This is a double-blind, double-blind, randomized control trial. Control group: 10 patients of both sexes, admitted to an intensive care unit, on invasive mechanical ventilation, received oral hygiene by the nursing team with 0.12% chlorhexidine, 3 times a day, according to hospital protocol. Intervention: 9 patients of both sexes, admitted to an intensive care unit, on invasive mechanical ventilation, received dental follow-up every 24 or 48 hours, characterized by removal and treatment of the focus of infection, adequacy of oral and professional prophylaxis, tooth brushing, stone removal, extractions as needed by each patient, every 24 or 48 hours, in addition to oral hygiene performed by the nursing team with 0.12% chlorhexidine, according to hospital protocol 3x a day.

Trata-se de um estudo clínico randomizado controlado de dois braços, duplo-cego. Grupo controle: 10 pacientes de ambos os sexos, internados em uma unidade de terapia intensiva, em ventilação mecânica invasiva, receberam higiene oral pela equipe de enfermagem com clorexidina 0,12% com frequência de 3 vezes ao dia conforme protocolo do hospital. Expected outcome 1: Patients who remain longer on mechanical ventilation are expected to suffer dysbiosis of the oral microbiome, increasing the risk of nosocomial infections. The diagnosis of the alteration of the oral microbiome will be carried out through the method of genetic sequencing of the oral biofilm. The analysis of the microbial community will be through the software QIIme (Quantitative Insights Into Microbial Ecology). The metagenomic sequences will be compared with already known sequences that are registered in national and international databases (reference sequences) to identify resistance genes and/or mutations. This allows us to compare OTUs (Operational Taxonomic Units) from different experiments.

Desfecho esperado 1: Espera-se que pacientes que permanecem mais tempo em ventilação mecânica, sofrem disbiose do microbioma oral, aumentando os riscos de infecções nosocomiais. O diagnóstico da alteração do microbioma oral será realizado através do método de sequenciamento genético do biofilme oral. A análise da comunidade microbiana será através do software QIIme (Quantitative Insights Into Microbial Ecology). As sequências metagenômicas serão comparadas com sequências já conhecidas que estão cadastradas em bancos de dados nacionais e internacionais (sequências de referência) para identificar genes e/ou mutações de resistência. Isso nos permite comparar OTUs (unidades taxonômicas operacionais) de experimentos diferentes.

Endpoint found 1: The analyzes of oral saliva samples from patients hospitalized in the intensive care unit of our study did not show a statistically significant difference in microbial communities between the groups, however the follow-up group showed a reduction in a richness index after the third day, which demonstrates that there was a negative effect on the microbial community in the patients receiving follow-up.

Of the 19 participants, 16 (84.2%) had comorbidities, none was an alcoholic, one is a smoker (5.3%), during the stay, four (21.1%) had oral, 10 (52.6%) had mucosal injury, five (26.3%) had oral bleeding, eight (42.1%) had normal salivation, nine (47.4%) had hyposalivation and two (10.5%) had hypersalivation. Regarding dentition, nine (47.4%) were completely edentulous, three (15.8%) with up to 10 teeth, one (5.3%) between 11 and 20 teeth and six people (31.6%) with more than 20 teeth.

