This is a double-blind, two-arm, randomized controlled clinical trial. Experimental group: 22 participants with discolored teeth, A3 shade or darker according to the Vita Classical shade guide, will receive in-office bleaching with 35% hydrogen peroxide. Control group: 22 participants with discolored teeth, A3 shade or darker according to the Vita Classical shade guide, will receive in-office bleaching with 37% carbamide peroxide. The treatments will go through the process of randomization through the random.org program to define the distribution in groups according to the bleaching agent to be applied. After randomization, each participant will have a number that will identify them. This will be written on a brown paper envelope and inside will be which group it will be assigned to. The envelope will be sealed, will remain in the possession of the professional who will carry out the randomization and will be opened, for the operator, only when the bleaching procedure will be carried out. The treatments drawn will be carried out randomly, in a period of 07, 14 and 21 days.

Trata-se de um estudo clínico randomizado controlado de dois braços duplo-cego. Grupo experimental: 22 participantes com dentes escurecidos com cor A3 ou mais escuros de acordo com a escala de cor Vita Classical receberão clareamento de consultório com Peróxido de hidrogênio a 35%. Grupo controle: 22 participantes com dentes escurecidos com cor A3 ou mais escuros de acordo com a escala de cor Vita Classical receberão clareamento de consultório com Peróxido de carbamida a 37%. Os tratamentos passarão pelo processo de randomização aleatória por meio do programa random.org para definir a distribuição nos grupos de acordo com o agente clareador a ser aplicado. Após a randomização, cada participante terá um número que irá identificá-lo. Esse, será escrito em um envelope de papel pardo e dentro estará para qual grupo será designado. O envelope será selado, ficará em posse do profissional que fará a randomização e será aberto, para o operador, somente no momento da realização do procedimento clareador. Os tratamentos sorteados serão realizados de forma aleatorizada, em um período de 07, 14 e 21 dias.

Patients who refuse to participate in the treatment. Pregnant women. Nursing mothers. Smokers. Patients with bruxism or another condition that causes gingival recession and/or dentine exposure. Expected Outcome 1: Evaluate the color difference between the pre and post-intervention periods verified by measurements with a clinical spectrophotometer and color difference calculation. The color difference will be evaluated and interpreted by the visual acceptability and perceptability thresholds. It is expected to find a mean color difference above 5.4 deltaE units for both treatments, corresponding with excellent whitening outcome according with the acceptability and perceptability thresholds.

Outcome Found 1: An average color difference of 3.38 and 4.48 deltaE 2000 units was observed for the groups treated with 37% carbamide peroxide and 35% hydrogen peroxide, respectively, verified through measurements with a clinical spectrophotometer and calculations of color difference between the pre- and post-intervention periods. At baseline, the number of participants included in the study was forty-four (44), of which twenty-two (22) were allocated to Group 1 (35% hydrogen peroxide) and twenty-two (22) to Group 2 (37% carbamide peroxide). The mean age of the patients included in the study was twenty-three years (23.6), with the youngest patient being eighteen (18) and the oldest thirty-five (35). Group 1 included eight men (8) and fourteen women (14), and Group 2 included thirteen men (13) and nine women (9). The mean values of the CIEL*a*b* color coordinates, obtained using a clinical spectrophotometer, did not show statistically significant differences between the two groups at baseline: L* - p value=0.916; a* - p-value=0.542 and b* - p-value=0.734, indicating that there was homogeneity between the groups' condition at the beginning of treatment.

There was no follow-up loss of patients during the study period, and all patients who started treatment were monitored during the clinical parameter assessment time points (every 7 days for 21 days of treatment). All baseline patients completed treatment after the study period.

However, the mean values of b* (p <.001) and WID (p <.001) showed statistically significant differences between the groups, with 35% hydrogen peroxide presenting outcomes related to more favorable whitening. The mean values of color difference (deltaE 2000) and whiteness difference (deltaWID) were interpreted based on the acceptability and perceptibility thresholds and their respective ratinings. At the end of the treatment, it was observed that the group treated with hydrogen peroxide showed excellent whitening effectiveness with mean values above ΔE00 > 5.4 and ΔWID > 7.8 and the group treated with 37% carbamide peroxide showed very good whitening effectiveness with mean values between 3.6 < ΔE00≤5.4 and 5.2 < ΔWID ≤ 7.8.

