A prospective study to evaluate the efficacy of intersticial percutaneous laser therapy ( ILT) in patients with stage I breast cancer was conducted. Inclusion criteria were patients between 18 and 80 years without clinical conditions that precluded any of the study interventions with invasive breast cancer les than 2 cm, presented as a unique lesion . MRI and US were used to estimate the primary tumor volume and rule out multicentric/ multifocal disease. ILT ablation was performed using an Echolaser unit (Elesta) through percutaneous laser optical fibers with a US-guided insertion in the lesion center. Power applied was 5W and the energy should reach 2000J per fiber. In tumors larger than 2 cm, 2 optic fibers have been inserted . All procedures were performed under local anesthesia and sedation. The post procedure image re-evaluation included an immediate US after the ablation and a new MRI about 3 hours after the procedure. The definitive proposed surgical treatment was carried out 8-13 days after the ILT ablation. Twenty patients were planned to be included in the study. The evaluated factors were: treatment time, adverse events, mean ablation index (evaluated by H&E and CK8/18) and MRI sensitivity and specificity in identify residual tumor ( through correlation between post ablation MRI images and Pathologic findings).

Foi realizado um estudo prospectivo para avaliar a eficácia da ablação percutânea a laser ( APL) em pacientes com câncer de mama em estado I. Os critérios de inclusão eram pacientes entre 18 e 80 anos com carcinoma invasivo unifocal menor que 2 cm. A ressonância magnética (RM) e a ultrassonografia (US) foram utilizadas para estimar o volume do tumor primário e descartar doença multicêntrica/multifocal. A APL foi realizada com aparelho Echolaser (Elesta) por meio de fibras ópticas com inserção guiada por US no centro da lesão. A potência aplicada foi de 5W e a energia deveria atingir 2.000J por fibra. Em tumores maiores que 2 cm, foram inseridas 2 fibras ópticas. Todos os procedimentos foram realizados sob anestesia local e sedação. A reavaliação das imagens pós-procedimento incluiu US imediatamente após a ablação e nova RM cerca de 3 horas após o procedimento. O tratamento cirúrgico definitivo proposto foi realizado 8-13 dias após APL. O tamanho inicial previsto da amostra era de 20 pacientes.Os fatores avaliados foram: tempo de tratamento, eventos adversos, índice médio de ablação (avaliado por hematoxilina/Eosina e citoqueratina 8/18) e sensibilidade e especificidade da ressonância magnética na identificação de tumor residual (através da correlação entre imagens de ressonância magnética pós-ablação e achados patológicos).

Expected outcomes 1: Observe a complete ablation rate in more than 90% of cases and describe the average ablation observed per patient. Sample size calculation: among similar studies published between 2000 and 2008, we observed an inclusion of 92 patients (average of 30 patients per study) with an average ablation rate of 51% and standard deviation of 3%. Considering the objective of estimating the average percentage of necrosis and its 95% confidence interval, assuming a standard deviation similar to that of the aforementioned studies and a maximum error of 17% in the estimate, it would be necessary to observe at least 20 patients.

Observed Outcomes 3: The mean laser application time was 10.1 minutes per nodule (ranging from six to 13 minutes). No severe adverse events were recorded during the ablative procedure. After five to six hours post-procedure, 11 patients reported mild pain, while three reported moderate pain, which significantly improved with analgesics. The following day after the ablative procedure, six patients reported mild pain, and the other eight patients reported no pain. One patient experienced a grade 2 skin burn of approximately 0.6 x 0.6 cm, caused by inadequate coupling of the optical fiber to the puncture needle during laser application. The area was excised during surgery, and there were no further consequences or aesthetic damage beyond what was expected from the surgery.

