Interventions:

Infants allocated to Intervention Group 1 (IG1) will undergo the breastfeeding process before vaccination. Mothers will be asked, still in the private room and in a comfortable chair, to breastfeed the infant for 5 minutes before vaccination, a time that will be monitored using a stopwatch. It is emphasized that monitoring will start from the moment there is proof that the infant is sucking effectively, evaluated from the key points established by the Ministry of Health to determine the adequate grip: 1. More areola visible above the mouth of the baby; 2. Mouth wide open; 3. Lower lip turned out; 4. Chin touching breast. Swallowing (visible and/or audible) will also be identified (BRASIL, 2015). Then, mothers will be instructed to stop breastfeeding during the entire vaccination procedure. Infants allocated to Intervention Group 2 (IG2) will undergo the breastfeeding process before and during vaccination. Mothers will be asked, still in the private room and in a comfortable chair, to breastfeed the infant for 5 minutes before vaccination, a time that will be monitored using a stopwatch. Continuing, mothers will be instructed to discontinue breastfeeding only while the Human Rotavirus Vaccine (HRV) is administered and, immediately, restart breastfeeding throughout the Pentavalent vaccination procedure. Infants allocated to the Control Group (CG) will receive the usual care from the health service, and therefore, without carrying out the breastfeeding process before or before and during the vaccination procedure. For the sample size calculation, an effect size of 2.3 was used, considering the parameters: significance level or type I error of α = 0.05, with 1 – α / 2 = 1.96, type II error of β = 0.1, 1 – β = 0.90, effect size or d (µ 1 -µ 2 ) = 2.3, and standard deviation (S 1 =0.4, S 2 =1.6) based on in a previous study (KARIMI et al., 2022). Using the values described above, a sample size of n = 9 was obtained for each of the three groups (total = 27). However, a possible loss rate of 30% will be considered, due to previous agitation of the child, failures in the technique of vaccine administration, allergic reaction to the immunizer, or withdrawal of the participants. Therefore, the final total sample will be estimated at 36 participants. To form the groups, grouped randomization was applied via the website www.random.org, using the List Randomizer form, which allowed organizing the items of a list in random order (RANDOM, 2022). Thus, the Basic Health Units and the respective vaccine rooms had the same chance of participating in the IG and the CG, as the computerized algorithm defined which groups will receive the intervention (GI1 or GI2) and which will be the control (CG) (COELHO et al., 2021).