In recent years, manufacturers have introduced different whitening products to the market in various forms of presentations and concentrations, with the aim of improving the whitening outcome. Research on the effectiveness of the 37% Carbamide Peroxide bleaching agent is limited, with only a few laboratory studies and clinical trials conducted. Through a double-blind randomized clinical trial, this study aimed to determine the effectiveness of two whitening products (Hydrogen Peroxide 35% and Carbamide Peroxide 37%) for in-office use. Volunteers were examined to assess whether they could participate in the research, using the inclusion and exclusion criteria. Subsequently, randomization was carried out to define the distribution of groups according to the bleaching agent to be applied. Study operator was blinded to the type of whitening treatment until the envelopes were opened. The evaluator was trained in color measurement procedures. In the first whitening session (T0 – Baseline), the initial color was assessed prior to the application of the bleaching agent. The protocols were carried out according to the manufacturers' instructions. The bleaching agents were applied in 3 sessions (T0, T1 and T2) with an interval of 7 days among them. In Group 1 (G1), a 35% Hydrogen Peroxide bleaching agent was used. In Group 2 (G2), a 37% Carbamide Peroxide bleaching agent was used. The objective analysis was carried out with a clinical spectrophotometer, the CIEL*a*b* color coordinates were measured three times in the middle third of the buccal surface of upper right central incisor of each patient, using the previously prepared positioning guide. The whiteness index for dentistry (WID), the CIEDE 2000 color difference (ΔE00) and the whiteness difference (ΔWID) between T0 and each evaluation period were calculated. At the end of treatment, the mean values of L*, a*, b* and WID for the group treated with 37% carbamide peroxide were: L*= 89.12 (4.063), a*=-2.00 (0.689), b*= 19.1 (2.07) and WID= 29.0 (3.96). At the end of treatment, the mean values of L*, a*, b* and WID for the group treated with 35% hydrogen peroxide were: L*= 89.75 (3.23), a*=-2.35 (1.00), b*= 16.6 (3.16) and WID= 33.5 (4.87). Both protocols were able to promote tooth whitening, with 35% hydrogen peroxide showing excellent effectiveness and carbamide peroxide showing very good effectiveness. Hydrogen peroxide was able to produce greater whiteness (WID) and a greater reduction in the yellowness of the teeth (b*).

Nos últimos anos, os fabricantes introduziram no mercado diferentes produtos clareadores em diversas apresentações e concentrações, com o enfoque de melhorar o resultado do clareamento. Porém, ao tratar-se do agente clareador Peróxido de Carbamida 37%, há poucos estudos laboratoriais e ensaios clínicos para avaliar sua eficácia. Dessa maneira, o presente estudo tem como objetivo avaliar a efetividade de dois agentes clareadores para uso em consultório (Peróxido de Hidrogênio 35% e Peróxido de Carbamida 37%), a partir de um ensaio clínico randomizado duplo cego. Os voluntários foram examinados para avaliar se poderiam participar da pesquisa, através dos critérios de inclusão e exclusão. Posteriormente foi realizada a randomização para definir a distribuição dos grupos de acordo com o agente clareador a ser aplicado. O operador do estudo foi cegado para o tipo de tratamento clareador até o momento de abertura dos envelopes. O avaliador foi treinado para os procedimentos de aferição da cor. Na primeira sessão de clareamento (T0), a cor inicial foi avaliada previamente a aplicação do agente clareador. Os protocolos foram realizados conforme a instrução dos fabricantes. Os agentes clareadores foram aplicados em 3 sessões (T0, T1 e T2) com um intervalo de 7 dias entre elas. No Grupo 1 (G1) foi utilizado o agente clareador a base de Peróxido de Hidrogênio 35%. No Grupo 2 (G2) foi utilizado o agente clareador a base de Peróxido de Carbamida 37%. Na análise objetiva com o espectofotômetro clínico, as coordenadas de cor CIEL*a*b* foram mensuradas três vezes no terço médio da face vestibular do incisivo central superior direito de cada paciente, utilizando a guia de posicionamento previamente confeccionada. O índice de brancura para odontologia (WID), a diferença de cor CIEDE 2000 (ΔE00) e a diferença de brancura (ΔWID) entre o T0 e cada período de avaliação foram calculados. Ao final do tratamento os valores médios de L*, a*, b* e WID para o grupo tratado com peróxido de carbamida a 37% foram de: L*= 89,12 (4.063), a*=-2,00 (0,689), b*= 19,1 (2,07) e WID= 29,0 (3,96). Ao final do tratamento os valores médios de L*, a*, b* e WID para o grupo tratado com peróxido de hidrogênio a 35% foram de: L*= 89,75 (3,23), a*=-2,35 (1.00), b*= 16,6 (3.16) e WID= 33,5 (4,87). Os dois protocolos foram capazes de promover o clareamento dental, sendo que o peróxido de hidrogênio a 35% apresentou excelente efetividade e o peróxido de carbamida apresentou muito boa effetividade. O peróxido de hidrogênio foi capaz de produzir uma maior brancura (WID) e uma maior redução no amarelo dos dentes (b*).