Inclusion criteria were: patients admitted to the Intensive Care Unit, on mechanical ventilation, aged ≥18 years. Exclusion criteria were: patients who had a current diagnosis and/or salivary gland disorders (Sjogren's syndrome, salivary gland tumor, parotitis), cancer patients regardless of the stage of the disease, psychiatric patients, postoperatively with discharge within the first 24 hours , patients admitted from other institutions already undergoing mechanical ventilation and oral and maxillofacial trauma.
Patients who met the inclusion criteria were randomly divided into two groups. The control group (Ctrl) received oral care according to the institution's standard operating protocol, which is characterized by oral hygiene three times a day, performed by the nursing team with a brush and/or suction swab and 0.12% chlorhexidine solution. And a follow-up group (Acomp) which, in addition to the oral hygiene mentioned above, dental follow-up, characterized by removal and treatment of the focus of infection, adaptation of the oral environment and professional prophylaxis, tooth brushing, stone removal, extractions according to the needs of each patient, every 24 or 48 hours. It is noteworthy that this follow-up was performed by a single dental surgeon, who is the main researcher of this study.
A total of 108 patients on invasive mechanical ventilation, of which 40 were excluded due to diagnosis, 20 due to early extubation, 19 transferred from other institutions already on mechanical ventilation and 7 died before the initial collection. In total, 22 patients were chosen for the research. 11 patients in the follow-up group, where two patients died after the first sample collection, totaling 9 patients in the follow-up group. 11 patients in the control group who lost one patient due to death after the first collection, ending with n out of 10 patients in the group.pt-br
Os critérios de inclusão foram: pacientes admitidos na Unidade de Terapia Intensiva, em ventilação mecânica, com idade ≥18 anos. Os critérios de exclusão foram: pacientes que apresentavam diagnóstico atual e/ou distúrbios em glândulas salivares (Síndrome de Sjogren, tumor em glândulas salivares, parotidite), pacientes oncológicos independentemente do estágio da doença, psiquiátricos, pós-operatório com alta nas primeiras 24 horas, pacientes admitidos de outras instituições já em ventilação mecânica e trauma bucomaxilofacial.
Os pacientes que atenderam os critérios de inclusão foram divididos em dois grupos através de sorteio. O grupo controle (Ctrl) recebeu cuidados orais conforme protocolo operacional padrão da instituição, que é caracterizado pela higiene oral três vezes ao dia, realizada pela equipe de enfermagem com escova e/ou swab de sucção e solução de clorexidina 0,12%. E um grupo acompanhamento (Acomp) que, além da higiene oral citada anteriormente, acompanhamento odontológico, caracterizado por remoção e tratamento de foco de infecção, adequação de meio bucal e profilaxia profissional, escovação dos dentes, remoção de cálculos, exodontias conforme necessidade de cada paciente, a cada 24 ou 48 horas.. Ressalta-se que este acompanhamento foi realizado por um único profissional cirurgião-dentista, que é a pesquisadora principal deste estudo.
Total de 108 pacientes em ventilação mecânica invasiva, sendo que 40 foram excluídos devido diagnóstico, 20 por extubação precoce, 19 transferidos de outras instituições já em ventilação mecânica e 7 foram a óbito antes da coleta inicial. No total foram 22 paciente eleitos para a pesquisa. 11 pacientes no grupo acompanhamento, onde dois pacientes foram a óbito após a primeira coleta de amostra, totalizando 9 pacientes no grupo acompanhamento. 11 pacientes no grupo controle que apresentou perda de um paciente por óbito após a primeira coleta, finalizando com n de 10 pacientes no grupo. -

There was a statistically significant difference (p<0.00) in the number of days of orotracheal tube when comparing the follow-up and control groups. Eleven patients (57.9%) developed nosocomial infection during ICU stay, of which one (9.1%) patient had a bloodstream infection, five had ventilator-associated pneumonia (45.4%), two had aspiration pneumonia. (18.2%), two with urinary tract infection (18.2%) and one with surgical site infection (9.1%).
It was possible to determine 581 operational taxonomic units (OTUs), of which 189 were observed exclusively in the follow-up group and 256 in the control group.
The Shannon richness index, on the other hand, indicates the uniformity of species in the sample and showed a statistically significant difference between the control and follow-up groups (p=<0.05), that is, there is a decrease in diversity in the follow-up group between collections 1 and 3, which is not observed in the Control group.pt-br
Observou diferença estatisticamente significativa (p<0,00) no número de dias de tubo orotraqueal quando comparado os grupos acompanhamento e controle. Onze pacientes (57,9%) desenvolveram infecção hospitalar durante a internação na UTI, das quais um (9,1%) paciente com infecção de corrente sanguínea, cinco com pneumonia associada à ventilação (45,4%), dois com pneumonia aspirativa (18,2%), dois com infecção do trato urinário (18,2%) e um com infecção de sítio cirúrgico (9,1%).
Foi possível determinar 581 unidades taxonômicas operacionais (OTUs), destes 189 foram observados exclusivamente no grupo de acompanhamento e 256 no grupo controle.
Já o índice de riqueza de Shannon indica a uniformidade de espécies na amostra e apresentou diferença estatisticamente significativa entre os grupos controle e acompanhamento (p=<0,05), ou seja, há uma diminuição da diversidade no grupo acompanhamento entre as coletas 1 e 3, o que não é observado no grupo Controle.