No serious adverse events were observed with the ablative procedure. After 5 to 6 hours of the procedure, the pain was reported as mild by 11 patients and moderate by three of them, but with significant improvement with analgesics use.Pain was reported as mild by six patients and absent by the other eight patients the day following the ablative procedure. The initial evaluation by Hematoxylin and Eosin (HE), showed average lesion ablation rate of 61.5% (ranged from 0 to 93%). Considering only nodules less than 1.5 cm (9 lesions), the average ablation rate was 75% (range 30% to 93%).Comparison of the ablation area through the expression of estrogen receptors (ER) and CK8/18 proved to be overlapping with the area identified by HE. Despite being overlapping, a change in the CK8/18 expression pattern was noted in most regions of apparent residual tumor. As the cell viability in areas with anomalous expression pattern is uncertain, they were not considered as non-viable necrotic areas.Magnetic resonance imaging (MRI) performed after ablation showed an area of extensive edema around the metallic artifact in all cases. In 12 of the 15 cases, thin enhancement was identified around the ablation cavity. In one of the cases, in addition to the thin enhancement, a nodule was observed at the lower edge of the ablation. In this patient, the ablation rate observed in the anatomopathological evaluation was 69% and the nodular area on MRI was not observed on pathology. The ROC Curve results demonstrated a positive association between the pathological findings and the MRI findings (AUC = 0.931; 95 % CI = 0.797 – 1.000). The cutoff point that maximized specificity and sensitivity was an ablation rate of less than 85%. In the four cases with an ablation rate greater than or equal to 85%, no residual MRI enhancement was observed in three of them. On the other hand, in the 11 cases that presented an ablation rate below 85%, MRI was able to identify the presence of enhancement or residual nodule. Given these results, MRI had a sensitivity of 1.0 and specificity of 0.84 for identifying residual tumor greater than 15%, and MRI accuracy for the same characteristics was 93%.After a mean follow-up of 49 months (ranged from 38 to 63 months), 13 of the 14 patients evaluated were alive and without signs of local regional or distant recurrence. One patient died from complications of ovarian carcinoma, three years after ablative treatment.

Nesta paciente, a taxa de ablação observada na avaliação anatomopatológica foi de 69% e a área nodular à RM não foi observada à patologia.Os resultados da Curva ROC demonstraram associação positiva entre os achados patológicos e os achados à RM (ASC = 0,931; 95% IC = 0,797 – 1,000). O ponto de corte que maximizou a especificidade e sensibilidade foi de taxa de ablação inferior a 85%. Nos quatro casos com taxa de ablação maior ou igual a 85%, não foi observado qualquer realce residual à RM em três deles. Em contrapartida, nos 11 casos que apresentaram taxa de ablação abaixo de 85%, a RM foi capaz de identificar a presença de realce ou nódulo residual. Results summary:

Background: Screening programs are changing the scope of breast cancer, with a growing number of patients diagnosed at a nearly stage. The current standard treatment (breast conservative surgery and radiation therapy) offers an excellent prognosis for these patients. Besides tumor removal, other techniques can be used to destroy a mass in place. A range of minimally invasive techniques holds promise on local breast tumor ablation (i.e., thermotherapy and cryotherapy). Previous studies with Interstitial laser thermotherapy (ILT) for breast cancer report mean ablation rates between 33% to 87%. Most common cited causes for failure are equivocal tumor size estimation, inadequate technique and the learning curve. Objective: The primary objective aimed to evaluate the efficacy and safety of Ultrasound (US)-guided ILT in the treatment of stage I breast cancer. Secondarily, the study sought to standardize the technical criteria applied, identify histopathological and immunohistochemical criteria that can predict treatment success and evaluate the effectiveness of magnetic resonance imaging (MRI) in estimating tumor volume and residual lesion. Methods: A prospective study including patients with Stage I invasive breast cancer, presented as a unique lesion was conducted. MRI and US were used to estimate the primary tumor volume and rule out multicentric/ multifocal disease. ILT ablation was performed through percutaneous laser optical fibers with a US-guided insertion. The programmed power was 5 W and the energy should reach 2000 J per fiber. Post procedure image reevaluation included US and MRI. The definitive proposed surgical treatment was carried out 8 to13 days after the ILT ablation. Results: Eighteen patients were included in the study, of whom 14 (with 15 lesions) completed the protocol. The mean tumor size was 13.7mm (4-20mm). The mean treatment time was 10.1 minutes and all patients went home the same day, reporting only mild (78%) or moderate (22%) pain. The mean ablation index, was 61,5% (0-93%) and when only tumors under 15 mm were considered the mean ablation index was 75% (30%-93%). MRI sensitivity and specificity to detect residual tumors were 1.0 and 0.84, respectively, when residual tumors were larger than 15% from the original tumor. Conclusions: These results suggest that laser therapy can eliminate viable neoplastic tissue in vivo and MRI appears to be an appropriate method for selecting patients and evaluating any residual tumor. To achieve better results, standardization in patient selection and in technical parameters should be considered. There are several advantages of ILT for breast cancer treatment, including preserving the structure and function of breast, no bleeding and no scaring. More studies refining the technique should be conducted to allow the implementation of ILT as an alternative to surgical treatment in early breast cancer.

