This is a longitudinal, prospective study with a Non-Randomized Controlled Clinical Trial design. It is an unprecedented study, no protocol has been previously published and it was approved by the Ethics Committee for Research Involving Human Beings. This study was carried out between November 2021 and May 2022 in the Comprehensive Stroke Sector of a Public Hospital in Joinville/SC. The following criteria were adopted as inclusion criteria: individuals aged 18 years or over, of both sexes, capable of walking independently or with supervision, able to use an assistive device, diagnosed with stroke, clinically stable and agree to participate in the research by signing the Free and Informed Consent Form (TCLE). Exclusion criteria were established: individuals with severe cognitive deficit, severe aphasia, walking dependent on assistance from another person or unable to walk; severe heart disease and impossibility of telephone contact after hospital discharge. 58 individuals participated in the study, who were divided into two groups: the Experimental Group (EG) with 35 individuals and the Control Group (CG) with 23 individuals. The GE received a protocol associating the usual strategies within the Stroke Unit (U-Stroke), which involve encouraging walking 3 times a day and remaining active or sitting for longer periods of time, with an intervention protocol with educational activities, self-management and encouragement to reduce CS and increase PA during and after hospitalization. This protocol consists of: class on stroke risk factors and the importance of practicing PA, delivery of a booklet with PA suggestions to be carried out during the hospitalization period, balance and strengthening activities, availability of materials that were kept in the rooms to encourage mobility, such as cycle ergometer, bowling, dominoes, board games and the rehabilitation room to carry out social activities of the patient's choice. The CG received only the usual verbal instructions and interventions within the U-AVC. To monitor PA and CS levels before and after hospitalization, the International Physical Activity Questionnaire Short Version (IPAQ) was applied, where questions were asked regarding the PA performed by the patient during the last week and the individual was classified as Very active. , Active, Irregularly active A (one that meets at least one of the recommendation criteria regarding frequency or duration of the activity), Irregularly active B (one that does not meet any of the recommendation criteria regarding frequency or duration) and Sedentary. During hospitalization, the Behavioral Assessment Questionnaire (QAC) and the Questionnaire on Mobility Barriers (QBM), prepared by the researchers, were applied in the morning and afternoon during hospitalization with the aim of verifying the activities carried out during the periods (2 days during the week and 1 day on the weekend). In the QAC, some questions were asked, such as: whether the patient walked during the period, for how long, whether they spent most of the period lying, sitting or walking, whether they did any other activity such as: using a cell phone, watching TV, use the cycle ergometer, if you felt tired, among others. The QBM was performed when the patient did not perform any PA during the period, the question was “For what reason did you not perform PA during the period?” and the answer options were: I don't have a companion, I didn't have help, Tired, They banned me from walking down the corridor, I didn't feel like it, Other (description). Both groups received calls between 30 days and 3 months after hospital discharge to monitor PA levels and follow the instructions given during hospitalization.

Outcome found 1: The QAC demonstrated that during their hospital stay, 80% of the CG patients remained seated or in bed most of the day and reported walking for around 15 minutes during some periods, however, not daily. The other 20% reported walking daily and remaining seated most of the day. In the EG, 50% of individuals reported remaining seated or in bed for most of the day and 94% walked daily or performed some activity such as cycle ergometry for at least 15 minutes. The main barriers to performing PA found by the QAC during the hospitalization period were: tiredness (7.11%), cold (0.88%), intravenous device connected to the patient making active movement impossible (0.88%), performing exams (0.88%), waiting for a doctor's visit (0.44%), pain (0.44%), high blood pressure (0.44%). In the questionnaire on the practice of PA before and after the stroke, any activity that the patient performed, planned and structured, with the aim of improving or maintaining physical components, such as walking, gym, exercises at home or in the neighborhood square, was considered. , cycling, among others. The results indicated that before the stroke, the majority of patients in both groups (EG: 85.7% and CG 69.6%) did not practice physical activity. However, it was observed that after the stroke, the majority of patients started to perform physical activities (EG: 80.0% and CG: 56.5%). After 30 days to 3 months of hospital discharge, IPAQ demonstrated that the EG who received AE during hospitalization and specific health education against sedentary lifestyle, the number of individuals classified as sedentary or irregularly active A and B decreased in a statistically significant way (p=0.008). On the other hand, in the CG, although there was a reduction in the number of individuals classified as sedentary or irregularly active A and B, no significant difference was found (p=0.852).

Results summary:

Stroke is the second cause of death and disability in Brazil. It is common during hospitalization for patients to remain in sedentary behavior (CS), which may increase the risk of stroke recurrence and generate greater sequelae. Thus, the objective is to evaluate the feasibility and effectiveness of interventions to increase the level of physical activity (AF) and reduce CS after stroke. This is a longitudinal, prospective study, with a non-randomized Clinical Trial design, in which two groups were divided: the control group (CG) received verbal guidance and usual interventions within the AVC Unit (U-AVC) and the experimental group (GE) received an intervention protocol aimed at reducing CS and increasing AF during and after hospitalization. For data analysis, descriptive statistics (mean and standard deviation) and frequency distribution (absolute values and percentages) were used to characterize the participants, and Student's t test was used to compare pre- and post-discharge AF levels. 58 individuals participated in the study (GC: 23; EG: 35). Among our results, the Behavioral Assessment Questionnaire showed that during hospitalization, the GE remained more active most of the day compared to the GC. After discharge, the IPAQ showed that the GE had an increase in individuals classified as Very Active and Active. In conclusion, the application of an intervention protocol proved to be effective, as an increase in AF levels was observed in the GE, unlike the GC that maintained the CS after discharge.

Participaram do estudo 58 indivíduos (GC: 23; GE: 35).
