evaluations will be conducted by a researcher not involved with the interventions and blind to the allocation of groups (P2). The results of the evaluations will not be disclosed to the researchers responsible for the interventions. This same researcher (P2) will be responsible for the statistical analysis and will receive the data without identifying the groups.
Telemonitoring will be carried out by telephone or video call, weekly with the parents of both groups. The consultation will be structured to answer questions from parents and caregivers, schedule evaluations and encourage participation, as well as encourage the completion of the Tummy Time practice follow-up diary (for the experimental group).
A second researcher will be responsible for monitoring the primary and secondary outcomes (P2) Monitoring will be performed by four assessments performed at 0 months (beginning of intervention, baseline), 2, 3 and 4 months to follow up on outcome measures.

Os RNPT cujo pais consentiram a participação no estudo serão randomizados em dois grupos: controle e experimental e serão acompanhados por teleacompanhamento por um pesquisador responsável (P1) por ensinar aos pais do grupo experimental a prática Tummy Time, e manter contato periódico com os pais do grupo controle. A randomização será simples e a sequência será obtida através da geração de números randômicos por computador. Esta etapa será conduzida por pesquisador não envolvido nas avaliações. Este pesquisador organizará a alocação em envelopes opacos numerados de acordo com a codificação que será criada para os grupos de estudo (controle e experimental). As avaliações dos grupos serão realizadas em tempos diferentes a fim de que não ocorra o compartilhamento de informações e o contato direto entre os participantes. Todas as avaliações serão conduzidas por um pesquisador não envolvido com as intervenções e cego para a alocação dos grupos (P2). Os resultados das avaliações não serão divulgados aos pesquisadores responsáveis pelas intervenções. Este mesmo pesquisador (P2) será responsável pela análise estatística e receberá os dados sem a identificação dos grupos.
O teleacompanhamento será realizado por contato telefônico ou videochamada, semanalmente com os pais dos dois grupos. A consulta será estruturada para sanar dúvidas dos pais e cuidadores, marcar as avaliações e incentivar a participação, assim como para incentivar o preenchimento do diário de acompanhamento da prática Tummy Time (para o grupo experimental).

Results summary:

The aim of the study is to evaluate the effects of the Tummy Time practice on head control in moderate to late preterm infants. This is a randomized, controlled clinical trial with PTNBs below 2500g, who will be followed up from birth to 6 months of corrected age. The sample will consist of 42 preterm infants who will be randomized into a control group (n=21) and an experimental group (n=21), when they are discharged to the follow-up clinic. Both will be monitored by the outpatient team, however the PTNB randomized to the experimental group will also be instructed to perform the Tummy Time for 30 minutes daily, which can be distributed throughout the day. An illustrative booklet will also be offered as support material for this group. The control group will receive guidance routinely offered by the care team of the follow-up outpatient clinic. In addition, the study will also assess the effects of practice on the development of gross motor function, vertical alignment and horizontal leveling of the head, and palm grip strength. Data will be analyzed by inferential analysis and hypothesis testing, considering an alpha equal to 5% to differentiate between groups. It is expected that the Tummy time practice in PTNBs will strengthen the cervical muscles necessary to overcome the force of gravity and master motor skills early and allow the integration between family protagonism and environmental experiences that allow the organization between the nervous and musculoskeletal systems. skeletal face to the demands of the tasks exposed to the baby.

O objetivo do estudo é avaliar os efeitos da prática Tummy Time, sobre o controle de cabeça, em lactentes pré-termo de moderados a tardios. Trata-se de um ensaio clínico, aleatorizado, controlado, com RNPT abaixo de 2500g, os quais serão acompanhados do nascimento aos 6 meses de idade corrigida. A amostra será constituída por 42 prematuros que serão aleatorizados em grupo controle (n=21) e grupo experimental (n=21), quando receberem alta para o ambulatório de Follow-up. Ambos serão acompanhados pela equipe ambulatorial, entretanto os RNPT aleatorizados para o grupo experimental serão também orientados a realizar o Tummy Time por 30 minutos diários, que poderá ser distribuído durante todo o dia. Uma cartilha ilustrativa também será oferecida como material de apoio para este grupo. O grupo controle receberá orientações rotineiramente oferecidas pela equipe assistencial do ambulatório de seguimento. Além disso, o estudo também avaliará os efeitos da prática no desenvolvimento da função motora grossa, alinhamento vertical e nivelamento horizontal da cabeça e força de preensão palmar. Os dados serão analisados pela análise inferencial e teste de hipóteses, considerando um alfa igual a 5% para diferenciação entre os grupos. Espera-se que a prática Tummy time nos RNPT fortaleça os músculos cervicais necessários para vencer a força da gravidade e dominar as habilidades motoras precocemente e permita a integração entre o protagonismo familiar e as experiências ambientais que permitem a organização entre o sistema nervoso e musculo-esquelético frente as demandas das tarefas expostas ao bebê.
