Interventions:

Study Group: 33 patients admitted to the pediatric intensive care unit and using a mechanical ventilator will undergo an assessment of the degree of sedation using the COMFORT BEHAVIOR scale and will receive physical therapy assistance three times a day. Control Group: 33 patients hospitalized in the pediatric intensive care unit and using a mechanical ventilator will receive sedation adjustment through the evaluation of the medical team and will receive physical therapy assistance three times a day. Patients will be randomly divided into groups.

Grupo Estudo: 33 pacientes internados na unidade de terapia intensiva pediátrica e em uso de ventilador mecânico serão submetidos a avaliação do grau de sedação através da escala COMFORT BEHAVIOR e receberão assistência fisioterapêutica três vezes ao dia. Grupo Controle: 33 pacientes internados na unidade de terapia intensiva pediátrica e em uso de ventilador mecânico receberão ajuste de sedação através da avaliação da equipe médica e receberão assistência fisioterapêutica três vezes ao dia. Os pacientes serão divididos de forma aleatória para os grupos.

It is expected to carry out an analysis of the scores of the COMFORT - BEHAVIOR scale during the weaning period and if it can be considered a predictor at the time of extubation and to compare between the groups which showed the best extubation success. Providing relevant contributions to care, care and research, in the health care setting, providing a better approach for these individuals. However, the team will be trained in order to minimize risks and discomfort.

Introduction: Pediatric patients admitted to the intensive care unit usually progress to acute pulmonary respiratory failure and require ventilatory support. Due to the invasive procedures to which the child is submitted during the period of hospitalization, the use of sedation is necessary to control pain and synchronize patient ventilator. The COMFORT - BEHAVIOR scale is widely used within the pediatric environment to adjust sedation and synchronize with invasive mechanical ventilation. Objective: To analyze the applicability of the COMFORT-BEHAVIOR scale as a weaning predictor correlating the score with the success of extubation and time on mechanical ventilation. Methodology: This is a randomized, blinded clinical trial in individuals admitted to the pediatric intensive care unit in a hospital in the south of Minas Gerais. The subjects previously selected according to inclusion and exclusion criteria will be randomized into two groups: Experimental group - application of the COMFORT - BEHAVIOR scale will be carried out by trained and standardized physiotherapists in the sector before physical therapy, before and after the moment of extubation of the patient and Group Control - the sedation adjustment will be made by the medical evaluation. Both groups will receive physical therapy three times a day. Expected Results: It is expected to carry out an analysis of the scores of the COMFORT - BEHAVIOR scale during the weaning period and if it can be considered a predictor at the time of extubation and to compare between the groups which showed the best extubation success. Introdução: Pacientes pediátricos internados em unidade de terapia intensiva geralmente evoluem para insuficiência respiratória pulmonar aguda e necessitam de suporte ventilatório. Devido aos procedimentos invasivos no qual a criança é submetida durante o período da internação faz se necessário a utilização de sedação para controle da dor e sincronia paciente ventilador. A escala COMFORT – BEHAVIOR é muito utilizada dentro do ambiente pediátrico para o ajuste de sedação e sincronia com a ventilação mecânica invasiva. Objetivo: Analisar a aplicabilidade da escala COMFORT-BEHAVIOR como preditor de desmame correlacionando o score com o sucesso de extubação e tempo de ventilação mecânica. Metodologia: Trata-se de um ensaio clínico randomizado, cego, em indivíduos internados na unidade de terapia intensiva pediátrica em um hospital do Sul de Minas Gerais. Os sujeitos previamente selecionados segundo critérios de inclusão e exclusão serão randomizados em dois grupos: Grupo experimental- será feito aplicação da escala COMFORT – BEHAVIOR pelos fisioterapeutas do setor treinados e padronizados antes do atendimento fisioterapêutico, antes e após o momento da extubação do paciente e Grupo Controle- o ajuste de sedação será feito pela avaliação médica. Ambos grupos receberão atendimento fisioterapêutico três vezes ao dia. Resultados Esperados: Espera-se realizar uma análise dos scores da escala COMFORT – BEHAVIOR durante o período de desmame e se ela pode ser considerada um preditor no momento da extubação e comparar entre os grupos quais apresentaram melhor sucesso de extubação. Proporcionando contribuições relevantes à assistência, ao cuidado e às pesquisas, no cenário de atenção à saúde, proporcionando uma melhor abordagem destes indivíduos.
