This is a two-arm, non-blind, randomized controlled clinical trial. Experimental group: 35 patients with uncontrolled hypertension will undergo home blood pressure monitoring for 5 days in one week per month, for a period of three visits to the unit. Control group: 35 patients with uncontrolled arterial hypertension will be submitted only to measurement of arterial pressure in the office for a period of three visits to the unit. Randomization will be performed in a 1:1 ratio, using sealed envelopes containing previously generated numbers

Grupo experimental: 35 pacientes com hipertensão não controlada serão submetidos a monitorização residencial da pressão arterial durante 5 dias em uma semana por mês, por um periodo de três retornos na unidade. Grupo controle: 35 pacientes com hipertensão arterial não controlada serão submetidos somente a medição da pressão arterial no consultório por um periodo de três retornos na unidade. Patients diagnosed with hypertension associated or not with Diabetes Mellitus; Coronary Artery Disease or Chronic Renal Failure; aged over 18 years; diagnosis time greater than 6 months; blood pressure greater than or equal to 140/90 mm Hg obtained during initial clinical consultation; be following the same antihypertensive medication treatment for at least 4 weeks; present mobility conditions to attend follow-up appointments at the health unit

Pregnant women; women actively seeking to become pregnant; those with physical and/or cognitive disabilities; those who have undergone surgical procedures, suffered accidents and/or amputations; those undergoing oncological treatment that makes it impossible to self-monitor blood pressure; inability to maintain the monitor at home represented by the participant's refusal to receive and keep the device; acute cardiovascular event in the last three months

It is expected that individuals in the intervention group (IG), who undergo Home Blood Pressure Monitoring (HBPM), will present a significant reduction in mean systolic blood pressure (SBP) and diastolic blood pressure (DBP) values at the end of follow-up when compared with the control group (CG), which receives usual care only. The central hypothesis predicts a minimum reduction of 10 mmHg in SBP and 5 mmHg in DBP in the IG after the intervention period (three follow-up visits), with a statistically significant difference (p < 0.05) compared with the CG. This primary outcome was defined based on the study objectives and was used to calculate the sample size (n = 70).

A reduction of 16.71 mmHg in SBP (from 154.61 to 137.90 mmHg) and 12.47 mmHg in DBP (from 87.03 to 74.57 mmHg) was observed in the intervention group (IG), verified through the measurements obtained during the three follow-up visits. In the control group (CG), the reduction was 10.87 mmHg in SBP (from 152.67 to 141.80 mmHg) and 9.73 mmHg in DBP (from 88.67 to 78.94 mmHg) between the pre- and post-intervention periods. The difference between the groups was statistically significant (p < 0.001).

It is expected to find an improvement in medication adherence among participants in the Intervention Group (IG), who are submitted to Home Blood Pressure Monitoring (HBPM), verified through the medication adherence questionnaire administered during the third follow-up visit. An increase in adherence scores is expected in the post-intervention assessment compared with the pre-intervention period and with the Control Group (CG).

At baseline, 70 adult participants were randomly allocated into two groups: 31 in the Intervention Group (IG) and 39 in the Control Group (CG). The mean age was 60.98 ± 9.97 years. Overall, 64.29% were male and 75.71% had incomplete elementary education.
Regarding clinical characteristics, 30% were classified as pre-obese, 24.3% as grade II obesity, 41.43% had diabetes mellitus, and 2.86% had coronary artery disease. There were no statistically significant differences (p > 0.05) between groups, indicating homogeneity at baseline.pt-br
Na linha de base participaram do estudo 70 indivíduos adultos, alocados aleatoriamente em dois grupos: 31 no Grupo Intervenção (GI) e 39 no Grupo Controle (GC). A média de idade foi de 60,98 ± 9,97 anos. Do total, 64,29% eram do sexo masculino e 75,71% possuíam ensino fundamental incompleto.
Em relação às características clínicas, 30% apresentaram pré-obesidade, 24,3% obesidade grau II, 41,43% eram portadores de diabetes mellitus e 2,86% tinham doença arterial coronariana. Seventy participants were recruited and randomized between IG (n = 31) and CG (n = 39).
During follow-up, 43 participants completed the study.
In the CG, there were 19 losses: one death and 18 missed visits.
In the IG, there were eight losses: three due to absence, two withdrawals at the first visit, and three due to difficulty performing measurements.

No serious adverse events directly related to the intervention were reported.
Potential risks were considered low and comparable to usual care. Some participants experienced anxiety when obtaining high or low BP readings and were advised to seek emergency care if needed. One death occurred in the control group, unrelated to the intervention.pt-br

The primary outcome was the reduction of systolic (SBP) and diastolic (DBP) blood pressure over time.
Results demonstrated significant reductions in SBP and DBP means in the IG, particularly at the third follow-up visit (p < 0.001). The CG showed an initial reduction but later an increase in BP levels in subsequent visits.
Multiple regression analysis indicated that age significantly influenced SBP in the IG (p < 0.001), while time in the study affected DBP in the CG (p = 0.033).pt-br

O grupo intervenção realizou a monitorização residencial da pressão arterial (MRPA) por cinco dias consecutivos a cada mês, enquanto o grupo controle seguiu o acompanhamento usual.
A MRPA mostrou-se eficaz na redução dos níveis de pressão arterial, com diferenças significativas no grupo intervenção ao longo das consultas. O estudo reforça a aplicabilidade da MRPA como estratégia complementar no controle da hipertensão em atenção primária.