Lactentes alocados no Grupo Intervenção 1 (GI1) realizarão o processo de amamentação antes da vacinação. Solicitar-se-á para que as mães, ainda na sala privativa e em uma cadeira confortável, amamentem o lactente por 5 minutos antes da vacinação, tempo que será monitorado com uso de cronômetro. Enfatiza-se que o monitoramento iniciará a partir do momento em que houver a comprovação de que o lactente esteja sugando efetivamente, avaliado a partir dos ponto-chaves estabelecidos pelo Ministério da Saúde para determinação da pega adequada: 1. Mais aréola visível acima da boca do bebê; 2. Boca bem aberta; 3. Lábio inferior virado para fora; 4. Queixo tocando a mama. Também será identificada a deglutição (visível e/ou audível) (BRASIL, 2015). Em seguida, as mães serão orientadas a suspender a amamentação durante todo o procedimento da vacinação. Os lactentes alocados no Grupo Intervenção 2 (GI2) realizarão o processo de amamentação antes e durante a vacinação. Solicitar-se-á -para que as mães, ainda na sala privativa e em uma cadeira confortável, amamentem o lactente por 5 minutos antes da vacinação, tempo que será monitorado com uso de cronômetro. Dando continuidade, as mães serão orientadas a suspender a amamentação somente enquanto é administrada a Vacina Rotavírus Humana (VRH) e, imediatamente, reiniciar a amamentação durante todo o procedimento da vacinação da Pentavalente. Os lactentes alocados no Grupo Controle (GC) receberão o cuidado habitual do serviço de saúde, e, portanto, sem realização do processo de amamentação antes ou antes e durante o procedimento de vacinação. Para o cálculo amostral, foi empregado o tamanho do efeito de 2,3, considerando os parâmetros: nível de significância ou erro tipo I de α = 0,05, com 1 – α / 2 = 1,96, erro tipo II de β = 0,1, 1 – β = 0,90, tamanho do efeito ou d (µ 1 -µ 2 ) = 2,3, e desvio padrão (S 1 =0,4, S 2 =1,6) com base em estudo anterior (KARIMI et al., 2022). Utilizando-se os valores descritos acima, obteve-se um tamanho de amostra de n = 9 para cada um dos três grupos (ntotal = 27). No entanto, será considerada possível taxa de perda de 30%, decorrente de agitação prévia da criança, falhas na técnica de administração das vacinas, reação alérgica ao imunizante, ou desistência das participantes. Portanto, a amostra final total será estimada em 36 participantes. Para constituição dos grupos, aplicou-se a randomização agrupada via site www.random.org, por meio do formulário List Randomizer que permitiu organizar os itens de uma lista em ordem aleatória (RANDOM, 2022). Assim, as Unidades Básicas de Saúde e as respectivas salas de vacina tiveram a mesma chance de participar dos GI e do GC, pois o algoritmo computadorizado definiu quais grupos receberão a intervenção (GI1 ou GI2) e qual será o controle (GC) (COELHO et al., 2021).

Ninety-five mother-infant dyads were eligible for evalua tion. Five of them did not meet the inclusion criteria and thus ninety were randomized into three groups: breastfeeding before
vaccination (30 participants), breastfeeding before and during vaccination (30 participants) and control (30 participants). The lowchart for tracking participants included in the study is
shown in Figure 1. There was no statistically significant difference between the intervention and control groups in terms of socioeconomic characteristics age group, ethnicity, education, marital status, family income, and performing paid work (p > 0.05); similarly, the obstetric profile did not differ significantly between the groups regarding gestational age, prenatal consultation, breastfeeding guidance, pregnancy complications, and type of delivery (p > 0.05). The groups were also homogeneous regarding aspects related to the breastfeeding process: whether the infant was breastfed immediately after birth and whether there was
skin-to-skin contact at birth (p > 0.05).pt-br

Destes, 05 não atenderam aos critérios de inclusão, sendo 90 aleatorizados em três grupos: amamentação antes da vacinação (30 participantes), amamentação antes e durante a vacinação (30 participantes) e controle (30 participantes). A total of 95 participants were assessed for eligibility, of whom 5 were excluded due to not meeting inclusion criteria (n=4) or refusal to participate (n=1). Therefore, 90 participants were enrolled and randomized.
Participants were allocated into three groups (n=30 per group): control group, intervention group 1 (breastfeeding five minutes before and during vaccination), and intervention group 2 (breastfeeding five minutes before vaccination).
Participants in the control group received standard care without additional intervention. All participants allocated to intervention groups received the assigned interventions as planned.
There were no losses to follow-up or discontinuation of interventions in any group.
All randomized participants were included in the final analysis (n=90), with no exclusions.pt-br
Foram avaliados para elegibilidade 95 participantes, dos quais 5 foram excluídos por não atenderem aos critérios de inclusão (n=4) ou por desistência (n=1). Assim, 90 participantes foram incluídos e randomizados.
Os participantes foram alocados em três grupos (n=30 por grupo): grupo controle, grupo intervenção 1 (amamentação cinco minutos antes e durante a vacinação) e grupo intervenção 2 (amamentação cinco minutos antes da vacinação).