Introduction: Critical illness is associated with a state of catabolic stress, leading to dysbiosis in which patients become more susceptible to nosocomial infections, bringing a significant impact on health. The oral cavity is a complex environment and offers several distinct habitats for colonization, being considered an ideal microbial incubator. Objective: Evaluating the interference of dental treatment in the modulation of the oral bacterial microbiome of critically ill patients. Method: Patients were divided into two groups, the control group (n = 10) received oral hygiene according to the institution's protocol performed by the nursing team, while the monitoring group (n = 9) received dental care, in addition to oral hygiene care of nursing. Three samples of biological material were collected from each patient every 72 hours, then DNA extraction was performed and the bacteria present in the samples were identified based on the DNA sequencing of the V3-V4 regions of the ribosomal16S RNA (16SrRNA) gene. The identification of Operational Taxonomic Units (OTUs) were evaluated using the FastQC software (version 0.11.5) and a comparative analysis of the microbiome community was performed. Results: It was observed that the monitoring group showed a 45.4% reduction in the nosocomial infection rate (p <0.00) and the number of days of the orotracheal tube when compared to the control group. Differential abundance analysis shows that species such as Prevotella melaninogenica (p = 0.023), Peptrosteptococcus stomatis (p = 0.034), Staphylococcus epidermidis (p = 0.016) and Vellionella dispar (p = 0.032) were more abundant in the control group samples. The monitoring group had fewer specific OTUs than the control group, indicating an improvement in the modulation of the oral microbiota. Conclusion: The analysis of the oral saliva samples showed no statistically significant difference in the microbial communities between the groups, however the monitoring group showed a reduction in a wealth index after the third day, which demonstrates that there was a negative effect on the microbial community in the patients receiving follow-up showing that dental treatment improves the oral condition of critically ill patients by controlling the presence of nosocomial microorganisms such as Klebsiella sp, E. coli, Serratiae sp., S. epidermidis. And the metagenome directly assists in the characterization of the microbiome and rapid identification of pathogens allowing customized pharmaceutical applications.

Introdução: A doença crítica está associada a um estado de estresse catabólico, levando a disbiose no qual os pacientes se tornam mais suscetíveis a infecções nosocomiais, trazendo um impacto significativo para a saúde. A cavidade oral é um ambiente complexo e oferece vários habitats distintos para colonização, sendo considerada um incubador microbiano ideal. Objetivo: Avaliar a interferência do tratamento odontológico na modulação do microbioma bacteriano oral de pacientes críticos. Método: Os pacientes foram divididos em dois grupos, o grupo controle (n=10) recebeu higiene oral conforme protocolo da instituição realizado pela equipe de enfermagem, enquanto o grupo acompanhamento (n=9) recebeu atendimento odontológico, além dos cuidados de higiene oral da enfermagem. Foram coletadas três amostras de material biológico de cada paciente a cada 72 horas, em seguida foi realizada extração do DNA e as bactérias presentes nas amostras foram identificadas com base no sequenciamento de DNA das regiões V3-V4 do gene do RNA ribossomal16S (16SrRNA). A identificação das Unidades Taxonômicas Operacionais (OTUs) foram avaliados utilizando o software FastQC (versão 0.11.5) e realizado a análise comparativa da comunidade microbioma. Resultados: Observou-se que o grupo acompanhamento apresentou uma redução no índice de infecção nosocomial de 45,4% (p<0,00) e no número de dias de tubo orotraqueal quando comparado ao grupo controle. A análise de abundância diferencial demonstra que espécies como Provetella melaninogenica (p=0.023), Peptrosteptococcus stomatis (p=0.034), Staphylococcus epidermidis (p=0.016) e Vellionella dispar (p=0.032) foram mais abundantes nas amostras do grupo controle.
