This will be a longitudinal, non-randomized intervention study to evaluate the impact of combined sodium hyaluronate and triamcinolone hexacetonide therapy on the quality of life of patients with knee osteoarthritis (OA). The study population will consist of patients aged 45 years or older, diagnosed with primary knee OA, treated at an orthopedic clinic in Maringá, Paraná, between January and March 2023. Individuals with a history of knee surgery or trauma, or who have used oral chondroprotectors, corticosteroid injections, or have undergone viscosupplementation in the last year will be excluded. After signing the informed consent form, participants will receive the intervention, which will consist of three intra-articular injections of sodium hyaluronate (25mg/2.5ml) combined with triamcinolone hexacetonide (20mg/1ml), with a 7-day interval between injections. Data will be collected at three time points: baseline (T0), 3 months (T3), and 6 months (T6), using the WHOQOL-bref (quality of life) and WOMAC (pain, stiffness, and functional difficulty) questionnaires. Additionally, sociodemographic and clinical data will be collected.

A população do estudo, com um tamanho amostral de 18 participantes, consistirá em pacientes com 45 anos ou mais, com diagnóstico de OA primária de joelho, atendidos em uma clínica de ortopedia em Maringá, Paraná, entre janeiro e março de 2023. Serão excluídos indivíduos com histórico de cirurgia ou trauma no joelho, ou que tenham utilizado condroprotetores orais, injeções de corticosteroides ou que tenham realizado viscossuplementação no último ano. Após assinarem o termo de consentimento livre e esclarecido, os participantes receberão a intervenção, que consistirá em três injeções intra-articulares de hialuronato de sódio (25mg/2,5ml) combinadas com hexacetonido de triancinolona (20mg/1ml), com intervalo de 7 dias entre as aplicações. Os dados serão coletados em três momentos: no início do estudo (T0), após 3 meses (T3) e após 6 meses (T6), utilizando os questionários WHOQOL-bref (qualidade de vida) e WOMAC (dor, rigidez e dificuldade funcional). Adicionalmente, serão coletados dados sociodemográficos e clínicos.

The study population consisted of 18 patients with knee osteoarthritis. The mean age of the participants was 64.28 (±6.94) years, with a predominance of females (83.33%). The majority of participants had an education level of ≤ 8 years (72.22%), were sedentary (83.33%), and were overweight or obese (77.78%). Regarding the disease, 77.78% were classified with grade 3 osteoarthritis (Kellgren-Lawrence), 55.56% had the disease for 5 years or less, and 47.06% had bilateral involvement. The duration of pain was 5 years or less for 50% of the sample.

Introduction: Knee osteoarthritis (OA) is a degenerative disease that causes pain and functional disability. Viscosupplementation with hyaluronic acid combined with corticosteroids such as triamcinolone is a therapeutic option for symptom relief. Objective: To analyze the impact of the combined therapy of sodium hyaluronate and triamcinolone on the quality of life and on symptoms of pain, stiffness, and function in patients with knee osteoarthritis. Methods: A longitudinal, non-randomized, interventional study with 18 patients with knee OA. Participants received three intra-articular injections of the combined therapy, one week apart. Quality of life (WHOQOL-bref) and function (WOMAC) assessments were performed at baseline (T0), 3 months (T3), and 6 months (T6). Data analysis included descriptive statistics, Wilcoxon test, and cluster analysis. Results: Following the therapy, statistically significant improvements (p < 0.01) were observed in overall quality of life (Cohen’s d = 1.18) and in WOMAC scores for pain, stiffness, and function (Cohen’s d = 2.10). The most significant improvement occurred between T0 and T3. Cluster analysis identified three distinct subgroups of patients with different response profiles to the treatment. No adverse events were reported. Conclusion: The combined therapy of sodium hyaluronate and triamcinolone proved to be effective and safe, significantly improving quality of life and reducing symptoms in patients with knee osteoarthritis in the short and medium term.

A viscossuplementação com ácido hialurônico associada a corticosteroides como a triancinolona é uma opção terapêutica para alívio dos sintomas. Objetivo: Analisar o impacto da terapia combinada de hialuronato de sódio e triancinolona na qualidade de vida e nos sintomas de dor, rigidez e função em pacientes com osteoartrite de joelho. Métodos: Estudo longitudinal, de intervenção, não randomizado, com 18 pacientes com OA de joelho. Os participantes receberam três injeções intra-articulares da terapia combinada, com intervalo de uma semana. As avaliações da qualidade de vida (WHOQOL-bref) e da função (WOMAC) foram realizadas no início (T0), após 3 meses (T3) e 6 meses (T6). A análise dos dados incluiu estatística descritiva, teste de Wilcoxon e análise de cluster. Resultados: Após a terapia, foram observadas melhoras estatisticamente significativas (p < 0,01) na qualidade de vida geral (Cohen’s d = 1.18) e nos escores de dor, rigidez e função do WOMAC (Cohen’s d = 2.10). A melhora mais expressiva ocorreu entre T0 e T3. A análise de cluster identificou três subgrupos distintos de pacientes com diferentes perfis de resposta ao tratamento. Não foram relatados eventos adversos. Conclusão: A terapia combinada de hialuronato de sódio e triancinolona demonstrou ser eficaz e segura, melhorando significativamente a qualidade de vida e reduzindo os sintomas em pacientes com osteoartrite de joelho a curto e médio prazo.