Introdução: Os programas de rastreamento têm contribuído para a mudança do cenário do carcinoma de mama, com número crescente de diagnósticos em fase inicial da doença. Apesar do atual tratamento local padrão para os casos iniciais (cirurgia conservadora da mama e radioterapia) oferecer excelente prognóstico, técnicas minimamente invasivas, cujos resultados se mostram promissores no tratamento de outros tipos de neoplasias, têm despertado interesse para o tratamento do câncer de mama em estádio I. Dentre elas, técnicas ablativas como a termoterapia e a crioterapia são as mais estudadas. Estudos anteriores com ablação percutânea com laser intersticial (APL) para câncer de mama relatam taxas médias de ablação entre 33% a 87%. As causas mais comumente citadas para o insucesso são: estimativa equivocada do tamanho do tumor, técnica inadequada e curva de aprendizado. Objetivos: O objetivo primário visou avaliar a eficácia e segurança da APL guiada por Ultrassonografia (US) no tratamento do câncer de mama em estádio I. Secundariamente, buscou-se padronizar os critérios técnicos aplicados, identificar critérios histopatológicos e imuno-histoquímicos que possam predizer o sucesso do tratamento e avaliar a eficácia da ressonância magnética (RM) na estimação do volume tumoral e de eventual lesão residual. Métodos: Foi realizado um estudo prospectivo com pacientes com câncer de mama invasivo em estádio I, unifocal. RM e US foram utilizados para estimar o volume do tumor primário e descartar doença multicêntrica/multifocal. A APL foi realizada por meio de fibras ópticas com inserção guiada por US. A potência programada foi de 5 W e a energia deveria atingir 2000J por fibra. A reavaliação das imagens após-procedimento incluiu US e nova RM. O tratamento cirúrgico definitivo foi realizado entre 8 e 13 dias após a APL. Resultados: Foram incluídas 18 pacientes, das quais 14 (com 15 lesões) completaram o protocolo. O tamanho médio do tumor à RM foi de 13,7 mm (4 a 20 mm). O tempo médio de tratamento foi 10,1 minutos e todas os pacientes receberam alta no mesmo dia relatando dor leve (78%) ou moderada (22%). O índice médio de ablação foi de 61,5% (com variação de 0 a 93%). Quando foram considerados apenas tumores menores de 15 mm, o índice médio de ablação foi 75% (variando entre 30% e 93%). A sensibilidade e a especificidade da RM para detecção de tumores residuais foram de 1,0 e 0,84, respectivamente, quando os tumores residuais eram maiores que 15% do tamanho original do tumor. Conclusões: Estes resultados preliminares sugerem que a APL é capaz de eliminar tecido neoplásico viável in vivo e a RM parece ser métodos adequado para seleção de pacientes e avaliação de eventual tumor residual. Para obter melhores resultados, deve-se considerar a padronização na seleção dos pacientes e aprimoramento dos parâmetros técnicos empregados.Dentre as vantagens da APL para o tratamento do câncer de mama, incluímos preservação da estrutura e função da mama, sem sangramento e sem cicatrizes. Mais estudos com refinamento da técnica devem ser realizados para permitir a implementação do APL como alternativa ao tratamento cirúrgico no câncer de mama inicial.