Adverse events:

Among the infants breastfed during vaccination, none presented complications, such as choking, coughing, aspiration, or cyanosis. Only one child in IG1 regurgitated after administration of the human rotavirus vaccine, a condition that may be asso ciated with the vaccine itself. The belief in the possibility of these complications, often verbalized by nursing professionals, is limiting and can have a negative impact by discouraging breastfeeding during vaccination to relieve pain in newborns and infants. In a systematic review of the Cochrane database, none of the included studies reported complications related to
breastfeeding during invasive procedures, thus suggesting that there is no risk of adverse effects such as those mentioned.pt-br
Ressalta-se que, dentre os lactentes amamentados durante o procedimento de vacinação, nenhum apresentou complicações, como engasgo, tosse, aspiração ou cianose. Apenas uma criança do GI1 regurgitou após a administração da Vacina Rotavírus Humana, condição que pode estar associada à própria vacina. Objective: To analyze the effect of breastfeeding on reducing Pentavalent vaccination pain
in infants and to identify the necessary breastfeeding interval for antinociceptive action.
Method: Open parallel randomized clinical trial. Ninety mother-infant dyads participated, distributed into intervention group 1 (n = 30), which breastfed five minutes before vaccination;
intervention group 2 (n = 30), which breastfed five minutes before and during vaccination;
and control group (n = 30), which did not breastfeed. The outcome variable was the pain level
measured by the Face, Legs, Activity, Cry, and Consolability (FLACC) Scale. Data analysis was conducted using descriptive and inferential statistics, applying Fisher’s Exact, Kolmogorov-Smirnov, Kruskal-Wallis and Dunn’s multiple comparison tests, with 0.05 significance level. Results: Pain induced by the Pentavalent vaccine was reduced in intervention groups 1 and 2 (mean pain of 6.06 versus 3.83, respectively) compared to the control group (mean of pain of 7.43), which was significant for intervention group 2 (p < 0.001), indicating that, to achieve lower levels of pain, breastfeeding should be carried out before and during vaccination. Conclusion: Longer breastfeeding, conducted five minutes before and during vaccination, reduces the pain induced by the Pentavalent vaccine. No vaccination risks were identified to outweigh the benefits. These results endorse that health professionals should encourage breastfeeding at least five minutes before and during vaccine injection for an antinociception effect. Objetivo: Analisar o efeito do aleitamento materno na redução da dor da vacinação pentavalente em lactentes e identificar o intervalo de amamentação necessário para a ação antinociceptiva. Método: Ensaio clínico randomizado paralelo aberto. Noventa díades mãe-lactente participaram, distribuídas em grupo de intervenção 1 (n = 30), que amamentaram cinco minutos antes da vacinação; grupo de intervenção 2 (n = 30), que amamentaram cinco minutos antes e durante a vacinação; e grupo controle (n = 30), que não amamentaram. A variável de desfecho foi o nível de dor medido pela Escala Face, Legs, Activity, Cry, and Consolability (FLACC). A análise dos dados foi realizada utilizando estatística descritiva e inferencial, aplicando os testes exato de Fisher, Kolmogorov-Smirnov, Kruskal-Wallis e de comparações múltiplas de Dunn, com nível de significância de 0,05. Resultados: A dor induzida pela vacina pentavalente foi reduzida nos grupos de intervenção 1 e 2 (dor média de 6,06 versus 3,83, respectivamente) em omparação com o grupo controle (dor média de 7,43), sendo essa redução significativa para o grupo de intervenção 2 (p < 0,001), indicando que, para alcançar níveis mais baixos de dor, o aleitamento materno deve ser realizado antes e durante a vacinação. Conclusão: O aleitamento materno prolongado, realizado cinco minutos antes e durante a vacinação, reduz a dor induzida pela vacina pentavalente.
Não foram identificados riscos da vacinação que superassem os benefícios. Esses resultados reforçam a importância de os profissionais de saúde incentivarem o aleitamento materno por pelo menos cinco minutos antes e durante a injeção da vacina para um efeito antinociceptivo.
